Hereditary haemochromatosis (HH), which is mainly associated with a C282Y polymorphism in HFE, is common among Caucasians of north European descent, but is very rare among Asians. Herein, we report a 43-year-old Japanese man who was diagnosed as having HH. A laboratory examination revealed an elevated serum iron level (280 μg/dl), hyperferritinemia (1698 ng/ml) and a low serum level of hepcidin-25 (4.0 ng/ml). Abdominal magnetic resonance imaging revealed findings suggestive of iron accumulation in the liver and pancreas. HFE gene sequencing in the patient revealed a novel homozygous TAC nucle…

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome caused by dysregulated immune activation. Viral infections, particularly Epstein-Barr virus, are well-known triggers; however, HLH associated with acute viral hepatitis is rare, especially in the setting of hepatitis A (HAV) and hepatitis E (HEV) coinfection. A 30-year-old man presented with fever following the onset of jaundice, along with fatigue and dyspnea. Laboratory evaluation revealed severe hyperbilirubinemia, markedly elevated transaminases, anemia, hyperferritinemia, and hypertriglyceridemia. He…

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a fatal hyperinflammatory complication of scrub typhus, often with delayed diagnos … eschar.
RESULTS: Laboratory findings fulfilled HLH-2004 diagnostic criteria, including cytopenias, hyperferritinemia (>3,000 ng/mL), hypofibrinogenemia, and hemophagocytosis on bone marrow aspirate. The patient was diagnosed with secondary HLH triggered by scrub typhus. Immediate treatment with oral doxycycline, combined with immunomodulation using intravenous imm …

There are several types of inflammatory myopathies, including juvenile dermatomyositis (JDM), which is characterized by muscle inflammation … muscles. Laboratory findings revealed elevated aldolase, hepatitis, high lactate dehydrogenase, and hyperferritinemia, leading to a diagnosis of JDM complicated by MAS. Pulse steroid therapy combined with cyclosporine was initiated, and she showed significant improvement initially. Elevated iron stores as indicated by hyperferritinemia with normal or mildly elevated transferrin saturation and mostly mild hepatic iron deposition are a characteristic finding in subjects with non-alcoholic fatty liver disease (NAFLD). Excess iron is observed in approximately one third of NAFLD patients and is commonly referred to as the "dysmetabolic iron overload syndrome". Clinical evidence suggests that elevated body iron stores aggravate the clinical course of NAFLD with regard to liver-related and extrahepatic disease complications which relates to the fact that excess iron catalyses t…

In this case report, a 56-year-old woman with a reversible multiple organ disease with septic fever, arthritis, rash, weight loss, thrombocytopenia, severe disturbance of liver function, renal tubular dysfunction, general muscular weakness, pleural and pericardial effusions and elevated CRP, leukocytosis and a striking hyperferritinemia is presented. An autoimmune disease and a septic process were excluded. Because of the significantly decreased plasma phosphorus concentration (0.2 mmol/l), caused by malabsorption after Whipple's operation with lowered vitamin D and secondary hyperparathyroidi…

PURPOSE: The study was performed to clarify in the hemophagocytic syndrome (HPS) how cytokinemia plays a role in its pathogenesis and if cyt … ytic syndrome (IAHS; n = 15) and those comparing surviving and fatal cases were assessed.
RESULTS: Hyperferritinemia and hypercytokinemia were present in all patients with HPS. Eleven of the 17 MAHS and three of the 15 IAHS cases were fatal (p < 0.05). No significant difference in cytokine concentrations was observed between MAHS and IAHS.
CONCLUSIONS: In terms o …

Parvovirus B19 (PVB19) has tropism for erythroid precursors and may cause clinically important hematological syndromes, including pure red c … ow examination showed hemophagocytosis and erythroid precursors with cytoplasmic blebs/pseudopodia. Hyperferritinemia and hypofibrinogenemia supported an HLH phenotype, and anti-PVB19 IgM was positive. Both patients received supportive care with corticosteroid-based immunomodulation; intravenous immunoglobulin was used as part of management. Hematological recovery …

Hemophagocytic lymphohistiocytosis (HLH) is a severe disorder caused by excessive activation of the immune system and is characterized by hyperinflammation and life-threatening multiple organ failure. We report a case of a 39-year-old female patient with rapidly progressive disease who presented with pancytopenia, coagulopathy, hepatic dysfunction, and hyperlipidemia. Laboratory tests revealed marked cytopenia, hypofibrinogenemia, hypertriglyceridemia, hyperferritinemia, elevated soluble CD25 levels, decreased natural killer cell activity, and hemophagocytosis in the bone marrow, meeting the d…

Macrophage activation syndrome (MAS) is a severe hyperinflammatory condition often associated with autoimmune diseases, particularly systemic lupus erythematosus (SLE). It presents significant diagnostic challenges due to overlapping clinical features with SLE flares. This case report presents the diagnostic complexities of MAS in a 19-year-old female with SLE and lupus nephritis. She initially presented with persistent fever, cytopenias, hyperferritinemia, hypertriglyceridemia, and elevated inflammatory markers. Despite comprehensive evaluation, including a bone marrow biopsy that did not rev…

On day 28, tofacitinib replaced tacrolimus due to persistent hyperferritinemia. The patient was discharged without home oxygen therapy on day 54. Diagnosis of hemophagocytic lymphohistiocytosis is a challenge in Nepal because of limited resources and the high prevalence of tropical feb … ations included fever, splenomegaly, hyponatremia, liver function derangement, hyperfibrinogenemia, hyperferritinemia, and cytopenia. With a mortality of 29% in our study cohort, hemophagocytic lymphohistiocytosis should be considered a lethal disease, and clinicians should maintain a high index of suspicion to diagnose this disease. Hemophagocytic lymphohistiocytosis (HLH) is an aggressive life-threatening disease that consists of uncontrolled activated lymphocytes and macrophages that secrete excessive cytokines. Symptoms and laboratory findings of HLH include prolonged fever, cytopenia, hepatosplenomegaly, liver dysfunction, hypertriglyceridemia, hyperferritinemia, increased soluble interleukin-2 receptor, low fibrinogen, and neurological problems. HLH has two forms: primary (familial autosomal recessive) or secondary (related to infections, malignancy, autoimmune and metabolic disorders, transplantations, chimeric anti…

RATIONALE: Adult-onset Still's disease (AOSD) is a rare inflammatory disease characterized by a classic triad of daily spike fever, arthritis, and a typical salmon-pink rash. The involvement of inflammatory cytokines by various factors such as infection, drug, or neoplasm causes refractory AOSD.
PATIENT CONCERNS: We report a 63-year-old man with a high fever, rash, hyperferritinemia, and M proteinemia. His serum levels of interleukin-6 and interleukin-18 were remarkably high at 192 and 114,250 pg/mL, respectively.
