Laboratory experience with radiometric detection of bacteremia with three culture media.

In two long-term studies, the BACTEC radiometric system for detection of bacteremia was evaluated with three culture media each: (i) BACTEC media 6A (for aerobes) and 7B (for anaerobes) plus a thioglycolate medium and (ii) BACTEC media 6A, 7B, and 8A (hypertonic). In study 1, clinically significant isolates were identified in 1,873 (13.9%) of 13,432 blood cultures with all three media. The thioglycolate medium revealed 143 (1.1%) organisms not recovered from the 6A and 7B media. In study 2, isolates were identified in 1,135 (12.9%) of 8,759 cultures with all three media; 104 (1.2%) organisms w

Superiority of hypertonic culture medium for detection of Haemophilus influenzae by the BACTEC procedure.

A comparison of hypertonic (sucrose), aerobic, and anaerobic media for the detection of eight recent clinical isolates of Haemophilus influenzae type b by the BACTEC procedure revealed that the hypertonic medium, without exception, yielded diagnostic growth index values of 30 or above from 4 to 24 h earlier than did the other media. This culture medium was also superior when an increase in growth index units of 10, before a value of 30 was reached, was taken as a criterion. The number of colony-forming units increased more rapidly in the hypertonic medium as compared with the other media. Thes

Randomised comparative study on the efficacy of 3% versus 30% NaCl solution in the treatment of umbilical granuloma.

AIM: This study aimed to compare the efficacy of 3% versus 30% NaCl solution in treating umbilical granuloma (UG) and to evaluate parental satisfaction. METHODS: A randomised, double-blinded, comparative study in neonates with UG was conducted. Stratified, block-of-four randomisation was used to allocate neonates into two groups, treated with 3% or 30% pharmaceutical-grade NaCl solution, three times daily. The time to resolution was determined by daily photographs of UG and weekly hospital visits. If there was no response after 2 weeks, silver nitrate was used. Parental satisfaction was measu

Negotiated care improves fluid status in diabetic peritoneal dialysis patients.

BACKGROUND: Volume overload is common in diabetic patients on continuous ambulatory peritoneal dialysis (PD), especially when the patient's residual renal function decreases with time on PD. Due to the higher dietary salt and fluid intake, diabetic PD patients tend to use more hypertonic glucose solution to remove excess fluid, which in turn may lead to increased membrane permeability. In the present study, we investigated the effect of negotiated care on fluid status in diabetic PD patients. METHODS: All diabetic PD patients who had been on PD for at least 3 months by the end of year 2002 in

Availability of a rich source of sodium during the perinatal period programs the fluid balance restoration pattern in adult offspring.

Osmoregulatory mechanisms can be vulnerable to electrolyte and/or endocrine environmental changes during the perinatal period, differentially programming the developing offspring and affecting them even in adulthood. The aim of this study was to evaluate whether availability of hypertonic sodium solution during the perinatal period may induce a differential programming in adult offspring osmoregulatory mechanisms. With this aim, we studied water and sodium intake after Furosemide-sodium depletion in adult offspring exposed to hypertonic sodium solution from 1 week before mating until postnatal

Molecular mechanisms of peritoneal dialysis-induced microvascular vasodilation.

Peritoneal dialysis (PD) solutions dilate microvessels by undefined mechanisms. This vasodilation directly affects ultrafiltration and solute exchange during a PD dwell and is thought to account for the variable mass transfer area coefficient for small solutes during a glucose-based hypertonic dwell. We hypothesized that PD-mediated vasodilation occurs by endothelium-dependent mechanisms that involve endothelium energy-dependent K+ channels (K(ATP)), adenosine A1 receptor activation, and NO release. We used intravital videomicroscopy to study 3 levels of microvessels (A1 inflow arterioles abou

Altered spontaneous and osmotically induced drinking for fowls with permanent access to dilute quinine.

