Overview of cases and prevalence of jaundice in neonatal intensive care unit.

This study was undertaken to know the pattern of jaundice prevalent among the babies admitted at the Neonatal Intensive Care Unit (NICU) of the B. P. Koirala Institute of Health Sciences (BPKIHS). A total of 293 neonates including 201 (68.6%) males and 92 (31.4%) females were admitted over a period of one year (15th June 2001 to 14th June 2002). Prematurity (30.0%), birth asphyxia (29.0%), neonatal septicemia (25.9%) and respiratory distress (23.9%) were the most common reasons for admission to the NICU. There were 42 cases of neonatal jaundice, among which babies born to primigravidae (59.5%)

Neonatal intensive care unit characteristics affect the incidence of severe intraventricular hemorrhage.

OBJECTIVES: The incidence of intraventricular hemorrhage (IVH), adjusted for known risk factors, varies across neonatal intensive care units (NICU)s. The effect of NICU characteristics on this variation is unknown. The objective was to assess IVH attributable risks at both patient and NICU levels. STUDY DESIGN: Subjects were <33 weeks' gestation, <4 days old on admission in the Canadian Neonatal Network database (all infants admitted in 1996-97 to 17 NICUs). The variation in severe IVH rates was analyzed using Bayesian hierarchical modeling for patient level and NICU level factors. RESULTS:

[Fungal colonization of newborn in neonatal intensive care unit: a prospective study].

Invasive candidiasis in neonates has become an increasing problem over the past decade in Neonatal Intensive Care Units (NICUs) in the world; it is a relatively common cause of late onset sepsis associated with a high mortality. Prior colonization is the major risk factor for candidemia. To determine the rate of colonization, risk factors and the possible modes of acquisition of Candida spp. in neonates in NICU of Pediatric University Hospital, Sarajevo, during 3 months. Samples (mucocutaneous swabs, urine, stool, blood) were obtained weekly until the time of discharge or death (one infant enr

[Comparison between different mortality risk scores in a neonatal intensive care unit].

OBJECTIVES: To evaluate and compare birthweight and scores as predictors of neonatal mortality in a Neonatal Intensive Care Unit (NICU). METHODS: The survey included 494 newborns admitted to the neonatal intensive care unit (NICU) of a general hospital in Porto Alegre, southern Brazil, immediately after delivery, between March 1997 and June 1998. Birthweight and scores were evaluated in terms of the variable "death while in NICU". Exclusion criteria were: discharge or death less than 24 hours after admission, admission not immediately following delivery, incomplete study protocol, and congeni

Nosocomial infections in a neonatal intensive care unit: incidence and risk factors.

BACKGROUND: Nosocomial infections (NIs) have become a matter of major concern in neonatal intensive care units (NICUs). The objectives of this study were to determine the incidence rate and the most frequent sites of infection in a Brazilian NICU from January 1999 to March 2000 and to study the risk factors for NIs. METHODS: A cohort study was carried out in which 225 neonates who remained at least 24 hours in the NICU were followed-up; neonates with NIs were identified, and the presence of risk factors was studied. Results were submitted to chi(2) distribution. RESULTS: The incidence rate a

Uses and abuses of sodium bicarbonate in the neonatal intensive care unit.

Despite the lack of evidence for its effectiveness in the treatment of acid-base disturbances in critically ill patients of all ages, and despite several lines of evidence that indicate it might be dangerous, bicarbonate therapy is used routinely in many neonatal intensive care units. The justification for the persistent use of this controversial therapy comes from a variety of sources, many based more in philosophy than in science. Clinicians contemplating the use of bicarbonate therapy should consider what they expect the intervention to accomplish and what evidence exists that their therape

[The examinations of hearing in infants treated in neonatal intensive care units].

The aim of the study was the estimation of hearing and the analysis of the risk factors for hearing loss in infants treated in neonatal intensive care units. The examinations were performed in 287 infants, who we examined in ENT Department between the ages of three and six months after discharge from neonatal intensive care unit. The infants were investigated with the use of ABR elicited with clicks and tone burst of 500 and 1000 Hz. Elevated auditory thresholds were found in 48 neonates in the first examination and in 27 in the second examination. Among the risk factors we observed the most f

Septicaemia in an Austrian neonatal intensive care unit: a 7-year analysis.

The results of blood cultures and clinical data of 101 neonates with 110 episodes of septicaemia during a 7-y study period were reviewed. The overall incidence of culture-proven sepsis within the study period was 6.0 per 100 neonatal intensive care unit admissions and the mortality rate was 14%. Three groups of pathogens accounted for 70% of all isolates: coagulase-negative staphylococci (27%), aerobic Gram-negative rods (24%) and Enterococcus faecalis (19%). Group B streptococcus was the major pathogen of very early-onset septicaemia (within 24 h of birth), whereas late-onset infections were

Retinopathy of prematurity in a Danish neonatal intensive care unit, 1985-1991.

