Pulmonary edema during hysteroscopic surgery: Three cases report.

Hysteroscopic surgery has become a routine gynecologic procedure. The advantages are associated with more accurate removal of lesion, its short operating time, rapid post-operative recovery and low morbidity. However, there are potentially serious complications which can be occured during and following hysteroscopic surgery. The complications are uterine perforation, fluid overload and electrolyte disturbance due to intravasation and absorption of uterine distention media, hemorrhage and, rarely, gas or air embolism. We experienced pulmonary edema during hysteroscopic surgery in three consecut

Dilutional hyponatremia developed during hysteroscopic myomectomy: A case report.

Hysteroscopy is a procedure that may appear minimally invasive, but may result in potentially disastrous complications. A hysteroscopy requires the insertion of a hysteroscope into the uterine cavity and the installation of a suitable distention medium for the visualization of the endometrium. Fluid overload due to the absorption of distention media during hysteroscopy can cause mild to severe complications, including hyponatremia, hypoosmolarity, nausea, vomiting, headache, arrhythmia, blindness, confusion, seizure, cerebral edema, brain herniation, and death. We report a case of a 41 year-ol

Transcervical suction of interstitial pregnancy under laparoscopic and hysteroscopic guidance.

Interstitial pregnancy is a rare but life-threatening condition. It is usually treated using cornual resection at operative laparoscopy or laparotomy; however, this may adversely affect future fertility and pregnancy. Transcervical suction using an 8F pediatric catheter placed through the cornua under laparoscopic and hysteroscopic guidance may be possible in some patients if the pregnancy is accessible vaginally. We describe a case of interstitial pregnancy treated with transcervical suction using an 8F pediatric catheter placed through the cornua under laparoscopic and hysteroscopic guidance

[Do hysteroscopic metroplasties really improve really reproductive outcome?].

The aim of metroplasties is to restore a normal uterine anatomy to improve obstetrical outcomes in some uterine malformations. The hysteroscopic septoplasty cures the septate uterus. It is an effective procedure in the case of recurrent abortion losses. It probably improves the rate of live birth in women without obstetrical antecedent. For some authors, it could be considered at the time of the diagnosis, because of the simplicity of the gesture and the low complication rate. The enlarging hysteroscopic metroplasty has certainly a positive impact on the obstetrical outcome in patients present

Fluoroscopically guided hysteroscopic division of adhesions in severe Asherman syndrome.

BACKGROUND: Severe Asherman syndrome that is stage III disease according to the American Fertility Society, with obliteration of the uterine cavity and the inability to visualize isolated pockets of the intrauterine cavity, makes safe and effective hysteroscopic division of adhesions difficult, if not impossible. TECHNIQUE: A 16-gauge, 80-mm Tuohy needle is introduced into the endocervical canal alongside a 5-mm diagnostic hysteroscope. The surgeon probes the area beyond the adhesion with the needle. Ultravist 76.9% is injected through the needle under fluoroscopic and hysteroscopic control.

Long-term results of hysteroscopic myomectomy for abnormal uterine bleeding.

OBJECTIVE: To analyze the efficacy of transcervical resection of submucous myomas and to identify prognostic factors for long-term results. METHODS: Two-hundred eighty-five women were treated with transcervical resection of submucous myomas without endometrial ablation. In case of incomplete resection a repeat procedure was offered. Long-term follow-up was obtained. Recurrence was defined as the need for further surgery. The relation of several variables with the outcome was analyzed using Cox proportional hazard regression analysis. RESULTS: Seventeen cases (6%) were lost to follow-up. The

[The effectiveness of hysteroscopic intrauterine septum resection in terms of reproductive outcome].

Uterine septum is the major intrauterine disorder associated with infertility and recurrent abortions. The aim of the study was to find out what is the effectiveness of hysteroscopic intrauterine septum resection in terms of reproductive outcome and in comparison with conventional procedure. The mean follow up period was 18 +/- 8.3 months. The pregnancy wastage before operation in study group was 79% and decreased to 45% after hysteroscopic treatment. Hysteroscopic resection of intrauterine septum is the treatment of choice for patients suffering from infertility or recurrent pregnancy wastage

Can hysteroscopic evaluation of endometrial carcinoma influence therapeutic treatment?

235 cases of primary endometrial adenocarcinoma (AC) (age range, 37-94; mean age, 61 years) were collected during the period 1980-1992. Hysteroscopic examination of both the endometrial cavity and the cervical canal was performed in every patient prior to hysterectomy, and evaluations of cancer extension in the endometrium (focal: 97 pts; partial: 82 pts; massive: 47 pts; unevaluable: 9 pts) and of endocervical involvement (positive: 45 pts) were compared to the histological findings and survival rates. The chi 2 test was used for statistical analysis, and statistical significance was consider

[Hysteroscopic endometrium ablation to avoid hysterectomy in "high risk" patients].

Hysteroscopic endometrial ablation under maximal anaesthesiological surveillance was performed in 34 high-risk patients to avoid hysterectomy. It was a collective of patients with heavy thrombo-embolic or thrombotic disease, either under permanent anticoagulation due to residual disease or multiple endoprosthetic treatment, or with endogenous coagulopathy. In all these women, hysterectomy was either a relative or an absolute contraindication. In 22 patients, treatment resulted in complete amenorrhoea or at least hypomenorrhoea (without menometrorrhagia) respectively cyclic spotting. In 6 furth

Uterine rupture during pregnancy in a patient with a history of hysteroscopic metroplasty.

BACKGROUND: Uterine septa may be responsible for spontaneous abortion. Hysteroscopic metroplasty may improve pregnancy outcome. CASE: A patient with a history of hysteroscopic resection of a uterine septum presented in preterm labor. This was her second pregnancy following the procedure; the first resulted in a term vaginal delivery. This second pregnancy ended with a preterm cesarean delivery. A 3.5 x 3.5-cm fundal rupture was discovered during the cesarean. The metroplasty had been performed with the KTP laser under direct laparoscopic visualization. No surgical complications had occurred.

Intraoperative ultrasound guidance for hysteroscopic retrieval of intrauterine foreign bodies.

Retained bony fragments after a therapeutic abortion represent a rare complication of this gynecologic procedure. Management has been previously described to include blind dilatation and curettage, hysterectomy, and in select cases and with variable success, hysteroscopy. We describe a case of retained bones after a second-trimester abortion managed by operative hysteroscopy using a resectoscope and wire loop under intraoperative transabdominal ultrasound guidance.
