Rectal stimulation with prebiotics and probiotics before ileostomy reversal: a study protocol for a randomized controlled trial.

BACKGROUND: Ileostomy closure is associated with a high rate of postoperative morbidity, and adynamic ileus is the most common complication, with an incidence of up to 32%. This complication is associated with delayed initiation of oral diet intake, abdominal distention, prolonged hospital stay, and more significant patient discomfort. The present study aims to evaluate the rectal stimulus with prebiotics and probiotics before ileostomy reversal. METHODS: This is a protocol study for an open-label randomized controlled clinical trial. Ethical approval was received (CAAE: 56551722.6.0000.0071)…

Less is more-the best test for anastomotic leaks in rectal cancer patients prior to ileostomy reversal.

PURPOSE: No clear consensus exists on how to routinely assess the integrity of the colorectal anastomosis prior to ileostomy reversal. The objective of this study was to evaluate the accuracy of contrast enema, endoscopic procedures, and digital rectal examination in rectal cancer patients in this setting. METHODS: A systematic literature search was performed. Studies assessing at least one index test for which a 2 × 2 table was calculable were included. Hierarchical summary receiver operating characteristic curves were calculated and used for test comparison. Paired data were used where para…

The effect of a diverting ileostomy formation on nutritional status and energy intake of patients undergoing colorectal surgery.

BACKGROUND AND AIMS: The effects of ileostomy construction and colonic bypass on the general nutritional status of the patients have not yet received much attention. The aim of the present study was to assess the effect of a diverting ileostomy formation, on the nutritional intake, body composition and nutritional status biochemical markers of patients with a newly formed ileostomy. METHODS: This was an observational study. Patients scheduled for elective rectosigmoid resection at a surgical unit of a public university hospital, were considered for study inclusion. Patients in whom a divertin…

Single-anastomosis duodenal ileostomy with sleeve is a safe and effective option for patients in an ambulatory surgical center.

BACKGROUND: The single-anastomosis duodenal ileostomy with sleeve gastrectomy (SADI-S) is gaining popularity in the United States as an alternative to the gastric bypass for patients with a high body mass index (BMI) or who are less likely to succeed with a sleeve. While SADI-S has similar weight loss to the gastric bypass, the complication rate is significantly lower, allowing surgeons to perform SADI-S in an ambulatory surgical center with a 23-hour stay. OBJECTIVES: To determine if SADI-S can be safely performed in an ambulatory surgical setting. SETTING: Private practice. METHODS: All S…

Psychometric validation of the Stoma-QOL questionnaire in a Canadian cross-sectional sample of colostomy and ileostomy patients.

OBJECTIVES: The Stoma-QOL questionnaire is a patient-reported outcome (PRO) used to measure quality of life in patients with ileostomy or colostomy. This study assesses the Stoma-QOL's overall and item-level psychometric characteristics in patients with temporary stomas, and whether stoma-related quality of life differs by demographic characteristics. MATERIALS AND METHODS: Analysis of cross-sectional observational PRO data from hospitals in Vancouver, Canada. Patients registered for elective ileostomy or colostomy closure, over the age of 18, and able to read English were eligible for partic…

Obstructed ileostomy in the third trimester of pregnancy due to compression from the gravid uterus: diagnosis and management.

Ileostomy obstruction in pregnancy, although rare, is a significant complication with associated morbidity and mortality. Early studies recommended immediate surgical intervention for cases of ileostomy obstruction in pregnancy. We present a case of ileostomy obstruction at 29-week gestation in which a laparotomy was performed for presumed adhesions. When adhesiolysis failed to resolve the obstruction, it became clear that the obstruction was caused by external compression from the enlarging gravid uterus. The remainder of the pregnancy was successfully managed by daily aspiration of bowel con…

Oral Antibiotic Preparation Is Associated with Lower Surgical Site Infection After Ileostomy Closure: A Retrospective Cohort Study.

Background/Objectives: Surgical site infection (SSI) remains a common complication after ileostomy closure. Mechanical bowel preparation (MBP) and oral antibiotic preparation are often bundled together, so their independent contributions to SSI risk are unclear. We evaluated the adjusted associations of oral antibiotic preparation and MBP with SSI after ileostomy closure, a setting in which these two components have rarely been examined separately. Methods: We retrospectively reviewed 432 consecutive patients undergoing loop or end ileostomy closure at a single center between January 2022 and …

Beyond the stoma: Exteriorized ileal loop without ostomy as a simple, feasible alternative to defunctioning loop ileostomy.

A defunctioning loop ileostomy is widely used to protect distal bowel repair and anastomoses, but is associated with significant stoma-related morbidity and the obvious need for a second procedure for closure. We therefore evaluated the creation of an unopened exteriorized (covering) ileal loop without ostomy as a simple, feasible alternative. In our prospective interventional study, 26 patients undergoing emergency surgery for ileal perforation or distal intestinal obstruction underwent this procedure, with an option for bedside conversion to ileostomy if required. Stoma conversion was requir…

Risk Assessment of Stoma Outlet Obstruction Development when a Temporary Ileostomy is Created during Rectal Cancer Surgery.

