Chlamydia pneumoniae, an obligate intracellular human pathogen, causes infections of the respiratory tract. It is a significant cause of both lower and upper acute respiratory illnesses, including pneumonia, bronchitis, pharyngitis and sinusitis. Most respiratory infections caused by C. pneumoniae are mild or asymptomatic. Some studies have suggested a possible association of C. pneumoniae infection and acute exacerbations of asthma and chronic obstructive pulmonary disease (COPD). Seroepidemiological studies showing antibody prevalence rates in a range of 50 to 70% suggest that C. pneumoniae …

Prompt diagnosis and treatment of hand and upper extremity infections is imperative because they have the potential to be life threatening and pose a risk of functional impairment. Serious infections may result in prolonged hospitalization and resultant loss of productivity. The mainstay of treatment continues to be antibiotic therapy, heat, elevation, adequate surgical drainage, and débridement. Prompt specialty consultation should be obtained in cases in which there is a doubt about the diagnosis or when there is failure to improve in the face of seemingly appropriate treatment.

Monkeypox is an uncommon viral zoonosis caused by the monkeypox virus (MPV), a member of the genus Orthopoxvirus. MPV is a large, complex, double-stranded DNA virus of the chordopoxvirus family. MPV is endemic in central and western Africa and is a classic zoonosis acquired through contact with infected rodents and squirrels. Observational studies of human monkeypox in central and western Africa have revealed that MPV infection has an incubation period of 10-14 days and a period of infectivity during the first week of rash. MPV enters the body through skin abrasions, the upper respiratory trac

Globalization has produced an increase in the number of people at risk for contracting parasitic infection. Central nervous system infection by nematodal parasites can be devastating. Early recognition and treatment of infection can significantly decrease morbidity of the parasitic infection, as well as the risk of secondary superinfection. The clinical presentation, diagnosis, and treatment for five of the more common nematodal infections of the nervous system--Angiostrongylus spp., Baylisacaris procyonis, Gnathostoma spinigerum, Strongyloides stercoralis, and Toxocara spp.--is reviewed.

Candidiasis is the most common oral fungal infection diagnosed in humans. Candidiasis may result from immune system dysfunction or as a resu … pond to topical therapy. Appropriate systemic therapy should be employed for the treatment of these infections. Additionally, a biopsy should be undertaken in individuals with the hyperplastic variant of Candida because there is some degree of risk for malignant transformation. Deep fungal infections should be managed in association with appropriate medical …

Superficial fungal infections arise from a pathogen that is restricted to the stratum corneum, with little or no tissue reaction. In this Seminar, three types of infection will be covered: tinea versicolor, piedra, and tinea nigra. Tinea versicolor is common worldwide and is caused by Malassezia spp, which are human saprophytes that sometimes switch from yeast to pathogenic mycelial form. Malassezia furfur, Malassezia globosa, and Malassezia sympodialis are most closely linked to tinea versicolor. White and black piedra are both common in tropical regions of the world; white piedra is also end…

Infections of the mother-to-be continue to be a problem for a successful birth. However, on the basis of the case history, special examination methods (infection serology, pathogen determination) and the noninvasive and invasive prenatal diagnostic work-up, the risks for the fetus can be much better assessed than 10-15 years ago. This avoids unnecessary terminations of the pregnancy, better counseling of the parents-to-be, appropriate monitoring of the fetus, and improved treatment of mother and child during the pregnancy, or of the neonate. To ensure optimal care of mother and fetus, the fami…

"Emerging infections" have been defined as infections that have newly appeared, that have appeared previously but are expanding in incidence and geographic range, or that threaten to increase in the near future. This article focuses on nine emerging viral infectious agents. These viruses illustrate how such agents emerge: by encroaching on previously unvisited habitats (eg, hantaviruses), by air travel (eg, SARS), and by accidental importation (eg, monkeypox). Additionally, the example of SARS demonstrates not only how quickly emerging viral infections can spread but also how quickly they can …

Cutaneous gonococcal infections are frequent in the United States and other industrialized countries. This bacterial infection commonly affects the genitourinary tract and may rarely involve the skin. Cutaneous lesions are either primary or secondary to disseminate disease. The diagnosis of gonorrhea is based on clinical ground and is best confirmed by the isolation of the organism. Preventive measures including periodic screening of high-risk individuals and behavioral counseling are effective in controlling the disease.

Subcutaneous mycoses are caused by a variety of mostly tropical organisms, usually when they are implanted into the dermis or the subcutaneous tissue. They rarely disseminate or become systemic. Sporotrichosis, mycetoma, and chromoblastomycosis are more common subcutaneous mycoses than are rhinosporidiosis, zygomycosis, pheohyphomycosis, and lobomycosis.

The last 20 years have witnessed a profound reshaping of the spectrum of foodborne infections in industrialized countries. Food products are overall very safe, but the industrial scale and standardisation of food production have spawned spectacular epidemics, bringing into the light previously little known microorganisms such as Listeria or Escherichia coli O157:H7, the causative agent of the "hamburger disease". The recent "mad cow disease" crisis is another illustration of a food industry gone astray under the pressure of underlying economic stakes. Through unprecedented efforts from the cou…

Ticks, obligate, blood-sucking members of the order Acarina and class Arachnida, are the most common agents of vector-borne diseases in the United States. Ticks play an important role in transmitting viruses, bacteria, spirochetes, parasites, and rickettsia. This article reviews the epidemiology, microbiology, diagnosis, and treatment of the major tick-borne diseases in the United States.

Urinary tract infections can be found in either sex at any age. While the majority occur in adult females as acute cystitis, recurrent symptomatic bacteriuria, or asymptomatic bacteriuria, adult males with prostatitis or acute pyelonephritis and children with symptomatic urinary tract infections comprise a considerable portion of patients seen. Management in pregnant females or in males with indwelling catheters or before prostatic surgery presents special problems. The choice of drug and dosage schedule should vary according to the infecting agent and the clinical state of the patient.

From the viewpoint of prevention of nosocomial infections, the urinary indwelling catheter should be carefully managed. Under long term indwelling, prophylactic antimicrobial chemotherapy is no sense and generally bladder irrigation is also not necessary. In this paper, the characteristics of urinary tract infections due to MRSA and P. aeruginosa is described. The incidence of MRSA isolation from urine is increasing in recent several years. The distinction between infections and colonizations is important.

The most important nosocomial cardiac infections include nosocomial infective endocarditis on native and prosthetic valves, and nosocomial infections related to transvenous permanent pacemakers, implantable cardioverter-defibrillators and left ventricular assist devices. Although representing rare complications, they are of great importance because they are associated with high morbidity and mortality. Most of them are encountered in older-age groups, related to nosocomial invasive procedures performed within the preceding four to eight weeks of hospital admission. Nosocomial bacteraemia assoc…
