A comparison of steroid dose with or without local anesthetic in lumbar interlaminar epidural steroid injections.

INTRODUCTION: Epidural steroid injections (ESIs) are commonly used as a treatment for lumbar radiculopathy. Currently, most research on comparative efficacy of various steroids in epidural steroid injections is focused on transforaminal ESIs (TFESIs). Through this study, we aimed to compare various steroid doses with or without local anesthetic in interlaminar ESIs (ILESIs). METHODS: We reviewed charts for all adult patients who received ILESIs identified by CPT code 62323 between January 2017 to April 2021. Baseline demographic data including age, sex, BMI, and smoking status were recorded.

Repeat Epidural Injections of SP-102 (Dexamethasone Sodium Phosphate Injectable Gel) in Subjects with Lumbosacral Radiculopathy.

PURPOSE: SP-102 is a novel epidural steroid injection (ESI) formulation of 10 mg dexamethasone sodium phosphate in a viscous gel solution. Repeat dosing of ESIs is possible if required for pain relief, but with consideration of hypothalamic-pituitary-adrenal (HPA) axis suppression from prolonged systemic exposure. This phase I/II study investigated the effect of initial and repeat SP-102 injections on HPA suppression and analgesia. METHODS: Subjects with lumbosacral radiculopathy received an initial epidural SP-102 injection (T1) on day 1, followed by a repeat injection (T2) on ≥28 days later

Persistent Singultus Associated with Lumbar Epidural Steroid Injections in a Septuagenarian: A Case Report and Review.

Singultus are rare but notable adverse effect of epidural steroid injections (ESIs). To date, reports of persistent hiccups associated with ESIs have been reported mostly in adults aged 65 years or older. We present the first case of persistent hiccups in a septuagenarian who underwent repeated transforaminal ESIs for chronic lumbar radiculopathy. Under fluoroscopic guidance, 1.5 mL of 1% lidocaine (preservative free) and 0.8 mL of dexamethasone solution (10 mg/mL) was injected into the bilateral L4-L5 neural foramen and epidural space. After the first epidural injection, episodes of singultus

CT-Fluoroscopic Cervical Transforaminal Epidural Steroid Injections: Extraforaminal Needle Tip Position Decreases Risk of Intravascular Injection.

BACKGROUND AND PURPOSE: Cervical transforaminal epidural steroid injections are commonly performed for temporary pain relief or diagnostic presurgical planning in patients with cervical radiculopathy. Intravascular injection of steroids during the procedure can potentially result in cord infarct, stroke, and even death. CT-fluoroscopy allows excellent anatomic resolution and precise needle positioning. This study sought to determine the safest needle tip position during CT-guided cervical transforaminal epidural steroid injection as determined by the incidence of intravascular injection. MATE

The effectiveness of lumbar interlaminar epidural injections in managing chronic low back and lower extremity pain.

BACKGROUND: Intervertebral disc herniation, spinal stenosis, intervertebral disc degeneration without disc herniation, and post lumbar surgery syndrome are the most common diagnoses of chronic persistent low back and lower extremity symptoms, resulting in significant economic, societal, and health care impact. Epidural injections are one of the most commonly performed interventions in the United States in managing chronic low back pain. However the evidence is highly variable among different techniques utilized - namely interlaminar, caudal, and transforaminal - and for various conditions, nam

Epidural steroid injections are useful for the treatment of low back pain and radicular symptoms: con.

Lumbar epidural steroid injections are commonly performed in the United States for treating radicular low back pain. However, the best available data suggest that the benefit afforded by these injections is quite limited; in fact, new data suggest that in geographic areas where many such injections are performed, more and not fewer spine surgeries are actually completed annually. We suggest that further high-quality studies are required and their results respected through their implementation in daily practice to better ensure that only appropriate patients are advised to undergo this procedur

The Infraneural Approach to Lumbar Transforaminal Epidural Steroid Injections: A Practical Review for Trainees and Interventionalists.

Lumbar transforaminal epidural steroid injections (TFESIs) are commonly performed for the treatment of lumbosacral radicular pain related to disc herniation, foraminal stenosis, lateral recess stenosis, and degenerative spinal conditions. The conventional supraneural or subpedicular approach targets the superior aspect of the neural foramen, historically described as the "safe triangle." This is what is commonly taught in pain medicine fellowships. However, this region may contain radicular or radiculomedullary arteries, and inadvertent intra-arterial injections have been implicated in rare bu

Comparison of the effectiveness of thoracic epidural steroid and local anesthetic injections: A randomized controlled trial.

This study primarily aims to evaluate the effectiveness of thoracic interlaminar epidural injections in alleviating pain and enhancing function in patients with chronic mid and/or upper back pain. One hundred patients were randomly divided into 2 groups, 50 patients in each. Group I received only a local anesthetic, whereas Group II received a combination of local anesthetic and steroids. The random assignment to either Group I or Group II was determined using a simple computer-generated sequence. The results were evaluated using the numeric rating scale and the revised Oswestry Disability Ind

Evaluating the Effectiveness of Epidural Steroid Injections in Relieving Pain in a Single-Center Retrospective Cohort.

Background Epidural steroid injections (ESIs) are a common conservative treatment for mitigating radicular pain and are often used to relieve pain, increase function, and improve mobility. However, their efficacy and duration of pain relief are relatively unclear because of the variability in clinical indications, injection techniques, injection mixtures, the number of allowable injections, and the lack of standard and objective outcome measures in the literature. Objectives This study aimed to characterize the effectiveness of ESIs in improving pain, measured with numerical rating scale (NRS)
