[The Extended Deltoid-Split Approach for Plating Four-Part Proximal Humeral Fractures].

The deltoideopectoral approach is established as the gold standard in the surgical treatment of proximal humeral fractures. As an alternative, we demonstrate the extended deltoid approach with an intraoperative video. A direct lateral incision is performed and the anterior parts of the axillary nerve are identified and preserved. In our experience, this approach allows improved visualisation of the greater tuberosity and easier positioning of locking plates. Clinically relevant neurological injuries cannot be seen in our patients or in the literature.

Biomechanical Analysis of Screws Versus K-Wires for Lateral Humeral Condyle Fractures.

BACKGROUND: Good outcomes have been described for pediatric lateral condyle fractures treated by open reduction and fixation using either screws or Kirschner wires (K-wires). No studies have compared the biomechanical properties of the 2 fixation methods. We hypothesized that screw fixation would be more biomechanically stable than K-wire fixation. METHODS: Synthetic humerus models were used for biomechanical testing, following a previously published protocol. A miter saw was used to make an oblique cut to simulate a Milch type II fracture. Fractures were anatomically reduced and fixed with e

Long-term Functional Results of Neurological Complications of Pediatric Humeral Supracondylar Fractures.

PURPOSE: It has been reported that most neurological injuries after sustaining a supracondylar fracture will usually resolve spontaneously in the first months; however, studies are lacking regarding the influence of these injuries in the upper limb functionality in the long-term follow-up. The aim of this study is to report the long-term functional outcome of neurological complications associated to pediatric supracondylar fractures. METHODS: Medical records of 448 children who sustained a supracondylar humeral fracture were reviewed. Patients with a concomitant neurological injury were inclu

The impact of arterial vessel injuries associated with pediatric supracondylar humeral fractures.

BACKGROUND: Pediatric supracondylar humeral fractures are regularly complicated by vascular compromise. However, the treatment regimen for pulseless hands with good capillary refill time is discussed controversially. The aim of the present study was to present our treatment strategy in a series of children sustaining supracondylar humeral fractures associated with impaired peripheral blood flow. METHODS: Medical charts of all children sustaining supracondylar humeral fractures treated between 1995 and 2009 were reviewed. Treatment and the course of patients with impaired peripheral blood flow

Mid-term outcome of reverse shoulder prostheses in complex proximal humeral fractures.

The functional outcome of hemiarthroplasty in displaced proximal humeral 3- and 4-part fractures or fracture dislocations in elderly patients is frequently unpredictable and depends on the position of the prosthesis and tuberosity fixation. Reverse shoulder arthroplasty represents an alternative in elderly patients. The purpose of this study was to report the results of a retrospective series of 30 reverse shoulder prostheses in trauma indications. We also compared the results of a less medialized reverse shoulder prosthesis (Arrow) with those reported for the traditional (Delta III) reverse p

[Lateral humeral condyle fractures in children call for follow-up X-ray].

Lateral humeral condyle fractures are among the most common elbow fractures in children (20%), often in children aged six to ten years. X-rays of children this age are hard to interpret, and a seemingly undislocated fracture can still be unstable. Failure to recognise this problem can lead to the wrong treatment, non-union and malfunction of the elbow. An 11-year-old boy fell on his left arm. A plaster cast was administered and worn for four weeks without control X-ray. Four months later the boy had an extension defect of ten degrees, and a control computed tomography confirmed non-union.

A simple technique for double plating of extraarticular distal humeral shaft fractures.

Plate fixation of extraarticular distal humeral shaft fracture is often difficult. Traditional techniques do not allow for stable fixation. A single DCP plate may have inadequate purchase in the distal fragment. The use of large plates extending distally over the lateral supracondylar ridge is often associated with pain and sometimes interferes with elbow range of motion. In this study, 22 patients with extra articular distal humeral fractures were managed with dual plating using a paratricipital approach. The first plate--a narrow DCP--was fixed on the dorsal surface of the humerus. The secon

Surgical versus non-surgical interventions for treating humeral shaft fractures in adults.

BACKGROUND: Fractures of the shaft of the humerus account for 1% to 3% of all fractures in adults. The management of these fractures, including surgical intervention, varies widely. OBJECTIVES: To assess and compare the effects of surgical versus non-surgical intervention for non-pathological fractures of the humeral shaft in adults. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, trial registers, and bibliographies of trial reports. The full search was conducted in October

Functional and radiographic long-term outcomes of hemiarthroplasty for proximal humeral fractures.

BACKGROUND: Hemiarthroplasty is the preferred surgical procedure for 3- and 4-part proximal humeral fractures. Our retrospective study examined functional and radiographic outcomes at 2- and 5-year follow-up in patients who received a hemiarthroplasty for 3- and 4-part proximal humeral fractures. MATERIALS AND METHODS: Forty-seven consecutive patients of a single surgeon treated with a cemented shoulder hemiarthroplasty between 2000 and 2006 were followed up. Standard surgical technique was used in all cases. Postoperative radiographs were compared with radiographs at 2 and 5 years along with

Treatment of pathological humeral shaft fractures with intramedullary nailing. A retrospective study.

