[Disturbances of water metabolism in two dogs and one cat with central nervous system disorders].

Hypernatremia due to different pathophysiological mechanisms results in a rise in plasma osmolality. Dependent on its severity and on the speed of its development hyperosmolality can be life-threatening. This article describes 2 dogs and 1 cat with central nervous system disorders (adenoma of the pituitary gland, cerebral trauma). All patients developed normovolemic hypernatremia due to pituitary gland and hypothalamus dysfunction, respectively. Plasma sodium concentrations ranged from 163 to 185 mmol/l. Neurological examinations revealed lethargy, disturbances of consciousness, and ataxia, re…

Central pontine myelinolysis caused by hyperglycemia: A case report and mechanistic review.

This report highlights a rare case of a man in his 50s with central pontine myelinolysis who initially presented with dysphagia and dysarthria. The patient also exhibited unsteady gait, dizziness, and weakness in all extremities. Imaging studies revealed a typical "bat-wing" lesion at the base of the pons. The patient was diagnosed with central pontine myelinolysis secondary to a diabetic hyperglycemic hyperosmolar state complicated with hypernatremia. Following insulin therapy and rehydration, his symptoms were resolved within 1 week, while the imaging-detected lesions persisted. In most case…

Distal Renal Tubular Acidosis due to Primary Sjögren's Syndrome: Presents as Hypoakalemic Paralysis with Hypokalemia-Induced Nephrogenic Diabetes Insipidus.

Classic distal renal tubular acidosis (dRTA) is characterized by inability to acidify the urine maximally (to <pH 5.5) in the presence of systemic acidosis. dRTA is found in 5% of patients with Sjögren's syndrome. The major clinical renal manifestation of patients with Sjögren's syndrome is tubulointerstitial involvement, leading to dRTA, impaired concentrating ability, hypercalciuria, and less frequently proximal tubular defects. Nephrogenic diabetes insipidus (DI) is characterized by reduced responsiveness to vasopressin, leading to hypotonic polyuria and consequent hypernatremia. The common…

Hypernatremic myopathy.

We describe a 21-year-old man presenting with proximal muscle weakness associated with hypernatremia. His manifestations other than muscle weakness included dry skin, loss of axillary and pubic hair, decreased libido and loss of thirst sensation. His serum sodium level was elevated to 169-171 mEq./l but all other electrolytes were normal. In addition, serum CK was elevated and an EMG study showed myogenic changes. Endocrinological studies revealed hypothalamic hypopituitarism, while MRI revealed a suprasellar mass. A partial correction of hypernatremia led to an immediate recovery of the muscl…

Central diabetes insipitus in a patient with congenital toxoplasmosis.

A 33-day-old male infant who developed central diabetes insipitus as a complication of congenital toxoplasmosis is presented. He had polyuria and hypernatremia on admission and responded to Intranasal desmopressin acetate with the normalization of above mentioned findings. Computed tomographic (CT) scan of the brain showed obstructive hydrocephaly with periventricular and right basal ganglion calcification. CT scan of the pituitary gland, thyroid function tests, and serum cortisol levels were all normal. This is the first report of isolated diabetes insipitus with congenital toxoplasmosis in l…

Correction of hypervolaemic hypernatraemia by inducing negative Na+ and K+ balance in excess of negative water balance: a new quantitative approach.

BACKGROUND: Hypervolemic hypernatremia is caused by an increase in total exchangeable Na(+) and K(+) in excess of an increment in total body H(2)O (TBW). Unlike patients with hypovolemic or euvolemic hypernatremia, treatment needs to be targeted at correcting not only the elevated plasma Na(+) concentration, but also there is an additional requirement to achieve negative H(2)O balance to correct the increment in TBW. METHODS: Correction of hypervolemic hypernatremia can be attained by ensuring that the negative Na(+) and K(+) balance exceeds the negative H(2)O balance. These seemingly conflic…

Preventing neurological complications from dysnatremias in children.

