Choledochocele with obstructive jaundice: a case report and a review of the Japanese literature.

A case of a 57-year-old farmer with a rare type of choledochal cyst (choledochocele; Alonso-Lej's type III) is described. The patient was admitted because of obstructive jaundice and acute biliary infection. Abdominal computed tomography scan showed a cystic lesion in the head of the pancreas, and endoscopic retrograde cholangiopancreatography disclosed cystic dilatation of the terminal portion of the common bile duct. It was suspected that the choledochocele could swell and compress the common bile duct, causing obstructive jaundice and acute cholangitis; therefore, it was surgically resected

[Obstructive jaundice caused by metastasis to the head of the pancreas by a malignant melanoma].

We report on a 68-year-old patient who developed an obstructive jaundice 13 years after surgery of a malignant melanoma of the right ear. The initial sonographic and CT diagnosis of a benign pseudocyst of the head of the pancreas had to be revised based on ERCP, sonographically guided puncture and papillary biopsy. By Whipple's procedure, the solitary intrapancreatic metastasis could be successfully removed.

Lipoprotein-X and other tests in the diagnosis of obstructive jaundice in the infant.

The infant with elevated direct-reacting bilirubin levels requires an early specific diagnosis to identify those who would require early surgical intervention and those in whom the bilirubin levels will eventually return to normal. This study compares the accuracy of three tests: the serum lipoprotein-X (LP-X), the I131 rose bengal (IRB) excretion and the serum alpha-fetoprotein (AFP) in making a specific diagnosis in 15 patients. When used individually the accuracy of the tests varies from 56-100%. The LP-X and IRB excretion are more specific and when in agreement are 100% acurate in the diag

[Obstructive jaundice and acute pancreatitis due to an obstruction of the afferent loop after billroth-II-resection].

An obstruction of the afferent loop after Billroth-II-resection is an extremely rare late complication of this procedure. We report on a 76-year-old female patient with a history of Billroth-II-resection 11 years ago who was admitted due to acute pancreatitis and obstructive jaundice. Abdominal sonography lead to the suspicion of a dilated afferent loop, which could be proven by means of magnetic resonance imaging. A tumorous lesion as cause of the obstructive jaundice was not detectable. Intraoperatively a volvulus of the small intestine and strangling adhesions near the Braun's anastomosis w

Mechanical biliary obstruction. A review of the multisystemic consequences of obstructive jaundice and their impact on perioperative morbidity and mortality.

Whether obstruction is due to malignancy (ampullary or pancreatic) or benign disease (stones or stricture), mechanical biliary obstruction (MBO) is not an uncommon disease. While the outward manifestations of the disease such as jaundice or inanition may be obvious, the specific effects of the obstructive process on liver parenchymal or ductular function are subtle and, as yet, ill-defined. There are, however, multiple recent reports in the literature suggesting that deep jaundice and its coincident disorders of hepatic, coagulation, renal, or immune function may be alleviated via percutaneous

Effectiveness of transarterial drug-loaded microsphere chemoembolization against cholangiocarcinoma combined with obstructive jaundice.

BACKGROUND: Although transarterial chemoembolization (TACE) has been widely used to treat malignant cholangiocarcinoma (CCA), the efficacy of drug-loaded microsphere chemoembolization (DEB-TACE) remains to be comprehensively explored in patients with CCA combined with obstructive jaundice. METHODS: In this single-center retrospective cohort study, patients (n = 101) with CCA combined with obstructive jaundice admitted between January 2021 and December 2022 were treated with DEB-TACE or c-TACE. Initially, the baseline characteristics of the two groups of patients, i.e., DEB-TACE (n = 46) and c-

Gastrointestinal Stromal Tumor of the Pancreas With Hepatic Metastasis Presenting as Obstructive Jaundice.

Pancreatic gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors that present significant diagnostic and treatment challenges due to their unusual location and vague symptoms. This report details an unusual case involving a male in the seventh decade of life who experienced obstructive jaundice resulting from a pancreatic GIST situated in the pancreaticoduodenal groove. Diagnosis was confirmed through imaging, histopathological analysis, and immunohistochemical studies. The patient received treatment with imatinib, which led to notable clinical improvement and considerable tumor

Impact of obstructive jaundice on outcomes in acute biliary pancreatitis: a retrospective study.

OBJECTIVE: To examine the effects of obstructive jaundice on the outcomes of patients with acute biliary pancreatitis. METHODS: A retrospective chart review was conducted on 332 cases of acute biliary pancreatitis admitted to Rambam Health Care Campus, Israel, from January 1st, 2018, to December 31st, 2021. Patients were categorized based on the presence or absence of obstructive jaundice. Various clinical, laboratory, and radiological parameters were analyzed, including severity prediction scores, length of stay, interventions, and complications. RESULTS: Obstructive jaundice was observed i

Anaplastic large T-cell Lymphoma presenting as obstructive jaundice: Revisiting the pathologist's role.