Fowls were given a dilute quinine solution as their sole source of fluid, and effects on normal ingestion and on drinking responses to dipsogenic stimuli were examined. Compared to controls with water, daily fluid intakes were depressed by 25% with quinine. There was no significant effect of quinine on food intake, but growth was suppressed slightly. Drinking in response to hypertonic NaCl injections was attenuated with quinine, and whereas an initial peak in water intake was seen directly after hypertonic NaCl injection, this was absent with quinine. Moreover, increases in quinine intake afte

Symptomatic Hyponatremia after Bowel Preparation: Report of Two Cases and Literature Review.

INTRODUCTION: Bowel preparation for colonoscopy and/or colorectal surgery can cause electrolyte imbalances. The risk of electrolyte imbalances seems to be related to the type of bowel cleansing solution, age of patients and comorbidities. CASE REPORT: We report two cases of symptomatic hyponatremia (focal neurological signs and coma) after bowel preparation with sodium picosulfate/magnesium citrate for colonoscopy. In both cases, symptoms related to hyponatremia rapidly disappeared after sodium level correction with intravenous administration of hypertonic saline (3% NaCl). DISCUSSION: Elect

Dose-dependent mortality involving convulsions due to subarachnoid Urografin® injection in rats.

An ionically hypertonic contrast medium Urografin® was inadvertently administered into the subarachnoid space of an individual and this resulted in convulsions and acute respiratory failure. We examined the effects of subarachnoid Urografin® injections on the rat central nervous system. The onset and frequency of the convulsions, as well as fatality, were dependent on the amount of Urografin® administered. No convulsions were observed in rats receiving injections of hypertonic NaCl solution or saline. The results confirmed that subarachnoid injections of Urografin® cause convulsions and death,

Controversies in shock research: hypertonic resuscitation--pros and cons.

The proper fluid resuscitation of hemorrhagic shock is still controversial. Hypertonic saline has been suggested for prehospital resuscitation of hemorrhagic shock, because of its superior ability to expand blood volume and elevate systemic blood pressure and cardiac output in a small volume and during a short time period. We have defined two types of hemorrhagic shock: controlled hemorrhagic shock (CHS), where the bleeding source is immediately occluded following hemorrhage, and uncontrolled hemorrhagic shock (UCHS), where bleeding is induced by injury to blood vessels that are left unocclude

Osmosensitive single neurones in the hypothalamus of unanaesthetized monkeys.

1. We recorded with tungsten micro-electrodes the activity of single neurones in the supraoptic nucleus (NSO) and adjacent regions of the hypothalamus while repeatedly injecting solutions of varying tonicity into the common carotid artery of trained, unanaesthetized monkeys who accepted the experimental restraints without anxiety.2. Intracarotid injections of mildly hypertonic solutions of sodium chloride produced a characteristic behavioural response during and immediately after injection: e.e.g. ;arousal,' lip and tongue smacking, chewing, irregular sniffing respiration and associated mildly…

Hypertonic dextrose prolotherapy in osteoarthritis: mechanisms, efficacy, and future research directions.

Osteoarthritis (OA) is a degenerative joint disease that causes pain, stiffness, and reduced mobility, significantly affecting the quality of life for millions globally. Current treatments focus on symptom management, with limited efficacy in addressing the underlying disease progression. Hypertonic dextrose prolotherapy (DPT) has emerged as a potential regenerative treatment, utilizing dextrose injections to stimulate the body's natural healing mechanisms by promoting tissue regeneration, strengthening ligaments, and improving joint function. Recent clinical studies, including the latest rese

Common pitfalls in the use of hypertonic sodium bicarbonate for cardiac toxic drug poisonings.

BACKGROUND: Hypertonic sodium bicarbonate is advocated for the treatment of sodium channel blocker poisoning, but its efficacy varies amongst different sodium channel blockers. This Commentary addresses common pitfalls and appropriate usage of hypertonic sodium bicarbonate therapy in cardiotoxic drug poisonings. SODIUM BICARBONATE WORKS SYNERGISTICALLY WITH HYPERVENTILATION: Serum alkalinization is best achieved by the synergistic effect of hypertonic sodium bicarbonate and hyperventilation (PCO2 ∼ 30-35 mmHg [0.47-0.6 kPa]). This reduces the dose of sodium bicarbonate required to achieve ser
