Effectiveness and acceptability of a 24-h interval versus a 48-h interval between mifepristone intake and misoprostol administration for in-hospital abortion at 9-20 gestational weeks: an international, open-label, randomised, controlled, non-inferiority trial.

BACKGROUND: Optimising management of second-trimester medical abortion is important, as complications increase with gestational age. We aimed to compare a 24-h interval with a 48-h interval between mifepristone intake and misoprostol administration in in-hospital, second-trimester medical abortion for effectiveness and acceptability. METHODS: This open-label, randomised, controlled, non-inferiority trial was conducted at nine hospitals in India, Sweden, Thailand, and Viet Nam among adults undergoing medical abortion for a singleton viable pregnancy at a gestation of between 9 weeks and 20 wee

Delayed administration of epinephrine is associated with worse neurological outcomes in patients with out-of-hospital cardiac arrest and initial pulseless electrical activity: insight from the nationwide multicentre observational JAAM-OHCA (Japan Association for Acute Medicine) registry.

AIMS: The delayed administration of epinephrine has been proven to worsen the neurological outcomes of patients with out-of-hospital cardiac arrest (OHCA) and shockable rhythm or asystole. We aimed to investigate whether the delayed administration of epinephrine might also worsen the neurological outcomes of patients with witnessed OHCA and initial pulseless electrical activity (PEA). METHODS AND RESULTS: The JAAM-OHCA Registry is a multicentre registry including OHCA patients between 2014 and 2017. Patients with emergency medical services (EMS)-treated OHCA and initial PEA rhythm were includ

Methodological progress note: Applying co-production methods in hospital medicine research to increase impact.

Research co-production-engaging key partners such as patients, families, clinicians, hospital administrators, and/or policymakers early and throughout the research process-enhances research impact. Through authentic, meaningful, and sustained partnerships, research co-production helps generate evidence that is relevant and useful for patients, clinical practice, policy, and the healthcare system. This methodological progress note introduces the concept of research co-production and provides practical strategies and real-world examples to guide hospitalists in applying this approach to accelera…

Salary compression of nursing managers.

An American Hospital Association survey has shown that rapid escalation of staff nurse salaries and benefits, in the wake of local and national shortages, has pushed clinical wage structures beyond those that determine the salaries of nurse managers and administrators. This leads to difficulty both in recruitment and retention of these key personnel. In this hospital, attrition of nurse managers into clinical positions has ceased. Their approach makes nurse managers eligible for differentials in addition to a modest economic incentive for assumption of managerial responsibilities.

Assessing sexual assault survivors' access to emergency contraception: results from a mixed methods study in South Carolina.

BACKGROUND: Provision of emergency contraceptive pills (ECPs) is widely recognized as the standard of care to prevent pregnancy after a sexual assault. However, previous research has shown that hospitals do not routinely counsel sexual assault patients about or provide sexual assault survivors with ECPs or accurate referrals. METHODS: We undertook a mixed methods study to assess policies and practices regarding the provision of ECPs for sexual assault survivors in South Carolina. The study includes four components: An analysis of the South Carolina Victims' Rights Amendment, in-depth intervie

Time out for surgeons: when is the attending surgeon too tired?

Pediatric surgical subspecialty workforce shortages are here to stay without any expected solution for the short-term. Individual surgeons, hospital administrators, risk management and patient-safety teams need to recognize that patient safety must take precedence over clinical productivity and financial "bottom lines." Pushing attending surgeon work hours beyond the limits of exhaustion impairs patient safety. Just as resident surgeon work hours have been appropriately curtailed in the name of patient safety, so must attending surgeon work hours. This issue needs to be addressed by hospital p…

Radiation protection for nurses. Regulations and guidelines.

Rules and regulations of federal agencies and state radiation protection programs provide the bases for hospital policy regarding radiation safety for nurses. Nursing administrators should work with the radiation safety officer at their institutions to ensure that radiation exposures to staff nurses will be as low as reasonably achievable and that special consideration will be given to pregnant nurses. Nurses' fears about their exposure to radiation can be greatly reduced through education.

[Health policy considerations on the AIDS problems and risks in surgery].

Many physicians are afraid of contracting human immunodeficiency virus (HIV) infection from their patients, especially surgeons. In medieval times, it was common for physicians to abandon their patients during epidemics (particularly plague) and to leave the cities. Today, strict hygienic practice protects medical personnel from HIV infection. Routine HIV screening of patients and medical personnel is thus not necessary. Instead, those involved (patients, medical personnel, hospital administrators, policy makers) should act responsibly.

The Role of a Certified Registered Nurse Anesthetist Led Acute Pain Service in Preventing Persistent Postoperative Opioid Use.

Given the present opioid crisis, the use of opioids in the hospital setting is an increasing concern among hospital administrators and healthcare professionals. A serious problem related to surgical care is persistent postoperative opioid use among previously opioid-naïve patients. Certified registered nurse anesthetists (CRNAs) are strategically positioned within the hospital setting to address these concerns. These individuals are actively involved in managing the pain of their patients and can therefore lead change in relation to the opioid crisis. This article profiles a multidisciplinary …

Care for hospitalized patients with unhealthy alcohol use: a narrative review.

There is increasing emphasis on screening, brief intervention, and referral to treatment (SBIRT) for unhealthy alcohol use in the general hospital, as highlighted by new Joint Commission recommendations on SBIRT. However, the evidence supporting this approach is not as robust relative to primary care settings. This review is targeted to hospital-based clinicians and administrators who are responsible for generally ensuring the provision of high quality care to patients presenting with a myriad of conditions, one of which is unhealthy alcohol use. The review summarizes the major issues involved
