Robotic Arm-Assisted Total Knee Arthroplasty for Osteoarthritis Associated with Osteopetrosis: A Case Report.

CASE: A 74-year-old woman with osteopetrosis presented with severe right knee pain and difficulty walking due to osteoarthritis. She underwent robotic arm-assisted total knee arthroplasty (RA-TKA). Although the saw blade did not require replacement during surgery, a fracture associated with the tibial checkpoint occurred. The error of implant placement from the intraoperative plan was within 3.4°. After 1 year, the Forgotten Joint Score was 77, and she was independent in activities of daily living. CONCLUSION: RA-TKA allows safe and precise bone resection, achieving appropriate alignment even

Two Neighboring Spinal Dural Arteriovenous Fistulas with Multiple Bridging Veins Drainage.

Two neighboring spinal dural arteriovenous fistulas (SDAVFs) with multiple bridging veins drainage are extremely rare. Here, we report a 55-year-old man with 2 neighboring SDAVFs at the levels of T4-T5 supplied by the right T5 intercostal artery (common stem of T4 and T5 arteries) with multiple draining veins. Intraoperatively, 3 draining bridging veins between T4 and T5 nerve roots were identified and resected successfully. This case demonstrated the complex microscopic angioarchitecture features of 2 neighboring SDAVFs with multiple draining veins. Although these complex SDAVFs are extremely

Total Knee Arthroplasty Using Computer-Assisted Navigation in a Patient with Nail-Patella Syndrome: A Case Report.

CASE: Nail-patella syndrome (NPS) is a genetic disorder causing anatomical abnormalities about the knee, including significant patellar hypoplasia. We present a case of a patient with NPS and severe knee osteoarthritis undergoing computer-assisted total knee arthroplasty (TKA). Several intraoperative anatomical challenges were appreciated. Postoperatively, the patient developed arthrofibrosis requiring manipulation; however, his final outcome was favorable. CONCLUSION: Computer assistance may improve precision in patients with NPS undergoing TKA, but surgeons must be aware of the associated a

Management of Patella Fracture Nonunion and Large Bone Defect After Bone-Patellar Tendon-Bone Autograft: A Case Report.

CASE: A 32-year-old patient was diagnosed with a vertical patella fracture nonunion after a bone-patellar tendon-bone (BTB) anterior cruciate ligament reconstruction. In addition, a 1 × 2 × 1 cm patellar bone defect was noticed at the graft harvesting site. The patient was treated surgically with open reduction and internal fixation and iliac crest bone autograft which resulted in fracture union. CONCLUSION: Many intraoperative and postoperative risk factors for iatrogenic patella fracture when harvesting BTB autograft were identified. Surgeons should be aware of technical skills needed to pr

Postoperative episodic left bundle branch block.

Transient left bundle branch block (LBBB) associated with physical exertion has been described in patients with and without coronary artery disease. A 64-year-old woman with no history of coronary artery disease underwent Nissen fundoplication under general anesthesia. Preoperatively, an exercise-tolerance test revealed LBBB, without ischemic symptoms. Intraoperatively, the electrocardiogram revealed normal sinus rhythm. Postoperatively, LBBB appeared in the absence of clinical symptoms or physiologic stress. The patient reported several similar episodes previously. Cardiac enzymes were negati

Myometrial involvement of tumor-like cystic endosalpingiosis: a rare entity.

Endosalpingiosis is characterized by the presence of benign glands lined by tubal-type epithelium involving the pelvic and lower abdominal peritoneum and pelvic and para-aortic lymph nodes in women. Rarely, cystification can occur, resulting in a neoplasm-like mass associated with clinical manifestations, an intraoperative abnormality, or a striking finding on gross examination. Here we report the transvaginal ultrasound, magnetic resonance imaging and histopathological appearance of an unusual case of cystic endosalpingiosis involving the right ovary and full thickness of the wall of the uter

[Echinococcoses of the human. Diagnosis and therapy].

The clinical symptoms of hydatid disease result from the suppression and damage of the parenchyma of liver or lung. They resemble those of neoplastic diseases but show comparatively slower progression. For the diagnosis of cysts serological procedures, sonography, CT-scan and laparoscopy are discussed. One should be aware that exploratory puncture might be dangerous because of the possibility of generalized spread. Among 17 patients referred to Erlangen University Hospital with echinococcus cysticus the correct diagnosis was made in only 9 patients whereas the diagnosis on admission was correc

Exploratory study of electroencephalogram markers of responsiveness to surgical noxious stimuli under propofol anaesthesia.

BACKGROUND: While general anaesthesia typically induces unconsciousness, some patients retain the capacity to respond behaviourally to noxious stimulation. Reduced frontal alpha (8-12 Hz) power has been proposed as a marker of arousal, but its clinical reliability remains inconsistent. This exploratory study aimed to identify multichannel EEG spectral and connectivity markers associated with intraoperative behavioural responsiveness around noxious stimulation. METHODS: Sixty-four-channel EEG was recorded intraoperatively from seven patients undergoing microlaryngoscopy under propofol anaesthe

ED50 for intravenous midazolam-induced amnesia and its duration in surgical patients.

BACKGROUND: Intraoperative awareness is a serious adverse event under general anesthesia. Midazolam has a good anterograde amnesia-inducing effect, can prevent and reduce the occurrence of intraoperative awareness. However, if the dosage of midazolam is improperly controlled, it may not produce forgetting effect, or bring obvious adverse side effects, such as respiratory depression, and delay of recovery. However, the half maximal effective dose (ED50 ) of midazolam for amnesia, the duration of amnesia and the factors affecting the duration of amnesia are still inconclusive. Therefore, it is o…

Acute Intraoperative Airway Emergency During Submucosal Tunnel Endoscopic Resection for an Esophageal Lesion: A Report of an Unusual Case.

