Apple Peel Deformity and Malrotation of Gut: Autopsy Findings of a Rare Cause of Mortality in Utero.

One-third of all intestinal obstructions in the newborn are caused by atresias. The most common site is the duodenum followed by jejunoileal and colonic locations. Herein we report the autopsy findings of a rare case of jejunoileal atresia associated with malrotation of gut. Autopsy performed on a 36 weeks old male fetus still birth, born of a non-consanguineous marriage, demonstrated jejunoileal atresia with apple peel deformity and malrotation of gut. Although the diagnosis was established in the prenatal period, in utero fetal demise occurred before definitive surgical intervention could be

Incidence of congenital complications related to COVID-19 infection during pregnancy.

PURPOSE: Infection with COVID-19 during pregnancy has been associated with a hypercoagulable state. It is unknown if maternal COVID-19 infection results in congenital anomalies secondary to intrauterine vascular accidents. This study sought to determine if the rate of in-utero vascular complications (intestinal atresia and limb abnormalities) that may be attributable to the hypercoagulable states associated with COVID-19 and pregnancy increased after the onset of the pandemic. METHODS: Pregnancy, neonatal, and congenital defect data from a single academic medical center and the partner's chil…

A rare case of idiopathic intrauterine intestinal volvulus complicated with intestinal perforation: a case report from Syria.

Fetal intestinal volvulus is rare, but it is a serious condition due to its life-threatening complications. The bowel loop becomes twisted; thus, impaired venous return leads to bowel necrosis. Prenatal volvulus is most secondary to intestinal atresia, arterial supply defect or without any underlying cause, with consideration that cystic fibrosis is the cause of the intestinal obstruction, because of meconium ileus. We report a case of prenatal volvulus complicated with intestinal perforation and meconium peritonitis in the context of meconium ileus.

Microdissection study of the myenteric plexus in acardia, ataxia-telangiectasia, cystic fibrosis, extrahepatic biliary atresia, pediatric AIDS and Werdnig-Hoffmann disease.

Microdissection-point count morphometric study of the myenteric (Auerbach) plexus or esophagus, small intestine, and colon was done for infants and children with acardia (2), ataxia-telangiectasia (5), cystic fibrosis of the pancreas (CFP) (25), extrahepatic biliary atresia (EBA) (17), pediatric AIDS (10), and Werdnig-Hoffmann disease (WHD) (8). Values for fractional area of neural tissue in the plane of the plexus were compared to those of control patients in same age range as those in each disease category by t-test. Statistically abnormal values included low values for small intestine and c

Congenital absence of the superior mesenteric artery.

Atresias of the intestinal tract are one of the causes of congenital intestinal obstruction. The superior mesenteric artery supplies the midgut from the fourth portion of the duodenum to the midportion of the transverse colon. Absence of the superior mesenteric artery (SMA) with subsequent absence of jejunum, ileum, appendix, and right colon is a previously unreported condition. A fetal vascular accident might result in this intestinal catastrophe.

A case report of laparoscopic duodenal atresia repair in a neonate using a novel miniature stapling device.

INTRODUCTION: Laparoscopic duodenal atresia repair is an advanced procedure performed in select pediatric surgical centers. Recently, sub-6mm endosurgical staplers have been introduced, facilitating and accelerating the creation of intracorporeal intestinal anastomoses. PRESENTATION OF CASE: We performed a laparoscopic duodenojejunostomy in a one-day-old child with duodenal atresia due to annular pancreas using a novel 5.8mm articulating endostapler with excellent outcome. The technical details are reported. DISCUSSION: Laparoscopic duodenojejunostomy is a technically demanding procedure due

Epidermolysis bullosa with pyloric atresia: Report of two cases in consecutive siblings.

Pyloric atresia is a rare digestive malformation. It represents about 1% of intestinal atresias and is associated with some other genetic or anatomical alteration in 55% of the cases. In 20% of them, it is associated with epidermolysis bullosa, which is described as an established syndrome with a bad prognosis. We present two cases of consecutive siblings with this condition and fatal outcomes in both of them. We made a review of the literature and discussed the main topics.

Atresia of the Transverse Colon (Grosfeld Type IIIa) Diagnosed at Seven Days of Life: A Case Report.

Colonic atresia is a rare cause of neonatal intestinal obstruction, and involvement of the transverse colon is particularly uncommon. We report a seven-day-old term male neonate with failure to pass meconium since birth, bilious vomiting, abdominal distension, and jaundice. Plain abdominal radiography demonstrated diffuse bowel-loop dilatation with relative paucity of distal gas. Exploratory laparotomy revealed atresia of the transverse colon (Grosfeld type IIIa), with a markedly dilated proximal colon, an atrophic distal colon, and a V-shaped mesenteric defect. A right hemicolectomy was perfo

Diagnostic value of the microcolon using ultrasonography in small bowel atresia.