A consecutive series of 22 pathological fractures of the humeral shaft in 21 patients treated at one institution were included in this retrospective study. Patients were treated with anterograde locked intramedullary nailing. Mean follow-up was 22.7 months (range 3-60). Mean VAS score improved from 89.5 (range 80-100) to 14.5 (range 0-40). In most patients there was a satisfactory return to daily activities within six weeks of surgery. Seventeen of 19 patients reported to be satisfied. Mean duration of hospitalization after surgery was 4.3 days (range 2-15). There were no complications related

Operative treatment of humeral shaft fractures. Comparison of plating and intramedullary nailing.

Plate and screw fixation (PSF) has always been the more common surgical treatment of humeral shaft fractures. However, intramedullary nailing (IMN) of the humerus has gained in popularity over the last two decades. The purpose of this retrospective study was to evaluate the clinical outcome of plate fixation versus intramedullary nailing of midshaft humeral fractures. The study included 91 patients treated at the department of Trauma Surgery of the University hospital of Leuven; 42 fractures had been treated by plate fixation and 49 by IM nailing. Union, functional outcome, possible complicati

Postoperative management after open reduction and internal fixation of distal humeral fractures.

Postoperative care is a critically important component in treating patients with complex distal humeral fractures. The immediate, intermediate, and late phases of the postoperative period present different issues that require specific management to maximize the eventual functional outcome for patients with these injuries.

Surgical treatment of traumatic pediatric humeral diaphyseal fractures with titanium elastic nails.

BACKGROUND: Nearly all pediatric humeral shaft fractures can be successfully treated with closed methods. Some patients, however, require internal fixation either because of an inability to maintain an adequate reduction, significant soft tissue injury, or concomitant fractures. METHODS: This is a retrospective review of all traumatic humeral shaft fractures treated at our hospitals between 1999 and 2006. Thirteen pediatric patients ranging in age from 4.8 to 16.7 years (mean age 12.0 years) were treated surgically with titanium elastic nails (TENs). Relative surgical indications included ope

Internal fixation of proximal humerus fractures using the T2-proximal humeral nail.

INTRODUCTION: Surgical management of proximal humerus fractures remains controversial and there is an increasing interest in intramedullary nailing. Created to improve previous designs, the T2-proximal humeral nail (PHN) (Stryker) has been recently released, and the English literature lacks a series evaluating its results. We present a clinical prospective study evaluating this implant for proximal humeral fractures. METHOD: We evaluated the functional and radiological results and possible complications. Twenty-nine patients with displaced fractures of the proximal humerus were treated with t

Open medial placement of Kirschner wires for supracondylar humeral fractures in children.

PURPOSE: To assess the efficacy of our protocol for treatment of displaced Gartland type-3 supracondylar fractures of the humerus in children. METHODS: Records of 43 children with displaced Gartland type-3 supracondylar fractures of the humerus admitted from October 1997 to October 2003 were reviewed. Patients were treated within 12 hours of admission by closed reduction (n=33) or open reduction (n=10). Crossed medial and lateral Kirschner wires through a medial approach were used in all patients. A 3-cm incision was made medially to identify the correct entry point of the medial wire, and th

Valgus impacted proximal humeral fractures and their blood supply after transosseous suturing.

We treated 16 patients (11 women and five men, average age 45 years), all having four-part valgus impacted fractures of the proximal humerus, with transosseous suturing. All had preoperative angiography performed 6-12 h after admission. The average impaction angle was 43 degrees , and the mean lateral displacement of the humeral head was 1.4 mm. Postoperative angiography was performed 8-10 weeks after the operation followed by digital image processing using the segmentation technique. No statistically important reduction in the length and area of large (>0.5 mm) vessels was seen. Union was con

Minimally invasive reduction and osteosynthesis of articular fractures of the humeral head.

Percutaneous reduction and fixation of severe humeral head fractures would be the treatment of choice since it will not increase the risk of necrosis already inherent in these fractures. Nevertheless, the question arises of whether anatomical reduction is possible with the percutaneous technique and whether the reduced fracture can be adequately stabilized. It is important to study the fracture closely before the operation in order to determine the fracture type and identify the relationship of the individual fragments to each other. Radiographs taken in at least two planes are essential and a

Intramedullary nailing of humeral shaft fractures. A retrospective study of 126 cases.

Antegrade intramedullary nailing with four different implants was used in 126 humeral shaft fractures. There were 74 acute fractures, 17 pathologic fractures, 16 fractures malaligned in a hanging cast or brace, 15 fractures with delayed union and 4 fractures that were nailed after failed open reduction and internal fixation. The nonunion rate was 21/95 after primary operation, and after reoperations 14/95. Distraction of the fracture was a significant cause of nonunion, but not type of fracture, localization, implant, and delay between injury and surgery. Shoulder joint function was significan
