Multidirectional Shoulder Instability With Circumferential Labral Tear and Bony Reverse Hill Sachs: Treatment with 270° Labral Repair and Fresh Talus Osteochondral Allograft to the Humeral Head.

Traumatic posterior dislocations of the shoulder can result in bony defects, labral tears, and cartilage injuries of the glenohumeral joint. Although traditional Hill-Sachs lesions from anterior dislocations are more commonly identified, reverse Hill-Sachs lesions caused by posterior dislocation often leads to recurrent engagement of the humeral head with the glenoid and significantly greater damage to the humeral chondral surface. In severe traumatic cases, concomitant damage of the capsulolabral soft tissues, such as circumferential labral lesions, can lead to chronic shoulder instability an…

Bilateral humeral head osteonecrosis following spinal cord injury: a case report illustrating the importance of adhering to the recommendations of the Second National Acute Spinal Cord Injury Study.

Five years prior to the 1990 publication of the Second National Acute Spinal Cord Injury Study (SNASCIS), a 24-year-old man sustained traumatic paraplegia, and was treated with 797 mg of dexamethasone over the ensuing 26 days. Within three years he developed symptomatic bilateral humeral head osteonecrosis. Although his total steroid dose was less than one-third of the comparable dose recommended by the SNASCIS, the duration of administration of the steroid was much longer. This case illustrates the importance of adhering to the guidelines established by the SNASCIS, especially regarding the 2…

Glenoid wear and its impact on clinical results after humeral head replacement using a single prosthesis in cuff tear arthropathy with more than 8 years of follow-up.

BACKGROUND: Glenoid wear (GW) is a long-term complication after humeral head replacement (HHR) and is one of the major reasons for revision surgery. This study aimed to evaluate GW at >8 years of follow-up after HHR in patients with cuff tear arthropathy (CTA) by use of a modified classification of GW, to examine the relationship between GW and clinical outcomes, and to identify risk factors for GW progression. METHODS: This retrospective case-series study included 34 shoulders that were monitored for >8 years after HHR in patients with CTA (13 men and 21 women; mean age at surgery, 70.9 year…

Etiological factors in osteochondritis dissecans. An experimental study into the etiological factors in osteochondritis dissecans in the canine humeral head using overloading with and without somatotropin and thyrotropin hormone treatment and mechanical trauma.

The overloaded humeral heads of the six growing Labrador retrievers treated with somatotropin and thyrotropin hormone showed the most pronounced changes of the articular cartilage and subchondral home, resembling changes in the early stage of spontaneous osteochondritis dissecans. The overloading alone used in 16 dogs without hormone treatment produced no changes, except that two Labrador retrievers showed increased radiodensity subchondrally during the first 4-6 weeks of the observation period. The mechanical traumas of the articular cartilage and the epiphyseal bone regenerated and did not r…

Middle trapezius tendon transfer using Achilles allograft for irreparable isolated supraspinatus tendon tears effectively restores the superior stability of the humeral head without restricting range of motion: a biomechanical study.

BACKGROUND: Middle trapezius tendon (MTT) transfer has been suggested for promising treatment of irreparable isolated supraspinatus tendon t … s allograft for IISTTs. Total humeral rotational range of motion (ROM), superior translation of the humeral head, and subacromial contact characteristics were measured at 0°, 20°, and 40° glenohumeral abduction (representing 0°, 30°, and 60° shoulder abduction). Superior translation and subacromial contact pressures were measured at 0°, 30°, 60°, and 90° exter …

What Is the Critical Value of Glenoid Bone Loss at Which Soft Tissue Bankart Repair Does Not Restore Glenohumeral Translation, Restricts Range of Motion, and Leads to Abnormal Humeral Head Position?

BACKGROUND: A general consensus has been formed that glenoid bone loss greater than 20% to 25% is the critical amount at which bony augmentation procedures are needed; however, recent clinical results suggest that the critical levels must be reconsidered to lower values. PURPOSE: This study aimed to find the critical value of anterior glenoid bone loss when a soft tissue repair is not adequate to restore anterior-inferior glenohumeral translation, rotational range of motion, or humeral head position using a biomechanical anterior shoulder instability model. STUDY DESIGN: Controlled laborator…

A quantitative method for determining medial migration of the humeral head after shoulder arthroplasty: preliminary results in assessing glenoid wear at a minimum of two years after hemiarthroplasty with concentric glenoid reaming.

HYPOTHESIS: Glenoid erosion and medial migration of the humeral head prosthesis have been observed after most types of shoulder arthroplasty. A method of measuring the change in humeral head position with time after shoulder prosthetic arthroplasty was applied it to 14 shoulders that underwent humeral hemiarthroplasty with concentric glenoid reaming. We hypothesized that the measurement technique would be reproducible and that the rate of wear would be small in the series of shoulders studied. MATERIALS AND METHODS: Standardized anteroposterior and axillary radiographs were obtained after sur…

Arthroscopic retrograde disimpaction of reverse Hill-Sachs lesions in acute posterior shoulder dislocation type A2 leads to good clinical outcome and close to anatomic reconstruction of the articular surface of the humeral head.