OBJECTIVES: In surgery for lower rectal cancer, temporary ileostomy can be created to avoid complications, such as anastomotic leakage. However, various complications may occur with the stoma, including stoma outlet obstruction (SOO). The occurrence of SOO can prolong the length of hospital stay and delay the introduction of adjuvant chemotherapy, which can negatively affect the prognosis. We retrospectively reviewed cases of temporary ileostomy at our hospital and evaluated the risk factors for SOO. METHODS: We extracted data pertaining to patients with temporary ileostomy created during sur…

Quadratus lumborum block for postoperative pain management in patients undergoing ileostomy closure: a prospective, randomized controlled trial.

BACKGROUND: Quadratus lumborum (QL) block is used for multimodal analgesia following abdominal surgery. We introduced an ultrasound-guided QL block to treat postoperative pain for ileostomy closure. This study aimed to investigate the analgesic efficacy of the QL block compared to placebo after ileostomy closure. METHODS: Fifty-seven patients undergoing elective ileostomy closure were randomized (1:1) to the placebo or QL block group in this double-blind randomized controlled trial. After general anesthesia, a unilateral QL block was performed under ultrasound guidance. Opioid consumption and…

Machine learning-based prediction of permanent ileostomy in older adults after laparoscopic anterior resection for rectal cancer.

OBJECTIVE: Conversion from temporary stoma to a permanent stoma (PS) following loop ileostomy (LI) significantly impairs patients' quality of life and prognosis. However, few predictive tools are currently available in clinical practice. This study aims to develop and validate a machine learning (ML) model to predict the risk of PS following laparoscopic anterior resection (LAR) in older adults with rectal cancer. METHODS: Clinical data were retrospectively collected from 956 older adults (age ≥ 60 years) who underwent LAR with LI between June 2015 and December 2024 at two campuses of the Fir…

Prophylactic negative pressure wound therapy to improve wound healing rates following ileostomy closure: a randomized controlled trial.

BACKGROUND: Reversal of ileostomy is associated with morbidity including wound infection and prolonged wound healing. Negative pressure wound therapy (NPWT) has been shown to reduce time to wound healing by secondary intention. The aim of this study was to determine whether NPWT improved wound healing rates, compared with simple wound dressings, in patients undergoing reversal of ileostomy where the skin wound is closed with a purse-string suture. METHODS: This was a dual-centre, open-label, randomized controlled trial with two parallel intervention arms. Patients undergoing elective loop ile…

Short-term outcome of diverting loop ileostomy reversals performed by residents: a retrospective cohort prognostic factor study.

AIM: The reversal of diverting loop ileostomy (DLI) is one of surgical trainees' first procedures. Complications of DLI reversal can cause life-threatening complications and increase patient morbidity. This study compared DLI reversals performed by surgical trainees with those by attending surgeons. METHOD: This retrospective cohort study was performed at a single primary care center on 300 patients undergoing DLI reversal. The primary outcome was morbidity, according to the Clavien-Dindo classification (CDC), with special attention paid to the surgeon's level of training. The secondary endpo…

Synthetic mesh versus biological mesh to prevent incisional hernia after loop-ileostomy closure: a randomized feasibility trial.

BACKGROUND: Incisional hernia is a frequent complication after loop-ileostomy closure, rationalizing hernia prevention. Biological meshes have been widely used in contaminated surgical sites instead of synthetic meshes in fear of mesh related complications. However, previous studies on meshes does not support this practice. The aim of Preloop trial was to study the safety and efficacy of synthetic mesh compared to a biological mesh in incisional hernia prevention after loop-ileostomy closure. METHODS: The Preloop randomized, feasibility trial was conducted from April 2018 until November 2021 …

A rare presentation of appendicitis contained within an incisional hernia post loop ileostomy reversal - A case report.

INTRODUCTION AND IMPORTANCE: Appendicitis within an incisional hernia is rare, with current literature describing a small number of cases, occurring through a variety of surgical incisions. We describe a case of appendicitis contained within an incisional hernia following reversal of a loop ileostomy, on a background of previous sigmoid cancer resection. This is the second such case we were able to identify on literature review. CASE PRESENTATION: A 45 year old man presented with one day of migratory abdominal pain, predominantly focused at a tender, irreducible lump in his right lower quadra…

Multivariate Analysis of Risk Factors Associated With the Nonreversal Ileostomy Following Sphincter-Preserving Surgery for Rectal Cancer.

PURPOSE: A loop ileostomy is used to protect an anastomosis after anal sphincter-preserving surgery, especially in patients with low rectal cancer, but little information is available concerning risk factors associated with a nonreversal ileostomy. The purpose of this study was to identify risk factors of ileostomy nonreversibility after a sphincter-saving resection for rectal cancer. METHODS: Six hundred seventy-nine (679) patients with rectal cancer who underwent sphincter-preserving surgery between January 2004 and December 2011 were evaluated retrospectively. Of the 679, 135 (19.9%) under…

The utility of routine pouchogram before ileostomy reversal in children and adolescents following ileal pouch anal anastomosis.

INTRODUCTION: Pouchograms are used to assess the integrity of the ileal pouch anal anastomosis (IPAA) in patients who have undergone restorative proctocolectomy. Its benefits have been questioned, and there are no data to support the routine use in children. METHODS: We retrospectively reviewed the charts of 26 patients who had an IPAA and pouchogram at our institution between 2001 and 2009. Each patient also underwent an examination under anesthesia to assess the integrity of the IPAA on the day of the ileostomy closure. RESULTS: The mean age of the patients was 13.8 (± 0.7) years. The pouc…