Dysnatremias are among the most common electrolyte abnormalities encountered in hospitalized patients. In most cases, a dysnatremia results … with hypertonic saline, and should never be managed with fluid restriction alone. Hospital-acquired hypernatremia occurs in patients who have restricted access to fluids in combination with ongoing free-water losses. Hypernatremia could largely be prevented by providing adequate free-water to patients who have ongoing free-water losses or when mild hypernatremi …

Nephrogenic diabetes insipidus secondary to lithium therapy in the postoperative patient: a case report.

Nephrogenic diabetes insipidus (NDI) presents a rarely encountered but challenging fluid management problem in the perioperative period. This case is that of a patient with a perforated duodenal ulcer and previously undiagnosed NDI who received standard preoperative and postoperative hydration with normal saline, causing hypernatremia and an inappropriate diuresis. The resulting hypernatremia and hyperosmolality required aggressive hypotonic fluid replacement to return to preoperative values. Though refractory to 1-desamino-8-D-argenine-vasopressin (dDAVP), thiazide diuretics and nonsteroidal …

A non-fatal case of sodium toxicity.

A non-fatal case of sodium toxicity in a six-year-old boy is presented. Hypernatremia is the clinical term for an excessive concentration of sodium relative to water in the body. The diagnosis of hypernatremia was made at serum sodium (Na(+)) concentrations exceeding 150 mEq/L, and few people have been reported to survive concentrations greater than 160 mEq/L. This case involves a six-year-old boy who was taken to the hospital following a seizure attack, and lab analyses revealed serum sodium (Na(+)) levels of 234 mEq/L and serum chloride (Cl(-)) levels of 205 mEq/L. Clinical tests ruled out d…

Thirst and blood pressure regulation in a novel angiotensin II receptor type 1a knockout rat.

Angiotensin II (AngII) plays a pivotal role in body fluid balance through AngII type 1 receptors (Agtr1) expressed in circumventricular organs to stimulate thirst. Blockade of central Agtr1 attenuates thirst to hypotension but not acute hypernatremia. Data in genetic knockout models are limited as systemic AngII does not stimulate thirst in mice, and Agtr1a-/- mice are severely hypotensive. Therefore, we generated a novel Agtr1a-/- rat using CRISPR-Cas9 to produce an 11-bp frame-shift deletion in exon 3. Baseline mean arterial blood pressure (ABP) was approximately 10 mmHg lower in Agtr1a-/- v…

Successful treatment of hypernatremic thirst deficiency with chlorpropamide.

Two patients with hypodipsia and hypernatremia are described. The first patient, whose hypodipsia was of unknown cause, developed hypernatremia unless large volumes of fluid were urged upon him; upon treatment with chlorpropamide normal serum sodium levels were achieved with spontaneous fluid intake. The second patient had hypodipsia and diabetes insipidus resulting from a craniopharyngioma. Treatment with vasopressin and a prescribed daily water intake resulted in frequent hyper- and hyponatremia, but treatment with chlorpropramide yielded serum sodium values which were more often normal and …

Nutrition-focused evaluation and management of dysnatremias.

Alterations in sodium and water balance are commonplace and often occur concurrently. Dysnatremias (hyponatremia and hypernatremia) merely reflect a relative excess or deficit of plasma water in relation to plasma sodium. There is no predictable relationship between plasma sodium concentration, extracellular fluid volume, and total body sodium content. Evaluation and management of dysnatremias rely on an understanding of key terminology, normal body fluid composition and distribution, and regulation of water and sodium balance, combined with a directed history and physical and appropriate labo…

Fluid and electrolyte disturbances in patients with intracranial aneurysms.

Eighty-eight cases with water and electrolyte disturbances, including polyuria, hyper- or hyponatremia, were found in 1,000 cases of surgically treated cerebral aneurysms and 80 nonoperated cases. In this paper, the clinical courses of the 88 cases were studied and an investigation was made of the hypothalamic lesions seen in ten autopsy cases. Water and electrolyte disturbances were most numerous in cases of anterior communicating aneurysms, and the prognosis was poor. Those with hypernatremia had a poor prognosis, with a 42% mortality rate during hospitalization. In contrast, the mortality r…