BACKGROUND: Posterior shoulder dislocation frequently results in a centrally located impression fracture of the anterior humeral head, known as reverse Hill-Sachs lesion (RHSL). Depending on its size and location, the RHSL can lead to engagement with the posterior glenoid rim and subsequently redislocation of the shoulder joint. The objective of this study was to present the clinical and radiologic outcomes of anatomic reconstruction of the humeral articular surface using arthroscopically assisted disimpaction in patients with acute posterior shoulder dislocation and an engaging RHSL. METHODS…

Computer algorithms for three-dimensional measurement of humeral anatomy: analysis of 140 paired humeri.

BACKGROUND: In the presence of severe osteoarthritis, osteonecrosis, or proximal humeral fracture, the contralateral humerus may serve as a template for the 3-dimensional (3D) preoperative planning of reconstructive surgery. The purpose of this study was to develop algorithms for performing 3D measurements of the humeral anatomy and further to assess side-to-side (bilateral) differences in humeral head retrotorsion, humeral head inclination, humeral length, and humeral head radius and height. METHODS: The 3D models of 140 paired humeri (70 cadavers) were extracted from computed tomographic da…

Relationship Between Shoulder External Rotation and Glenohumeral Joint Alignment Changes in Baseball Players.

Clarification of glenohumeral joint alignment changes during the late cocking phase may reveal the mechanisms of throwing injuries. This study aimed to determine the effect of shoulder external rotation on humeral head center deviation relative to the scapular glenoid. Twenty-eight baseball players participated. The anteroposterior deviation of the humeral head center relative to the glenoid (humeral head translation) and the distance between the humeral head and posterior glenoid rim perpendicular to the glenoid articular surface (posterior glenohumeral distance) were measured. Magnetic reson…

Quantitative assessment of the vascularity of the proximal part of the humerus.

BACKGROUND: The current consensus in the literature is that the anterolateral branch of the anterior humeral circumflex artery provides the main blood supply to the humeral head. While the artery is disrupted in association with 80% of proximal humeral fractures, resultant osteonecrosis is infrequent. This inconsistency suggests a greater role for the posterior humeral circumflex artery than has been previously described. We hypothesized that the posterior humeral circumflex artery provides a greater percentage of perfusion to the humeral head than the anterior humeral circumflex artery does. …

Total shoulder arthroplasty using a subscapularis-sparing approach: a radiographic analysis.

BACKGROUND: Traditional total shoulder arthroplasty (TSA) involves releasing the subscapularis tendon for exposure. This can potentially lead to subscapularis insufficiency, compromised function, and dissatisfaction. A novel TSA technique preserves the subscapularis tendon by performing the procedure entirely through the rotator interval, allowing accelerated rehabilitation. However, early reports on this approach have noted malpositioning of the humeral component and residual osteophytes. In a randomized trial, we examined the incidence of humeral head malpositioning, incorrect sizing, and re…

Normal and abnormal mechanics of the glenohumeral joint in the horizontal plane.

This study was performed to evaluate the relationship of the humeral head to the scapula in the horizontal plane of motion and to describe in detail a method of obtaining and interpreting modified axillary roentgenograms. Twenty normal subjects and twelve patients who had anterior instability of the shoulder were evaluated with this technique. In the control group, the humeral head was centered in the glenoid cavity throughout the horizontal plane of motion except when the arm was in maximum extension and external rotation. In this position, the cocked stage of the throwing motion, the center …

Rapidly destructive arthrosis of the shoulder joints: radiographic, magnetic resonance imaging, and histopathologic findings.

BACKGROUND: Rapidly destructive arthrosis of the humeral head is a rare condition with an elusive pathophysiologic mechanism. In this study, radiographic and histopathologic findings were analyzed to determine the clinical characteristics of this rare condition. METHODS: We retrospectively analyzed 189 patients who underwent total shoulder arthroplasty from January 2001 to August 2012. Among them, 9 patients showed a particular pattern of rapid collapse of the humeral head on plain radiography and magnetic resonance imaging (MRI) within 12 months from symptom onset. Patients with trauma, rheu…

When to Abandon the Arthroscopic Bankart Repair: A Systematic Review.

CONTEXT: Bone loss is a major factor in determining surgical choice in patients with anterior glenohumeral instability. Although bone loss has been described, there is no consensus on glenoid, humeral head, and bipolar bone loss limits for which arthroscopic-only management with Bankart repair can be performed. OBJECTIVE: To provide guidelines for selecting a more complex repair or reconstruction (in lieu of arthroscopic-only Bankart repair) in the setting of glenohumeral instability based on available literature. DATA SOURCES: An electronic search of the literature for the period from 2000 …
