Gait dual-task cost in individuals with chronic low back pain and high avoidance or catastrophizing.

BACKGROUND: Gait is a complex motor skill requiring high-level cognitive resources such as attention. Chronic low back pain (cLBP) is associated with attention impairments that may impact gait. Fear-avoidance and pain catastrophizing in cLBP may additionally contribute to attention impairments that affect gait. METHODS: Forty-five participants with cLBP (age 37.0±11.5 (21-59), 64% female) completed a gait task with and without a concurrent auditory information processing choice reaction time task (IP-CRT). Dual-task cost (DTC) was computed for gait and cognitive performance metrics. Participa

Sleep-Related Infant Deaths: Updated 2022 Recommendations for Reducing Infant Deaths in the Sleep Environment.

Each year in the United States, ∼3500 infants die of sleep-related infant deaths, including sudden infant death syndrome (SIDS) (International Classification of Diseases, 10th Revision [ICD-10] R95), ill-defined deaths (ICD-10 R99), and accidental suffocation and strangulation in bed (ICD-10 W75). After a substantial decline in sleep-related deaths in the 1990s, the overall death rate attributable to sleep-related infant deaths has remained stagnant since 2000, and disparities persist. The triple risk model proposes that SIDS occurs when an infant with intrinsic vulnerability (often manifested

Antibiotic Use After Removal of Penicillin Allergy Label.

BACKGROUND: Penicillin allergy is commonly reported in the pediatric emergency department. We previously performed 3-tier penicillin allergy testing on children with low-risk symptoms, and 100% tolerated a penicillin challenge without an allergic reaction. We hypothesized that no serious allergic reactions would occur after re-exposure to penicillin and that prescription practices would change after testing. METHODS: We performed a follow-up case series of 100 children whose test results were negative for penicillin allergy. Research staff administered a brief follow-up phone survey to the pa

Different patterns of eye-specific input are relayed to overlapping populations in the mouse superior colliculus.

The mouse superior colliculus (SC) regulates critical behaviors such as prey capture and visual threat avoidance, each of which depend on binocular visual processing. In contrast to our previous understanding, recent work has demonstrated widespread and diverse binocular interactions in the mouse SC. The SC receives direct input from contralaterally and ipsilaterally projecting retinal ganglion cells (contra-and ipsi-RGCs), but the ways in which eye-specific information is filtered in the SC remain unclear. Here, we utilized a trans-synaptic, intersectional viral tracing strategy to label SC n

Value Added: Vicarious-Affirmation Strategies to Enhance Prediabetes Awareness Among High-Risk Hispanics.

Reducing the progression of type 2 diabetes mellitus hinges on accurate, timely recognition of prediabetes risk and the ability to make corresponding lifestyle changes. However, this critical step is often hindered by resistance to and avoidance of health information among high-risk individuals. Drawing on self- and vicarious-affirmation (VA) theory, this study experimentally tests a brief, culturally targeted, message-based vicarious-affirmation intervention among Hispanic adults at elevated risk (N = 1,039). Relative to standard messaging, the intervention did not improve primary outcomes; h

"There is nothing better than participating in this study": Living the PAPAartis cardiovascular randomised controlled trial.

Qualitative research can bring new dimensions of understanding decision-making process in clinical trials. Participating in a randomized clinical trial requires patients to accept complex information and make decisions in a context of uncertainty. It becomes especially complicated in the case of serious diseases in which the treatment itself implies unknown risks. This study examines these issues in the context of the PAPAartis randomized clinical trial, which aims to prevent spinal cord injuries that can occur as an adverse event following complex surgical repair of thoracoabdominal aneurysm.

Posttraumatic stress symptom clusters differentially predict late positive potential to cocaine imagery cues in trauma-exposed adults with cocaine use disorder.

BACKGROUND: While studies have investigated the effects of posttraumatic stress disorder (PTSD) symptoms on substance use, information on these associations in the context of drug cue reactivity is lacking, which can provide meaningful information about risk for relapse. The current study assessed the associations between PTSD symptom clusters and reactivity to cues in trauma-exposed adults with cocaine use disorder. METHODS: We recorded electroencephalogram on 52 trauma-exposed participants (Mage = 51.3; SD = 7.0; 15.4 % women) diagnosed with cocaine use disorder while they viewed pleasant (

Cybersecurity Hygiene in Health Care: Practical Steps for Clinicians to Protect Themselves and Their Organizations.

Cybersecurity has become a critical determinant of patient safety, business continuity, and organizational resilience in health care. Health care organizations are increasingly targeted by ransomware, data theft, and related attacks that disrupt clinical operations, threaten patient safety, delay care, and compromise sensitive information. Although often viewed as technical failures, many incidents originate in routine human behaviors, including phishing responses, credential misuse, unsafe downloads, and inappropriate use of personal devices or applications. This review focuses on clinician-l

Elevation of hilar mossy cell activity suppresses hippocampal excitability and avoidance behavior.

