IMCI approach in tertiary hospitals, India.

OBJECTIVE: This study was conducted to compare physicians' diagnosis with Integrated Management of Childhood Illness (IMCI) algorithm generated diagnosis in hospitalized children aged 2-59 months. METHODS: Recruited were patients aged 2-59 months admitted with one or more IMCI danger signs. IMCI and physician's diagnosis were noted and compared. RESULTS: In 222 included subjects, mean duration of illness was 9.4 (SD: 16.5) days. Among those with cough or difficult breathing, 44 (19.8%) and 66 (29.7%) were diagnosed as either severe pneumonia or mild to moderate pneumonia by physicians and IM

Granuloma multiforme: a report from India.

Granuloma multiforme (GM) is a reactive skin disorder of unknown etiology, characterized clinically by confluent annular lesions and histologically by focal necrobiosis and histiocytic granulomas. GM is significant because of its clinical resemblance to tuberculoid leprosy, with which it can be confused. Here, we report a case of granuloma multiforme from India in a 70-year-old male farmer, with multiple asymptomatic large annular rings of papules over the back and thighs on photodistributed sites. Histopathology helps to clinch the diagnosis and differentiate from similar clinical and histolo…

Presumed ocular histoplasmosis syndrome in India.

The authors report a case series of presumed ocular histoplasmosis syndrome (POHS) from India. Patients with progressive diminution of vision and having subretinal neovascularisation (SRNVM) were evaluated thoroughly to look for signs of POHS. Three patients had features suggestive of POHS, which to the best of the authors' knowledge is the first case series reported from India. This study shows that patients with clinical features suggestive of POHS do exist in India. A thorough fundus examination of young adults with supposedly idiopathic SRNVM may help to uncover more cases of POHS in India…

Ciprofloxacin-resistant Neisseria meningitidis, Delhi, India.

Decreased susceptibility of Neisseria meningitidis isolates to ciprofloxacin emerged from an outbreak in Delhi, India. Results of antimicrobial susceptibility testing of the meningococcal isolates to ciprofloxacin and further sequencing of DNA gyrase A quinolone-resistance-determining region confirmed the emergence of ciprofloxacin resistance in the outbreak.

Accessing modern contraceptive technologies in India.

The 'unmet' need for reproductive health care in India requires a wide demand net. Besides 'supply' of oral contraceptives, condoms and IUDs, the 'non-supply' approach for increasing contraceptive prevalence is recommended through education about the variety of safe contraceptives and awareness in the use of traditional methods. Expansion of education is partly responsible for the recent decline in fertility in India, however, gender inequality still prevails with poor sex ratio in urban regions, and deficit of around 35 million girls and women from higher mortality compared to males till the …

Cutaneous sporotrichosis in Himachal Pradesh, India.

During the period 1990-2002 in Himachal Pradesh, India, 103 new patients with cutaneous sporotrichosis were detected. Lymphocutaneous and fixed cutaneous varieties were the most common and seen in 49% and 43% respectively. Chronic non-specific inflammation without granuloma formation was seen in 46% skin biopsy specimens, 29% showed acute or chronic inflammation and tuberculoid granuloma formation was seen in 25%. In 32% of cases Sporothrix schenckii growth was seen on Sabouraud glucose agar. A saturated solution of potassium iodide was used as first-line treatment and in 93% patients healing …

Canine gastrointestinal parasitic zoonoses in India.

Although well recognized and studied in developed countries, canine parasitic zoonoses pose a lowly prioritized public health problem in developing countries such as India, where conditions are conducive for transmission. A study of the most recent parasite survey determining prevalence and epidemiology of canine parasitic zoonoses among tea-growing communities of northeast India demonstrated the endemicity of the problem. This particular study serves as a model using conventional, as well as molecular parasitological, tools to provide novel insights into the role of dogs as mechanical transmi…

Cutaneous sporotrichosis in Bangalore, southern India.

BACKGROUND: Sporotrichosis is a granulomatous infection caused by Sporothrix schenkii. Although world-wide in distribution, only four cases have been reported from southern India to date. METHODS: The medical records of all cases seen at the Department of Dermatology at St John's Medical College Hospital, Bangalore, over the last 15 years were reviewed. RESULTS: A total of seven cases of sporotrichosis were seen in this 15-year period. CONCLUSION: Sporotrichosis is considered to occur only sporadically in southern India. Our findings suggest that this may be a result of reporting bias.

Japanese encephalitis in Assam, northeast India.

Japanese encephalitis is one of the major public health problems in Assam, northeast India. We aimed to elucidated the clinical and epidemiological profile of the disease during several outbreaks in Assam in 3 consecutive years. Cerebro-spinal fluid and or serum samples of 348 out of 773 clinically-suspected viral encephalitis patients admitted to different hospitals during the period June to August of 2000 to 2002 were tested for detection of JE specific IgM antibody, employing MAC ELISA test at RMRC (ICMR), Dibrugarh. Diagnosis was confirmed in 53.7% patients with the ratios of 1.8:1 and 1.4…

Progress toward poliomyelitis eradication--India, 2003.

Since the World Health Assembly resolved in May 1988 to eradicate poliomyelitis, the estimated global incidence of polio has decreased >99%, and three World Health Organization (WHO) regions (Americas, Western Pacific, and European) have been certified as polio-free. Since 1994, when the countries of the WHO South-East Asia Region (SEAR) began accelerating polio-eradication activities, substantial progress toward that goal has been made. By 2001, poliovirus circulation in India had been limited primarily to the two northern states of Uttar Pradesh and Bihar, with 268 cases reported nationwide.…

Polio eradication in India: some observations.

In 1988, the World Health Assembly passed resolution WHA 41.28, which committed the World Health Organization (WHO) to the global eradication of poliomyelitis by the year 2000. In spite of the combined efforts by UNICEF, National Polio Surveillance Project (NPSP), Indian Academy of Pediatrics (IAP) and Rotary International, Polio Free India is still a distant dream. Though oral polio vaccine has succeeded in polio eradication from many countries but there is high incidence of vaccine failure in India. Oral polio vaccine (OPV) has failed to provide full protection to many children who have deve…

Shigella dysenteriae serotype 1, Kolkata, India.

Since July 2002, bacteriologically confirmed shigellosis cases have increased, and multidrug-resistant Shigella dysenteriae serotype 1 strains have reemerged in patients hospitalized with diarrhea in Kolkata, India. The isolated strains of S. dysenteriae 1 showed resistance to chloramphenicol (80%), ampicillin (100%), tetracycline (100%), co-trimoxazole (100%), nalidixic acid (100%), norfloxacin (100%), and ciprofloxacin (100%). Emergence of fluoroquinolone resistance in S. dysenteriae 1 strains complicated treatment of shigellosis patients. Six strains belonging to provisional serovars of S. …

Seronegative HIV-2 carriers in India.

The discordant cases of seronegative, but culture and proviral HIV-2 DNA positive were found in Mumbai, India. This was corroborated by the successful isolation of HIV-2-RNA in culture medium, HIV-2 cDNA sequence determination and the detection of the antigen. The sequence of the isolated HIV-2 genomic RNA does not seem to be altered to the extent that the change will alter antibody binding. Furthermore, antibody from the same individual (even at 8 months from initial sampling) from whom HIV-2 was isolated did not react with the antigen of this strain. Those evidences imply that extremely low …
