The triceps-splitting approach for repair of distal humeral malunion in children. A report of a technique.

Repair of cubitus varus in a child requires a distal humeral osteotomy. The triceps-splitting surgical approach was used for repair of distal humeral malunion in 10 children. Six patients had repair of cubitus varus, 3 had fixation of a fresh fracture, and 1 had repair of a nonunion. No problems with healing were noted. Triceps weakness and elbow stiffness did not occur. The triceps-splitting approach is particularly well suited for repair of cubitus varus in children because, compared with the lateral approach, it is cosmetically favorable, avoids ulnar and cutaneous nerves, does not damage e

Flexion-Type Supracondylar Humeral Fracture with Ulnar Nerve Injury in Children: Two Case Reports and Review of the Literature.

BACKGROUND Supracondylar humeral fracture is a common fracture in the pediatric population. Although extension-type is the most common fracture pattern (97% to 98%), flexion-type supracondylar fractures are rarely encountered (2% to 3%). The combination of a flexion-type supracondylar humeral fracture with an ulnar nerve injury represents a real challenge for an orthopaedic surgeon. CASE REPORT We report 2 cases of flexion-type supracondylar humeral fracture with ulnar nerve injury that open reduction and fixation was necessary because closed reduction could not achieve an acceptable result. Is there a place for external fixation in humeral shaft fractures?

There is a good indication for unilateral axial dynamic external fixation in fractures of the humeral shaft when the fracture appears in the distal third or in cases of bilateral fractures. A non-union or a posttraumatic paralysis of the radial nerve may be indications for external fixation as well as fractures associated with multiple injuries. Further indications include osteitis, infected non-union and comminuted fracture. There is maximum protection of the soft tissue with this method of treatment. External fixation combines the advantages of conservative and operative treatment by influen

Intra-articular fractures of the upper extremity: new concepts in surgical treatment.

New techniques of internal fixation, postoperative rehabilitation, and emphasis on functional as well as radiographic outcome have refined the surgical treatment of complex fractures of the glenoid, humeral head, supracondylar and intracondylar humerus, olecranon, radial head, distal radius, and distal radioulnar joint over the past decade. Early stabilization and rehabilitation of these injuries leads to soft-tissue stabilization and facilitates the patient's ability to place the hand in three-dimensional space.

Validation of minimally invasive articular cartilage sparing technique for olecranon osteotomy.

BACKGROUND: The authors present a cadaveric validation of a minimally invasive articular cartilage preserving olecranon osteotomy technique for use in the operative management of distal humeral fractures. METHODS: Twenty-four elbows in six male and six female formaldehyde embalmed cadavers were dissected. With the cadaver placed in a lateral decubitus position, a posterior sub-periosteal dissection was performed to the medial and lateral aspects of the olecranon at the level of the joint and Mini Hohmann retractors were inserted into each side of the ulnohumeral joint. The medial (M) and the

Implant-Free Subpectoral Biceps Tenodesis Is Biomechanically at Higher Risk of Spiral Fracture of the Humerus Compared With Implant-Free Suprapectoral Biceps Tenodesis.

PURPOSE: To compare the likelihood of spiral fracture of the humerus using torsional load to failure after intraosseous biceps tenodesis at the position of the arthroscopic suprapectoral tenodesis versus the subpectoral meta-diaphyseal location. METHODS: Eight matched pairs of humeri were dissected. Unicortical tenodesis holes were drilled, either at the bottom of the bicipital groove (group 1) or just below the pectoralis major tendon insertion (subpectoral) in the humeral diaphysis (group 2). Tenodesis was performed in a 7-mm bone tunnel, with suture fixation distal to this site using 2 sep

Does timing of total elbow arthroplasty after distal humerus fracture affect 2-year complication rates?

BACKGROUND: Open reduction and internal fixation (ORIF) remains the gold standard for adult distal humerus fractures (DHF). However, indications for total elbow arthroplasty (TEA) continue to expand and the incidence of primary and salvage TEA for DHF has increased. The objective of this study was to compare complication and reoperation rate for acute vs. delayed primary and salvage TEA performed for DHF. METHODS: Patients who underwent TEA for DHF were identified in the PearlDiver database. Patients were sorted into 3 cohorts: (1) acute TEA (within 2 weeks of diagnosis), (2) delayed TEA (bet

Treatment of pathological fractures of the humerus with a locked intramedullary nail.

OBJECTIVE: The humerus is a common site for metastasis. Intramedullary nail fixation has been reported to be the best form of fixation for this disease but complications occur. This study aimed to assess the use of a new humeral nail to treat pathological fractures and impending pathological fractures of the humerus. METHODS: 29 patients received 31 Austofix locked intramedullary humeral nails: 25 for pathological fractures and 6 for impending fractures; 24 nails were inserted anterograde and 7 retrograde. Cement augmentation was applied in 4 patients, and adjuvant therapy was used in 28 pati

Bone Healing Process of a Multiple Humeral Fracture in a Caretta caretta: Clinical, Surgical, Radiographic and Histomorphometric Assessments.

This study describes the surgical treatment of multiple humeral fractures in a Caretta caretta sea turtle referred by the 'Centro Faunistico del Parco Regionale Bosco e Paludi di Rauccio', in the area surrounding the city of Lecce, in southern Italy. Radiographs showed an evident detachment of the distal humeral epiphysis, compatible with a type II Salter-Harris fracture, as well as a transverse fracture of the diaphysis. After the surgical fracture reduction, radiographic follow-up was performed at 2, 4, 12, 16, and 24 months, showing a progressive healing and the formation of poorly minerali

Finite element analysis of modified Slongo's external fixation in the treatment of supracondylar humeral fractures in older children.