During the 7 year period 1985 to 1991, 170 infants born in Hvidovre Hospital, Denmark, with birthweight < or = 1500 g and gestational age < or = 32 completed weeks survived at least 8 weeks or more and had eye examinations carried out. Forty-five infants had ophthalmoscopic evidence of retinopathy of prematurity (ROP). Eighteen developed blindness or severely reduced vision and 6 developed unilateral blindness. In 21 the ROP changes regressed. Eight infants eventually developed severe myopia. The 45 infants with ROP were compared with the 125 without ROP. There was no difference in birth weigh

Rate of bronchopulmonary dysplasia as a function of neonatal intensive care practices.

Some differences among neonatal intensive care units (NICUs) in incidence of bronchopulmonary dysplasia may reflect variations in medical care practices. After adjusting for differences in the inherent risk of bronchopulmonary dysplasia among 223 infants of less than 1751 gm birth weight who were admitted to three Harvard-affiliated NICUs, we used multivariate analysis to explore the extent to which medical care practices during the first days of life varied with the rate of bronchopulmonary dysplasia. In our analyses, variables were grouped by three major hypotheses: oxygen toxicity, barotrau

Bacteremia detected by lysis direct plating in a neonatal intensive care unit.

The density of bacteremia was determined in 787 neonatal blood specimens by using the 1.5-ml Isolator microbial tube. Coagulase-negative staphylococci were the organisms isolated most frequently from both true-positive cultures (25 of 50) and contaminated cultures (57 of 131). Based on the first positive culture in an episode of sepsis, there were no cases of coagulase-negative staphylococcal sepsis associated with counts of less than or equal to 5 CFU/ml. Indwelling intravascular lines were associated with the majority of the episodes of sepsis. The distribution of pathogens causing sepsis in

Anaerobic bacteremia in a neonatal intensive care unit: an eighteen-year experience.

A review of anaerobic bacteremia in the Neonatal Intensive Care Unit identified 29 episodes of clinically significant bacteremia occurring over the past 18 years. This experience suggested that certain clinical settings were associated with specific anaerobic infections. Although Gram-positive and Gram-negative anaerobes were isolated with similar frequency, 8 of 12 infants bacteremic within the first 48 hours of life were infected with Gram-positive, penicillin G-susceptible organisms whereas 11 of 17 infants older than 2 days were bacteremic with Gram-negative, penicillin G-resistant anaerob

The risks of umbilical vessel catheterization in a neonatal intensive care unit.

Five hundred and fourteen high-risk neonates who had indwelling umbilical catheters at the neonatal intensive care unit of the University of Benin Teaching Hospital were studied. of these 514 neonates, 122 (23.8%) had their catheters in-situ for longer than 24 h. Of the 122, fifty-four (44%) had positive bacterial cultures from their catheter tips. Seven (5.7%) and four (3.2%) of the 122 neonates studied developed septicaemia and necrotizing enterocolitis respectively. Catheterization for periods in excess of 48 h significantly increased the risk of bacterial colonization. Malposition of umbil

Recognition and Management of Delirium in the Neonatal Intensive Care Unit: Case Series From a Single-Center Level 4 Intensive Care Unit.

Delirium often goes unrecognized in neonates and children because of lack of experience in evaluating behavior and cognition, insufficient awareness of the prevalence, and nondistinctive symptoms in this population. Although there are increasing reports of the presence of delirium in neonates, there are few data to guide the pharmacologic treatment in this population. In this retrospective single-center case series, we present our experience using quetiapine to treat delirium in 9 medically complex neonates. Based on an extensive literature review, expert opinion, and institutional experience,

Parents' Adaptation Process in Providing Care to Preterm Neonates in the Neonatal Intensive Care Units in Rwanda: A qualitative study.

BACKGROUND: Preterm birth remains a global health issue, and prematurity poses a significant health risk to neonates and often requires prolonged care in the Neonatal Intensive Care Unit (NICU). This experience can be overwhelming for parents, who must adapt emotionally and practically to the demands of caring for their fragile infants. Investigating parental adaptation processes in caring for preterm neonates is essential for designing effective parental involvement in the Neonatal Intensive Care Unit. OBJECTIVE: This study aimed to explore parents' adaptation process in caring for preterm n

An Evaluation of Vital Signs in Intubated Neonates Undergoing Lung Point-of-Care Ultrasound in the Neonatal Intensive Care Unit.

PURPOSE/AIMS: Point-of-care ultrasound (POCUS) within the neonatal intensive care unit (NICU) is an emerging field that is being utilized with increasing frequency. Lung POCUS is employed with multiple validated uses, which include determining the need for surfactant in respiratory distress syndrome and the timing of appropriate extubation in neonates receiving mechanical ventilation. Previous data showed cardiorespiratory stability with other indications of POCUS, though it remains unclear whether this is observed in lung POCUS exams in intubated neonates. Our goal is to evaluate the cardiore
