Developing an HIV Pre-Exposure Prophylaxis Toolkit for Adolescent and Young Adults: Serving Primary Care Providers to Support Future Implementation Using the Consolidated Framework for Implementation Research.

Pre-exposure prophylaxis (PrEP) is an effective HIV prevention strategy for adolescents and young adults (AYAs, ages 14-26). PrEP uptake among AYAs remains low. Barriers to uptake include provider discomfort discussing sexuality, challenges engaging parents in sexual health conversations, and low perceived HIV risk. To address these gaps, we aimed to develop a toolkit to enhance AYA-serving primary care providers' (PCPs) comfort and confidence in discussing and prescribing PrEP. The Consolidated Framework for Implementation Research informed our approach. Four focus groups with youth-serving P

A process evaluation of how the routine vaccination programme is implemented at GP practices in England.

BACKGROUND: In recent years, the incidence of several pathogens of public health importance (measles, mumps, pertussis and rubella) has increased in Europe, leading to outbreaks. This has included England, where GP practices implement the vaccination programme based on government guidance. However, there has been no study of how implementation takes place, which makes it difficult to identify organisational variation and thus limits the ability to recommend interventions to improve coverage. The aim of this study is to undertake a comparative process evaluation of the implementation of the rou

Schools' Readiness for Child Sexual Abuse Prevention Education: an Overview of Theoretical Models.

Schools' readiness appears an important factor influencing their implementation of violence prevention programs. This review was undertaken … tional readiness for change theories. These models were appraised using four criteria for selecting implementation science theories and frameworks (Birken et al. 2017). We propose Weiner's (2009) organizational readiness for change as the most plausible theoretical model with both descriptive and analytical potential for assessing schools' readiness for child sexual abu …

Designing a Behaviour Change Wheel guided implementation strategy for a hypoxaemic respiratory failure and ARDS care pathway that targets barriers.

BACKGROUND: A significant gap exists between ideal evidence-based practice and real-world application of evidence-informed therapies for patients with hypoxaemic respiratory failure (HRF) and acute respiratory distress syndrome (ARDS). Pathways can improve the quality of care provided by helping integrate and organise the use of evidence informed practices, but barriers exist that can influence their adoption and successful implementation. We sought to identify barriers to the implementation of a best practice care pathway for HRF and ARDS and design an implementation science-based strategy ta…

Uptake of voluntary medical male circumcision among men with sexually transmitted infections in Lilongwe, Malawi: a protocol for a pre-interventional and post-interventional study.

INTRODUCTION: Voluntary medical male circumcision (VMMC) is one of the key interventions for HIV prevention. However, its uptake among men in Malawi is low. Implementation science strategies for demand creation of VMMC increase uptake. We designed an implementation science demand-creation intervention to increase the uptake of VMMC among men with sexually transmitted infections (STIs). METHODS AND ANALYSIS: We designed a pragmatic pre-interventional and post-interventional quasi-experimental study combined with a prospective observational design to determine the uptake, acceptability, appropr…

The "D&I Bridge": introducing a teaching tool to define the D, the I, and the why.

Interest in learning dissemination and implementation (D&I) science is at an all-time high. As founding faculty and fellows of a new center focused on D&I science, we have found that non-specialist researchers and newcomers to D&I science often express confusion around the difference between the D and the I. Relatedly, they struggle to identify what their specific D&I projects target to impact public health within the amorphous "black box" that is the singular, loosely defined "research-to-practice gap." To improve conceptual clarity and enhance engagement with D&I science, we developed a grap

Implementation of Food is Medicine Programs in Healthcare Settings: A Narrative Review.

Food is Medicine (FIM) programs to improve the accessibility of fruits and vegetables (FVs) or other healthy foods among patients with low income and diet-related chronic diseases are promising to improve food and nutrition security in the United States (US). However, FIM programs are relatively new and implementation guidance for healthcare settings using an implementation science lens is lacking. We used a narrative review to describe the evidence base on barriers and facilitators to FIM program integration in US healthcare settings following the Exploration, Preparation, Implementation, and…

Implementing Serosurveys in India: Experiences, Lessons Learned, and Recommendations.

Serological surveillance for vaccine-preventable diseases, such as measles and rubella, can provide direct measures of population immunity across age groups, identify gaps in immunity, and document changes in immunity over time. Rigorously conducted, representative household serosurveys provide high-quality estimates with minimal bias. However, they can be logistically challenging, expensive, and have higher refusal rates than vaccine coverage surveys. This article shares lessons learned through implementing nine measles and rubella household serosurveys in five districts in India-the challeng

ADOPTing a new method of partner management for genital chlamydia in New South Wales: findings from a pilot implementation program of patient-delivered partner therapy.

UNLABELLED: Background Patient-delivered partner therapy (PDPT) for chlamydia is an effective and safe additional partner management strategy. Some Australian regulatory changes have been made to support PDPT, but implementation guidance is lacking. This paper describes a pilot implementation program of PDPT in New South Wales (NSW), the Australian Development and Operationalisation of Partner Therapy (ADOPT). METHODS: ADOPT involved: (1) clarification of the NSW PDPT legal and policy framework; (2) development and implementation of PDPT service models, resources and data collection tools for

Conflicts of interest in medical science: peer usage, peer review and 'CoI consultancy'.

In recent years, the perception has grown that conflicts of interest are having a detrimental effect on medical science as it influences health policy and clinical practice, leading medical journals to enforce self-declaration of potential biases in the attempt to counteract or compensate for the problem. Conflict of interest (CoI) declarations have traditionally been considered inappropriate in pure science since its evaluation systems themselves constitute a mechanism for eliminating the effect of individual biases. Pure science is primarily evaluated by 'peer usage', in which scientific inf

Eight recommendations to enhance biodiversity outcomes of nature-based solutions at multiple scales.

Ambitions to mainstream Nature-based Solutions (NbS) and achieve global implementation are hampered by the lack of transdisciplinary integration across disciplines and practice. Ecological knowledge, including an understanding of how biodiversity can support, and be supported by, NbS design and implementation remains an under-studied component of the NbS concept despite its critical importance. Starting with a brief primer on the major roles that biodiversity plays in NbS performance, we present eight recommendations for enhancing biodiversity in NbS planning and implementation. This narrative

Virtual reality for teaching and learning in crime scene investigation.

Simulated crime scene investigation is an essential component of forensic science education, but its implementation is costly and poses challenges to accessibility; offering personal investigations in higher education scenarios is often impossible. Virtual reality (VR) is an emerging technology which offers exciting prospects for teaching and learning, especially for imparting practical skills. We document here a multidisciplinary experimental study in which a bespoke VR crime scene app was designed and implemented, after which it was tested by both undergraduate student and staff/postgraduate

The 'HAPI' project five years on: Incidence and prevalence of Hospital-Acquired Pressure Injuries in one local health district in Australia.

AIM: From 2015 to 2018, the 'Hospital-Acquired Pressure Injury (HAPI)' project was implemented in one local health district in Australia and utilised an implementation science approach to address rising pressure injury (PI) incidence and prevalence rates. This paper aims to examine whether the project was successful in sustaining low PI incidence and prevalence rates over the five-years following implementation (spanning the 2018/2019 to 2022/2023 financial years and the 2019-2023 calendar years). MATERIALS AND METHODS: A retrospective cohort study was conducted involving a comprehensive anal…