BACKGROUND Endoscopic gastrointestinal surgery rarely leads to airway compromise, but when mediastinal emphysema or pneumoperitoneum caused by perforation occurs, it can result in acute respiratory failure under anesthesia. Since the underlying mechanisms differ from those of typical pulmonary causes, reporting such rare cases is valuable for raising awareness among anesthesiologists and clinicians, and for emphasizing the importance of understanding surgical progress and performing timely physical examinations. CASE REPORT A 60-year-old woman with a submucosal esophageal mass underwent submuc

Magnetic Apposition across the Epiglottis: Radiographic and Operative Correlation of a Rare Hypopharyngeal Foreign Body.

UNLABELLED: Background and Aim. Rare-earth magnet ingestions are a subset of foreign body ingestions and can result in significant morbidity secondary to pressure necrosis. These magnets are best visualized radiographically, typically located in the gastrointestinal tract. However, unusual locations of magnetic adherence may include the hypopharynx along the epiglottis, where only 2 previous cases have been reported. Clinicians should be aware of the potential dangers of rare-earth magnet ingestion and consider atypical locations of attachment in the appropriate clinical setting. Case Presenta

Continuous Laryngeal Adductor Reflex Versus Intermittent Nerve Monitoring in Neck Endocrine Surgery.

OBJECTIVE: Intraoperative neuromonitoring (IONM) techniques aim to identify and potentially prevent nerve injury during surgeries. Prior studies into the efficacy of recurrent laryngeal nerve (RLN) IONM convey mixed results, with some claiming equivalence between IONM and no monitoring at all. The goal of the current study was to compare continuous RLN monitoring using the laryngeal adductor reflex (LAR) to intermittent RLN monitoring (intermittent IONM) to determine whether continuous monitoring reduces the incidence of intraoperative RLN injury during neck endocrine surgeries. METHODS: In t

Recipient Site Necrosis Following Hair Transplantation: A Rare Event.

Hair transplantation is generally considered a safe and effective procedure for androgenetic alopecia, with rare severe complications. Scalp necrosis is an uncommon but serious adverse event. The authors report a case of a 47-year-old male who developed parietal scalp necrosis following follicular unit extraction (FUE) with sapphire blade recipient-site creation in another country. By postoperative day 5, he experienced progressive scalp pain and developed a well-demarcated necrotic eschar. Initial topical therapy was insufficient, and surgical debridement with partial approximation of the wou

Haemophilic arthropathy of the knee: a surgeon's nightmare.

Surgical management of knee synovitis secondary to mild haemophilia, without any significant previous medical history and an adverse family history of haematological disorders, is arduous. Due to its rare occurrence, the diagnosis is often delayed or sometimes missed, leading to the grave and often lethal consequences in intraoperative and postoperative periods. Hardly isolated knee arthropathy due to mild haemophilia has been reported in the available literature. In this report, we present the management of a case of a 16-year-old male with isolated knee synovitis with undiagnosed mild haemop

A Rare Case of Small Bowel Obstruction in a Patient with Endosalpingiosis, Fitz-Hugh-Curtis Syndrome, and Chlamydia trachomatis Pelvic Inflammatory Disease.

A 19-year-old female has multiple presentations to emergency department with recurrent abdominal pain. During her third presentation, the radiological features were suggestive of high-grade small bowel obstruction in a virgin abdomen. A diagnostic laparoscopy has been performed. The intraoperative findings include a band adhesion between omentum and small bowel mesentery, and perihepatic adhesions consistent with Fitz-Hugh-Curtis syndrome. The histopathology from a biopsy of the macular lesions of the abdominal wall showed endosalpingiosis. The postoperative high vaginal swab was positive for

Nonconvulsive status epilepticus associated with Alzheimer's disease mimicking symptomatic focal epilepsy following the resection of a frontal parasagittal meningioma.

BACKGROUND: Epilepsies are frequent in patients with Alzheimer's disease (AD); however, epilepsies in AD can easily go unrecognized because they usually present as focal impaired awareness seizures or nonconvulsive status epilepticus (NCSE) and can overlap with other symptoms of AD. CASE DESCRIPTION: We performed an epilepsy surgery in a 69-year-old woman with progressive cognitive impairment and consciousness disorder, who was diagnosed with focal NCSE related to the resected meningioma in the right frontal parasagittal region. Sleep apnea syndrome in patients undergoing total joint arthroplasty.

Sleep apnea syndrome (SAS) is a condition of repeated episodes of apnea and hypopnea during sleep. It can cause life-threatening morbidities, including cardiac arrhythmia and ischemia, hypertension, and respiratory arrest, and even death. In a retrospective study at our institution of patients who underwent hip or knee total joint arthroplasty (TJA) with a diagnosis of SAS, we hypothesized that avoiding factors that exacerbate SAS in the perioperative period would minimize adverse outcomes. There were 19 patients with a preoperative diagnosis of moderate or severe SAS; 15 patients received con

Comparison of midazolam and diazepam to supplement total intravenous anaesthesia with ketamine for endoscopy.

Fifty patients undergoing endoscopy (laryngoscopy, bronchoscopy, mediastinoscopy) were anaesthetised in a double-blind prospective trial using total intravenous anaesthesia. Half of the patients were anaesthetised with an infusion of a solution of 250 mg ketamine and 12.5 mg midazolam. The other patients received an infusion of a solution of 250 mg ketamine and 20 mg diazepam. In addition, both groups were given increments of 50-100 micrograms of fentanyl. The immediate awakening time (t1) was not significantly different between groups, but the patients who had received midazolam-ketamine, had