Modulation of hippocampal dentate gyrus (DG) excitability regulates anxiety. In the DG, glutamatergic mossy cells (MCs) receive the excitatory drive from principal granule cells (GCs) and mediate the feedback excitation and inhibition of GCs. However, the circuit mechanism by which MCs regulate anxiety-related information routing through hippocampal circuits remains unclear. Moreover, the correlation between MC activity and anxiety states is unclear. In this study, we first demonstrate, by means of calcium fiber photometry, that MC activity in the ventral hippocampus (vHPC) of mice increases w

The Role of Vertebral Body Tethering in Treating Skeletally Immature Scoliosis.

This is a critical analysis of a study by Hoernschemeyer et al, "Anterior Vertebral Body Tethering for Adolescent Scoliosis with Growth Remaining: A Retrospective Review of 2 to 5-Year Postoperative Results" (J Bone Joint Surg Am, 2020;102[13]:1169-1176), that assessed the clinical and radiographic outcomes of vertebral body tethering (VBT) in the treatment of adolescent scoliosis. The authors demonstrated successful treatment in 74% of patients, based on radiographic outcomes and avoidance of subsequent posterior spinal fusion. Nearly a quarter of patients required revision surgery. Almost ha

Does Psychosocial Ego Development Partially Mediate the Association Between Defense Mechanisms and Death Attitude?

Some authors suggest the existence of links between the mechanisms of defense and attitude toward death, however, mechanisms underlying these relationships are not well understood. The objective of the present research was to investigate whether psychosocial ego development is a partial mediator in the association between mechanisms of defense and death attitude. The study included participants (N = 382) aged 19-85 (M = 47.02, SD = 16.19). Three measures were used: Defense Style Questionnaire-40, Psychosocial Inventory of Ego Strengths, and Death Attitude Profile-Revised. Findings partly confi

Eighteen-year-old man with autism, obsessive compulsive disorder and a SHANK2 variant presents with severe anorexia that responds to high-dose fluoxetine.

The SHANK2 gene codes for a protein involved in organising the postsynaptic density and disruptions have been associated with autism spectrum disorders (ASDs). ASDs are frequently comorbid with intellectual disability and anxiety disorders and emerging evidence suggests potentially common aetiologies. Here, we report the case of an 18-year-old man with ASD who presented with severe anorexia due to fear of food contamination, food avoidance and stereotypies attributable to underlying obsessive compulsive disorder (OCD). The patient was found to be heterozygous for c.2518C>T (p.Pro840Ser), a lik

Chorda tympani nerve modulates the rat's avoidance of calcium chloride.

Calcium intake depends on orosensory factors, implying the presence of a mechanism for calcium detection in the mouth. To better understand how information about oral calcium is conveyed to the brain, we examined the effects of chorda tympani nerve transection on calcium chloride (CaCl(2)) taste preferences and thresholds in male Wistar rats. The rats were given bilateral transections of the chorda tympani nerve (CTX) or control surgery. After recovery, they received 48-h two-bottle tests with an ascending concentration series of CaCl(2). Whereas control rats avoided CaCl(2) at concentrations

Personal contextual characteristics and cognitions: predicting child abuse potential and disciplinary style.

According to Social Information Processing theory, parents' cognitive processes influence their decisions to engage in physical maltreatment, although cognitions occur in the context of other aspects of the parents' life. The present study investigated whether cognitive processes (external locus of control, inappropriate developmental expectations) predicted child abuse potential and overreactive disciplinary style beyond personal contextual factors characteristic of the parent (hostility, stress, and coping). 363 parents were recruited online. Results highlight the relative importance of the

Type I latex allergy: a follow-up study.

BACKGROUND: There are several studies that deal with the evolution of patients with occupational rhinitis/asthma as a result of immunoglobulin (Ig) E-mediated allergy to latex. However, none have focused on the course of this illness in non-occupational settings. OBJECTIVE: To ascertain patient compliance in individuals diagnosed with latex allergy with respect to following avoidance measures, as well as to determine the frequency and type of symptoms that emerged as a result of exposure to latex when receiving healthcare (surgery, gynecology, dentistry), as well as other sources of exposure.

Trajectories of fear avoidance behavior and recovery after mild traumatic brain injury: Findings from the Toronto Concussion Study.

OBJECTIVE: Fear avoidance behavior is associated with more severe postconcussion symptoms after mild traumatic brain injury (mTBI). However, it remains unclear when after injury fear avoidance behavior becomes a barrier to recovery. This study investigated changes in early fear avoidance behavior after mTBI and its associations with postconcussion symptoms. METHOD: Adults with concussion (N = 308) were prospectively recruited from an outpatient concussion clinic in Ontario, Canada. They completed measures assessing fear avoidance behavior (Fear Avoidance Behavior after Traumatic Brain Injury