BACKGROUND: Microcolon helps diagnose small bowel atresia (SBA) using contrast enema. However, there are no ultrasonography (US) microcolon criteria for diagnosing SBA. Therefore, this study aimed to evaluate colon accuracy and other characteristics for diagnosing SBA by US, using surgical or clinical information as the reference standard. METHODS: US was performed on 46 neonates aged ≤ 7 days old. In the study group (n = 15), neonates with SBA were confirmed following surgery. In the study group without SBA (n = 15), neonates with other gastrointestinal problems besides SBA were confirmed by

Radiologically suspected gastric volvulus associated with non-hypertrophic pyloric stenosis and omphalomesenteric band-induced ileal volvulus with atresia in a neonate.

This report describes a term neonate with multiple gastrointestinal issues: non-hypertrophic pyloric stenosis (NHPS)-induced gastric volvulus (GV) and ileal volvulus with atresia due to an omphalomesenteric band. The patient presented with early non-bilious vomiting and abdominal distension. Radiological studies indicated GV, gastric outlet obstruction and ileal atresia. An exploratory laparotomy confirmed terminal ileal atresia with volvulus from the fibrous band and NHPS, but no GV was found during surgery. Treatment involved resecting the terminal ileum, ileocaecal valve and caecum, followe

Magnetic compression anastomosis for rectal atresia following necrotizing enterocolitis: A case report.

RATIONALE: Rectal atresia caused by necrotizing enterocolitis (NEC) is a serious and rare complication in children. Magnetic compression anastomosis (MCA) has been effectively applied in children with congenital oesophageal atresia and biliary atresia. Herein, we reported a case of successfully application of MCA in an infant with rectal atresia following NEC. PATIENT CONCERNS: A 30 weeks premature birth female fetal infant was transferred to our neonatal intensive care unit due to premature delivery, low birth weight, and neonatal respiratory distress. On postpartum day 11, the infant develo

Esophageal atresia and tracheo-esophageal fistula: a review.

The improved survival of neonates with esophageal atresia and tracheo-esophageal fistula reflects the advancement in neonatal care and anaesthesia over the years. Chick embryo studies have given new insights in the embryopathy of esophageal atresia. It is now apparent that the various types of esophageal atresia could be explained due to selective discrepancy in the growth of the 3 folds in the region of tracheo-esophageal separation. The early disturbances in organogenesis which result in esophageal atresia also lead to other associated anomalies, the incidence of which varies from 40 to 55%.

Ileal duplication cyst linked to type 3A ileal atresia: a rare case report and review of the literature.

BACKGROUND: Jejuno-ileal atresias represent a significant cause of neonatal intestinal obstruction, occurring in approximately 1 in 3000-5000 live births. Terminal ileal atresia, classified as type 3A according to the Louw and Barnard system, specifically involves a mesenteric defect. Ileal duplication cysts, typically cystic or tubular structures sharing a common muscular wall and blood supply with the native ileum, can be communicating or non-communicating and pose risks of obstruction, volvulus, bleeding, or perforation. This case highlights terminal ileal atresia (type 3A) concurrent with

[X-ray diagnosis of rare esophageal diseases in children].

Clinical and X-ray studies of 300 children aged hours to 14 years who had abnormalities of the esophagus have revealed its atresia with or without tracheoesophageal fistulas in 77 subjects, that with congenital narrowings in 21. It was proved that some varieties of atresias might be roentgenologically detected and comprehensive studies should be carried out to determine fistulas in the completely patent esophagus. Despite the age, all children with esophageal atresia with a lower fistula were found to develop catarrhal and erosive esophagitis and "corrosive" pneumonia. Hamartia and recanalizat

Biliary atresia: 50 years after the first kasai.

Biliary atresia is a rare neonatal disease of unknown etiology, where obstruction of the biliary tree causes severe cholestasis, leading to biliary cirrhosis and death in the first years of life, if the condition is left untreated. Biliary atresia is the most frequent surgical cause of cholestatic jaundice in neonates and should be evoked whenever this clinical sign is associated with pale stools and hepatomegaly. The treatment of biliary atresia is surgical and currently recommended as a sequence of, eventually, two interventions. During the first months of life a hepatoportoenterostomy (a "K

Emergency operations in the newborn.

With the present-day development and understanding of anesthetic methods, fluid and electrolyte therapy, antibiotic medications and pediatric care, many congenital anomalies once uniformly fatal are now being successfully treated by emergency operations in the neonatal period. The eight most common of these which demand emergency operation in the immediate postnatal period are esophageal atresia and tracheoesophageal fistula, diaphragmatic hernia with dislocation of the abdominal viscera into the chest, malrotation of the intestine with obstruction, intestinal atresia, meconium ileus, imperfor…