Older children over 8 years old are at higher risk of elbow joint stiffness after treatment of supracondylar humeral fractures. The objective of this study was to improve the Slongo's external fixation system for treating supracondylar humeral fractures in older children. This would be achieved by increasing fixation strength and providing a theoretical basis through finite element analysis and mechanical testing. A 13-year-old female patient with a history of previous fracture was selected for CT data processing to create a three-dimensional model of the distal humerus fracture. Two internal

Windshield Wiper Effect in Human Bone-Peri-Implant Benign Diaphyseal Expansile Lesion with Humerus Shaft Fracture Non-Union with Implant in situ - Case Report.

INTRODUCTION: Humerus shaft fractures account for 3% of all adult fractures. Union rate after surgical management is around 84-97% with no difference between compression plating and intramedullary (IM) nailing. Non-union of humeral shaft fracture is not unusual complication of both conservative and operative treatment. IM nailing has been known to have several benefits from its relative stability with minimal soft-tissue dissection but with drawbacks such as less perfect reduction with higher risk of distraction, inability to take down interpositional tissue, risk of radial nerve injury and te

Metastatic involvement of the humerus: a retrospective study of 51 cases.

The authors performed a retrospective review of 53 patients with 56 completed or impending pathological fractures of the humerus. Sixteen cases were treated with a proximal and one with a distal humeral prosthesis, 38 with an intramedullary device with cement and one with plate fixation. Forty-eight patients with 51 humeral metastases were available to follow-up. No or only occasional pain was observed in 92% of the operated extremities and adequate postoperative function in 90%. No major complications occurred, except one rod migration.

Troubleshooting in the Treatment of Pseudoarthrosis After a Humeral Diaphyseal Fracture: A Case Report.

We report a case of a patient with a history of surgery for insufficient pseudoarthrosis after a humeral diaphyseal fracture. Although most humeral nonunions are successfully treated with a single procedure, some humeral nonunions are more difficult to heal and require multiple procedures. A-61-year-old man presented at our hospital with pain and deformity in his left upper arm. The humeral diaphyseal fracture had been previously treated elsewhere with open reduction and internal fixation. Some days prior to his visit at our hospital, he had had pain in his left upper arm, without a particular

Using the Medial and Lateral Humeral Lines as an Adjunct to Intraoperative Elbow Arthrography to Guide Intraoperative Reduction and Fixation of Distal Humerus Physeal Separations Reduces the Incidence of Postoperative Cubitus Varus.

BACKGROUND: Distal humerus physeal separations are rare pediatric elbow fractures that are often misdiagnosed and difficult to treat. Adequate reduction is often technically challenging and up to 71% of children develop postoperative cubitus varus. We propose using the medial and lateral humeral lines as an adjunct to elbow arthrography in order to guide intraoperative fixation of distal humerus physeal separations to reduce the incidence of postoperative cubitus varus. METHODS: From 2009 to 2014, all pediatric patients under the age of 3 diagnosed with a distal humerus physeal separation and

Rotational radioulnar dissociation: a case report and review of the literature.

We report a rare case of radioulnar dissociation in an 11-year-old boy associated with fracture of the medial humeral epicondyle. Open reduction and Kirschner wire fixation was performed for the epicondylar fracture. The radial head was reduced closed, and the distal radioulnar joint reduced spontaneously. The mechanism of this unusual injury appears to be hyperpronation of the forearm and valgus stress to the elbow.

Treatment and outcomes of fractures in children: a registry-based cohort study of 10,144 fractures.

BACKGROUND AND PURPOSE: There is no universal consensus on evaluating the quality of pediatric fracture care. While U.S. News ranks hospitals based on surgical timing and procedural benchmarks, additional metrics such as complication rates, reoperations, and patient-reported outcomes could provide a more comprehensive assessment. We aimed to evaluate pediatric fracture care outcomes at a Level I trauma center, emphasizing standardization and transparency in benchmarking. METHODS: This retrospective study reviewed all fractures diagnosed at HUS New Children's Hospital, Helsinki from 2018 to 20

Ultrasound-guided cross-pin technique for paediatric supracondylar humerus fractures: minimizing iatrogenic ulnar nerve injury.

BACKGROUND: Previous ultrasound-guided cross-pin techniques, which employ 90° elbow flexion, have demonstrated effectiveness; however, they may be associated with an elevated risk of iatrogenic ulnar nerve subluxation or dislocation. The aim of this study was to evaluate the efficacy and safety of a modified ultrasound-guided cross-pin technique for reducing the risk of iatrogenic ulnar nerve injury in paediatric patients with supracondylar humerus fractures. METHODS: This retrospective study was conducted from December 2017- October 2019 and included paediatric patients with displaced supraco

Distal humerus fractures in subjects over 65 years old: about 62 cases in Moroccan population.

Distal humerus fractures, defined as interruptions of bone continuity below the insertion of the anterior brachial muscle, are relatively rare, representing 1 to 2% of all fractures. These fractures display a bimodal distribution, predominantly affecting young men (12-19 years old) due to violent trauma and elderly women (>80 years old) due to low-energy trauma associated with osteoporosis. This study aims to present the clinical, radiological, and therapeutic outcomes of distal humerus fractures in subjects over 65 years. A retrospective analysis of 62 cases treated surgically with plates at

The aim of this retrospective analysis was to evaluate the adequacy of internal fixation of distal humerus fractures in patients over 75 years of age. 49 patients were evaluated. The mean age was 80 (75-93) years. The fractures were classified as 28 C-, 13 B- and 8 A-types according to the AO-system. Primary stable fixation was followed by early assisted mobilisation. The mean average follow-up time was 18 months. Excellent and good functional results were observed in 85%. 66% of all patients have no pain. There were 6 sensible ulnar nerve lesions, one deep wound infection, one pseudarthrosis