An 18-year-old female presented with obstructive jaundice and significant weight loss for 2 months with acute cholangitis. Examination revealed pallor, icterus, left supraclavicular lymphadenopathy, and hepatosplenomegaly. USG of the neck showed a supraclavicular lymph node of size 6 × 5 × 3 cm. MRCP suggested Dilated CBD and IHBRs with an abrupt cut-off at the cut off at the distal end. CECT chest and abdomen showed cervical, mediastinal, and peri-pancreatic lymphadenopathy, which was causing distal CBD obstruction. EUS-guided FNAC from peri-pancreatic lymph nodes was taken, followed by ERCP-

Metataxonomic Analysis Demonstrates a Shift in Duodenal Microbiota in Patients with Obstructive Jaundice.

The human gastrointestinal tract (GIT) is home to an abundance of diverse microorganisms, and the balance of this microbiome plays a vital role in maintaining a healthy GIT. The obstruction of the flow of bile into the duodenum, resulting in obstructive jaundice (OJ), has a major impact on the health of the affected individual. This study sought to identify changes in the duodenal microbiota in South African patients with OJ compared to those without this disorder. Mucosal biopsies were taken from the duodenum of nineteen jaundiced patients undergoing endoscopic retrograde cholangiopancreatogr

An unusual cause of obstructive jaundice in childhood: Intra-choledochal malignant neuroendocrine tumour.

The extrahepatic biliary apparatus is a rare site for neuroendocrine tumours. A 13-year-old child presented with cholestatic symptoms of jaundice and pruritus with soft hepatomegaly and mild ascites. Magnetic resonance imaging and endoscopic ultrasound revealed a mid-common bile duct mass, and dilated intrahepatic biliary system. An en-bloc resection of the extrahepatic biliary apparatus, showed malignant cells disposed in lobules in a desmoplastic stroma with intramural invasion, staining positive for cytokeratin, chromogranin, synaptophysin and negative for CD56. At 3 months post-resection,

Inhibition of the norepinephrine transporter rescues vascular hyporeactivity to catecholamine in obstructive jaundice.

In patients with obstructive jaundice, the cardiovascular system exhibits hypotension and vascular hyporeactivity. Most norepinephrine is taken up through the neuronal norepinephrine transporter (NET), which is implicated in cardiovascular diseases. A previous study demonstrated that pharmacological NET inhibition could increase resting blood pressure. However, the role of NETs in vascular hyporeactivity induced by obstructive jaundice is poorly understood. This study used the NET inhibitor nisoxetine and a rat model of bile duct ligation (BDL) to investigate whether NET is associated with BDL

Fine-needle aspiration findings of a rare hematopoietic neoplasm presenting as obstructive jaundice.

A 51-year-old female who presented with obstructive jaundice was found to have masses in the pancreatic head and tail as well as suspicious liver and periaortic masses on imaging. Aspiration cytology of the pancreatic tail mass showed abundant large single cells with vacuolated eosinophilic cytoplasm, marked nuclear pleomorphism, large bizarre irregular nuclei, binucleation, and prominent nucleoli. Numerous cells also showed intracytoplasmic black to brown pigmentation. A cell block was obtained and extensive immunohistochemical staining was performed. S-100, HMB-45, Sox10, pancytokeratin, CK7

Obstructive Jaundice Caused by Hepatic Torsion in an Infant with Congenital Diaphragmatic Hernia.

Right sided congenital diaphragmatic hernia may cause biliary obstruction. We present a 2 months female infant who had respiratory distress and persistent jaundice since birth. Investigations suggested direct hyperbilirubinemia, right-sided diaphragmatic hernia with liver herniation in the thorax, and intra- and extrahepatic biliary dilatation. Laparotomy showed herniation of liver in the chest with hepatic torsion of about 180° causing obstruction of bile ducts. Liver torsion was corrected and liver relocated in the abdomen. An operative cholangiogram confirmed free passage of contrast to the

18F-FDG PET/CT in differentiating malignant from benign origins of obstructive jaundice.

BACKGROUND: The various origins of obstructive jaundice make the diagnosis of the disease difficult. This study was undertaken to evaluate the role of 18F-FDG PET/CT in differentiating malignant from benign origins of obstructive jaundice and to quantify the added value of 18F-FDG PET/CT over conventional imaging (enhanced CT and/or MRI). METHODS: Eighty-five patients with obstructive jaundice who underwent 18F-FDG PET/CT within 2 weeks after enhanced CT and/or MRI were reviewed retrospectively. All 18F-FDG PET/CT images were independently evaluated by 2 nuclear medicine physicians who were u
