Tendon sheath giant cell tumor is an idiopathic proliferative and destructive disease of the synovium. It is rare and tends to arise in large joints, for example, knee and ankle, but it can also arise in temporomandibular joints (TMJs). Because of its destructive nature, immediate treatment upon diagnosis is recommended. Radical resection proved to be an excellent choice for superior local control. However, the unfavorable anatomic location of TMJ and infratemporal fossa tumor with intradural extension make such a resection impractical. Hereby, we reported a case of resection of a TMJ tendon s…

In a 76-year-old Caucasian male, the right middle meningeal was replaced by a branch of the ipsilateral ophthalmic artery (O), i.e. the sphe … from the mandibular nerve by a pterygospinosus muscle. This muscle was merely present in the right infratemporal fossa. Interestingly, the left AM originated from the posterior deep temporal artery. No other anatomical variations were found in this individual. Since the coexistence of an S with variations of other structures derived from the first branchial arch has …

Trigeminal neurinomas account for about 0.2 per cent of all intracranial tumors and for about 2 to 3 per cent of all intracranial neurinomas. Trigeminal neurinomas occur most frequently in persons in the middle decades of life, with the highest incidence between the ages of 38 and 40 years. They show no sex predilection. Although very rare, trigeminal neurinomas are of considerable interest to the neurotologist because of their relation to the temporal bone, skull base, sphenoid and maxillary sinuses, nasopharynx, infratemporal fossa, and, in rare instances, the oral cavity. In addition to pro…

OBJECTIVE: To demonstrate the utility and limitations of the extradural endoscopic-assisted anterior temporal fossa approach to the pterygopalatine fossa (PPF), infratemporal fossa (ITF), paranasal sinuses (PS), parapharyngeal region (PPR), nasal cavities (NC), epipharynx (EP), and clivus.
METHODS: A frontotemporal orbitozygomatic craniotomy is performed. The dura is elevated from the cavernous sinus (CS). The anterior temporal fossa floor is drilled. Foramen rotundum and ovale are opened. The PPF is exposed and the lateral margin of inferior orbital fissure (IOF) is removed. The anterolatera…

CONCLUSIONS: The 5 endoscopic approaches effectively followed the course of the trigeminal nerve with minimal invasiveness. These approaches could provide alternative options for the management of trigeminal schwannoma.

Malignant melanoma (MM) with multiple metastases (including the oral mucosa) is an extremely rare condition that is difficult to manage due to its complexity. This article presents the case of a 27-year-old man who first developed MM on the scalp, which subsequently metastasized to the mandible, parotid gland, infratemporal fossa, and the cervical regions of the larynx, kidneys, liver, and lungs. The findings of the present case report are compared with 31 other cases published in the English literature. Multiple metastases of MMs in the head and neck region are rare and generally are associat…

Metastasis of a tumour to the mandibular condyle is rare. We describe a case in which a patient presented with isolated condylar metastasis mimicking acute internal derangement of the temporomandibular joint (TMJ). Open biopsy revealed a lesion seated within the condylar head and extending through the bony cortex to the infratemporal fossa. Histopathology confirmed the diagnosis of metastatic large-cell lung carcinoma. TMJ dysfunction may be associated with any pathologic entity involving the mandibular condyle. A panoramic radiograph should be considered in patients who do not respond to conv…

Dislocation of the temporomandibular joint (TMJ) is most often spontaneous, but it can be associated with direct or indirect head and neck trauma. The disorder may be treated in general by simple closed techniques, if managed acutely. If the dislocation becomes chronic, however, open reduction is usually required. The article discusses various methods for an open surgical approach described in the literature. A case of a prolonged TMJ dislocation is presented, involving a severe bilateral mandibular dislocation with the condyles displaced into the infratemporal fossa and the lateral poles of t…

