A 48 year-old white male not suffering from endocrine disease or polydipsia, not taking diuretics, and suffering from no renal disease was started on risperidone and discharged on no other drug from Western Missiouri Mental Health Center (WMMHC) after an 8-day hospitalization. Seven days later he was admitted to a university medical center with generalized seizures, hyponatremia, respiratory failure, and rhabdomyalysis. He eventually recovered, was transferred back to WMMHC, and stabilized on appropriate medication. A search of the literature indicates no case reports linking risperidone to hy

A 62-year-old man with colon cancer who presented with hyponatremia is described. Volume depletion, renal failure, and cardiac, adrenal, hepatic, and thyroid diseases were excluded as causes of hyponatremia. The urine sodium concentration was repeatedly increased, suggesting the presence of the syndrome of inappropriate antidiuretic hormone secretion. An intact urinary diluting ability and the ability to maintain sodium balance without correcting hyponatremia when the sodium intake was high were consistent with the diagnosis of the reset osmostat variant of the syndrome of inappropriate antidi

Chronic syndrome of inappropriate secretion of antidiuretic hormone (chronic SIADH) has been reported in adults after traumatic brain injury (TBI) but few similar cases have been reported in the pediatric population. We present a case of a 14-year-old boy who developed chronic SIADH after TBI in which caloric needs could not be adequately provided, with concomitant fluid restriction as the first line of treatment. Demeclocycline was ultimately used, which allowed for increased fluid liberalization and for provision of adequate calories. This form of therapy should be considered early in a chil

Since it was first recognized 35 years ago, the syndrome of inappropriate antidiuretic hormone (SIADH) secretion has become the most commonly recognized cause of hyponatremia among hospitalized patients. The syndrome is caused by excessive intake of fluids when urinary dilution is impaired by physiologically inappropriate secretion or administration of vasopressin or other antidiuretic hormones. Inappropriate secretion of the hormone may be ectopic by a malignancy or ectopic and can ensure from any of three different types of osmoregulatory defects. Rarely, there is no demonstrable defect in t

A 59-yr-old female with acute infectious mononucleosis developed a severe polyradiculoneuritis with quadriplegia and a transient syndrome of inappropriate antidiuretic hormone secretion. Both these complications of the Epstein-Barr virus infection are rare.

The syndrome of inappropriate antidiuretic hormone secretion (SIADH) can result from diverse conditions. There have been only two published reports linking this syndrome with herpes zoster infections--one disseminated and the other confined to the chest wall. We have reported a case in which herpes zoster infection of the chest wall probably precipitated the development of this syndrome.

An acute confusional state, which developed in a patient with polyarteritis nodosa (PAN), proved to be secondary to inappropriate secretion of antidiuretic hormone and consequent hyponatraemia. This is a very unusual complication of PAN and may well reflect a direct stimulation of the supraoptic nuclei owing to cerebral vasculitis.

In three patients with SIADH, mannitol or glycerol promoted a rapid free water diuresis. Use of osmotic agents requires less laboratory and clinical monitoring than use of furosemide, is appropriate treatment, and in fact may be the treatment of choice in some patients with symptomatic hyponatremia from SIADH.

This is a report about a 14-year old girl who, following chemotherapy for malignant teratoma, developed a clinical state of SIADH (syndrome of inappropriate secretion of antidiuretic hormone). The causative agent was most likely vincristine (VCR). The important feature in this case was that the urinary kallikrein activity was high when she was affected by SIADH and decreased when her hyponatremia improved. Sodium clearance was significantly correlated with the increase in urinary kallikrein activity. It is considered that the kallikrein-kinin system may in part participate in the excessive nat

A case of systemic lupus erythematosus with reversible psychosis and hyperventilation related to hyponatraemia secondary to a syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is presented. The SIADH was treated successfully by oral urea for more than 8 months.

Two patients with the syndrome of inappropriate secretion of antidiuretic hormone were studied in a metabolic ward during treatment with 1.2 g demeclocycline daily. In both patients, demeclocycline treatment led to increased renal water excretion with consequent correction of hyponatremia and hypo-osmolality. Three episodes of reversible deterioration in glomerular filtration rate developed in these patients. Each episode was accompanied by clinical evidence of extracellular fluid volume contraction, and on each occasion there was an inappropriate natriuresis with daily urinary sodium excretio

Hyponatremia is a common finding after subarachnoid hemorrhaging (SAH) and can be caused by either cerebral salt-wasting syndrome (CSWS) or syndrome of inappropriate antidiuretic hormone (SIADH). Distinguishing between these two entities can be difficult because they have similar manifestations, including hyponatremia, serum hypo-osmolality, and high urine osmolality. We herein report the case of a 60-year-old man who suffered from SAH complicated by hyponatremia. During his initial hospitalization, he was diagnosed with CSWS. He was readmitted one week later with hyponatremia and was diagnose

We report a 26-year-old woman admitted to our hospital for generalized tonic seizure. Laboratory investigations revealed severe hyponatremia possibly triggered by vomiting and diarrhea. 24 hours after correction of hyponatremia she developed diffuse myalgias and marked hyperCKemia. Syndrome of inappropriate antidiuresis (SIAD) was suspected as cause of hyponatremia. Abnormal vaginal bleeding prompts gynecological evaluation and a small-cell carcinoma of uterine cervix was detected.

Intravascular large B-cell lymphoma (IVL) is a rare subtype of B-cell lymphoma presenting with neurological and dermatological lesions in addition to generalized symptoms such as fever and malaise. It may also be associated with variable manifestations of affected organs due to extranodal progression predominantly in the lumen of the small vessels. Here, we report a case of IVL with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) as the sole manifestation at the initial presentation. The present case suggests that hormonal disturbances may progress in advance in IVL, be
