Normocalcemic primary hyperparathyroidism (NPHPT) is a phenotype of classic hypercalcemic primary hyperparathyroidism characterized by elevated parathyroid hormone levels in the setting of persistently normal serum calcium. While increasingly recognized as a significant cause for endocrine referral, its pathophysiology remains poorly understood. The diagnosis of NPHPT is one of exclusion, necessitating the rigorous investigation and exclusion of all secondary causes of hyperparathyroidism. Predominantly identified in postmenopausal women during evaluation for osteoporosis or nephrolithiasis, a

Primary hyperparathyroidism (PHPT) is a frequent disease. Prevalence continues to rise in Europe, long after the advent of automated calcemia meters, while incidence has stabilized. The disease is much more common in women, at around 75% of cases, and is more prevalent with advancing age, and particularly post-menopause.

INTRODUCTION: Parathyroidectomy is considered the most cost-effective treatment for primary hyperparathyroidism (PHPT), with a relatively low incidence of complications. Nonetheless, 5-10% of patients do not achieve cure after initial parathyroidectomy. We hereby report our experience to determine the causes of our uncured patients and explore factors that could be associated with failure.
METHODS: We conducted a retrospective case series study and reviewed all the patients who underwent surgery for PHPT from 2004 to 2020. Cure was defined as re-establishment of normal calcium homeostasis las

OBJECTIVES: Parathyroid glands exhibit a notable anatomical variability, complicating preoperative localization. Minimally invasive parathyroidectomy ideally requires two concordant imaging tests. This study aims to evaluate the degree of concordance of the different tests available with surgical findings and to create an algorithm to discern the order in which these tests should be performed.
METHODS: This prospective study included 93 patients with primary hyperparathyroidism (PHP) undergoing 99mTc-sestamibi SPECT/CT and cervical ultrasound. Discordant cases underwent secondary imaging (con

In hyperparathyroidism (hyperPTH), excessive amounts of PTH are secreted, interfering with calcium regulation in the body. Several drugs can control the disease's side effects, but none of them is an alternative treatment to surgery. Therefore, new drug candidates are necessary. In this study, three computationally repositioned drugs, DG 041, IMD 0354, and cucurbitacin I, are evaluated in an in vitro model of hyperPTH. First, we integrated publicly available transcriptomics datasets to propose drug candidates. Using 3D spheroids derived from a single primary hyperPTH patient, we assessed their

This is the case of a 76-year-old man admitted to hospital in a delirium state, previously diagnosed with a major depressive disorder at an age of 50 years, treated for years for chronic tension headache. The computed tomography of the head resulted negative. Inpatient laboratory tests revealed a mild hypercalcemia. Due to the progression of the disease (delirium state, dementia, tension headache, and depression), he was again admitted to hospital. The patient showed dysarthria, postural tremors, mirror movements and palmar hyperhidrosis, mild ataxia when walking, and rigidity. Sleep disturban

It is well recognised that over the past three to four decades the incidence of asymptomatic primary hyperparathyroidism has increased significantly. However, more recently this pattern has further evolved into a presentation with normocalcaemia and raised parathyroid hormone (PTH) level without the presence of a recognised stimulus for its rise, in the form of vitamin D deficiency or renal impairment. A diagnostic label of normocalcaemic hyperparathyroidism (NPHPT) has been given to this phenotype and in most such individuals, the initial PTH measurement is driven by the presence of metabolic

The article presents data on modern approaches to the diagnosis of various manifestations of hyperparathyroidism according to multispiral computed tomography, which do not require morphological verification and ensure the correctness of the diagnostic process. The radiological picture and differential diagnostics of changes from parathyroid glands and bone structures at hyperparathyroidism are described in detail.

OBJECTIVE: Little is known about the natural changes in parathyroid function after successful parathyroid surgery for primary hyperparathyroidism. The association of intact parathyroid hormone (iPTH) and calcium (Ca) with "temporary hypoparathyroidism" and "hungry bone syndrome" (HBS) was evaluated.
DESIGN: Potential risk factors for temporary hypoparathyroidism and HBS were evaluated by taking blood samples before surgery, intra-operatively, at postoperative day (POD) 1, at POD 5 to 7, in postoperative week (POW) 8 and in postoperative month (POM) 6.
PATIENTS: Of 425 patients, 43 (10.1%) ha

Primary hyperparathyroidism (PHPT) is one of the common endocrine disorders, which results clinically in nephrolithiasis, osteoporosis, muscle weakness, cardiac and psychiatric abnormalities even in a mild or asymptomatic disease. Parathyroidectomy (PTX) is the only definitive treatment for PHPT, however, some patients with sporadic PHPT refuse surgery, are medically unfit, or have residual or recurrent disease inaccessible to further surgery. These patients may require intervention for management of symptomatic or moderate to severe hypercalcemia, bone loss or kidney calculi.

Primary hyperparathyroidism (PHPT) is the most common cause of chronic hypercalcemia. With the advent of routine calcium screening, the classic presentation of renal and osseous symptoms has been largely replaced with mild, asymptomatic disease. In hypercalcemia caused by PHPT, serum parathyroid hormone levels are either high, or inappropriately normal. A single-gland adenoma is responsible for 80% of PHPT cases. Less frequent causes include 4-gland hyperplasia and parathyroid carcinoma. Diminished bone mineral density and nephrolithiasis are the major current clinical sequelae. Parathyroidect