We discuss a case of lower lip carcinoma which presented with atypical symptoms; facial paralysis, conductive type hearing loss, and ophthalmoplegia. Due to an earlier resection, no mass was evident on the primary examination. Diagnostic imaging revealed a mass originating from the lower lip, the perineural spread of the tumor along the left inferior alveolar nerve to the left infratemporal fossa and the left foramen ovale. Through a retrograde course from the foramen ovale, the tumor extended the ipsilateral cavernous sinus, Meckel's cave, and cisternal portion of the CN V. This atypical spre…

The review briefly presents the history of development of navigation systems in neurosurgery. The idea of the existing principles underlying the navigation systems used in neurosurgery is given. Currently, the basic principles of navigation are optical and electromagnetic. Studies are presented comparing the accuracy of various navigation systems. Optical navigation demonstrates greater accuracy compared to electromagnetic, but both methods demonstrate a submillimeter error in the experiment. The history of use of navigation in the surgery of the skull base is analyzed in detail, the most rele

First bite syndrome (FBS) is an uncommon pain syndrome characterised by severe cramping or spasm in the parotid region with the first bite of each meal. The proposed pathogenesis is sympathetic denervation of the parotid gland secondary to iatrogenic injury with resultant cross-stimulatory parasympathetic hypersensitivity response. FBS is a potential sequela of surgeries involving the infratemporal fossa, parapharyngeal space and/or deep lobe of the parotid gland, however, only four cases of FBS secondary to carotid endarterectomy have been documented to date. We present a case and management …

Iatrogenic tooth displacement is a rare complication during extraction of impacted molars, but displacement of a maxillary third molar into the maxillary sinus, infratemporal fossa, buccal space, pterygomandibular space, and lateral pharyngeal space has been reported. Currently, 6 published reports describe third molar displacement into the lateral pharyngeal space, only 1 of which involved the loss of a maxillary third molar into this area, which occurred after an attempted self-extraction by the patient. There have been no reported cases of iatrogenic displacement of the maxillary third mola…

Rhinocerebral mucormycosis is an invasive, opportunistic fungal infection usually seen in immunocompromised patients, and particularly in th … the retroglobal space of the orbit, resulting in ocular signs. The facial soft tissues, palate and infratemporal fossa can be infected through connecting pathways from the pterygo-palatine fossa; therefore, debridement of the pterygopalatine fossa seems to be the definitive method of managing this infection.

INTRODUCTION: Ossification of the pterygospinous and pterygoalar ligaments has been well documented forming pterygospinous and pterygoalar bars. However, the actual ligaments have been rarely shown in the existing literature. Therefore, this study aimed to reveal the anatomy of the pterygoalar ligament/bar and pterygospinous ligament/bar, and its relationship with the branches of the mandibular nerve.
METHODS: Thirty sides from fifteen Caucasian fresh frozen cadaveric heads were used in this study. The branches of the mandibular nerve and any ligaments or bony bridges between the lateral plat

PURPOSE: The primary objective was to evaluate the influence of needle tip positioning during suprazygomatic maxillary nerve block on bupivacaine concentration within the pterygopalatine fossa (PPF). The secondary objective was to correlate early injectate spread on computed tomography (CT) with PPF bupivacaine concentrations.
METHODS: Suprazygomatic maxillary nerve block attempts (n=34) were performed on 17 human cadaveric heads with different needle tip positions. Each attempt involved injection of 5 mL containing 0.4% bupivacaine, iohexol, and methylene blue. Computed tomography was used t

BACKGROUND: In the last decade, endoscopic skull base surgery has significantly developed and generated a plethora of techniques and approaches for access to the cranial ventral floor. However, the exploration for the least-aggressive, maximally efficient approach continues.
OBJECTIVE: To describe in detail an anatomical study, along with the technical nuances of a novel endoscopic approach to Meckel's Cave (MC) using a lateral transorbital (LTO) route.
METHODS: Eighteen orbits of injected cadaveric specimens were operated on, using an endoscopic LTO approach to MC, middle cranial fossa, and
