[Whipple's operation and tumors of Oddi's sphincter. A series of 17 cases].

Seventeen patients underwent a Whipple operation for carcinoma of the ampulla of Vater. The operative mortality was 11.7% (2/17). Anastomotic dehiscence at the level of the pancreatico-jejunostomy occurred three times and was responsible for one death. The actuarial five years survival rate was 51%. The duodenopancreatectomy remains the operation of choice for ampullary malignancy.

Surgical results of corrosive injuries involving esophagus to jejunum.

BACKGROUND/AIM: Severe corrosive injury involving esophagus to jejunum remains an unique surgical problem which is associated with high mortality and morbidity. MATERIAL AND METHODS: Herein we report the outcomes of 28 caustic patients who underwent resections of the stomach, duodenum, a segment of jejunum, and adjacent involving organs. RESULTS: In all of these patients except one, esophagectomy was also performed. The concomitant procedures included pancreaticojejunostomy (n = 24), choledochojejunosotmy (n = 4), cholecystostomy (n = 4), common bile duct or pancreatic duct drainage, feeding…

Gastrostomy tube transmigration: a rare cause of small bowel obstruction.

A 77-year-old woman with an indwelling gastrostomy tube presented with a clinical picture of pneumonia and small bowel obstruction. During gastrostomy closure and performance of a feeding jejunostomy, inadvertent transmigration of the gastrostomy tube was found to be the etiology of the small intestinal obstruction. This case illustrates a rare complication of enteric feeding tubes.

Bowel necrosis caused by water in jejunal feeding.

BACKGROUND: Fifteen reports of bowel necrosis in patients receiving jejunal feeding have been reported. Etiology remains unexplained. METHODS: A patient with a 60% burn receiving jejunostomy tube feeding developed hypernatremia and was given distilled water in the jejunum, 400 mL every 2 hours. One week later, he developed an acute abdomen with abdominal distention. At operation, he had 4 L of cloudy fluid containing jejunal feeding. Three large duodenal perforations were present. The jejunostomy site was normal. In an animal study, water or normal saline (0.85% NaCl) were infused into the mi…

[Use of fibrin glue in the prevention of leakage in pancreatico-jejunal anastomoses].

The aim of this study was to test the protective effects of fibrin sealing on the pancreatico-jejunostomy (PJA), the high-risk anastomosis following pancreas head resection. Experimental study was performed on the mongrel dogs, divided in two groups (20 animals each): Experimental group-with end to end "dunking" PJA, protected by temporary occlusion of the pancreatic duct with fibrin sealant/Tissucol/Immuno Ag/, while control group was without any protective procedure. The animals were followed 5 months in order to study: protective effects of such procedure on the PJA quantified with the perc…

Concurrent Cryptosporidium and parvovirus infections in a puppy.

Cryptosporidium parvum, an intestinal coccidian parasite, was isolated from faeces and intestinal biopsies of a 9-week-old puppy with acute parvoviral gastroenteritis. Gene sequence analysis identified a Cryptosporidium genotype not previously recorded in Australia. The puppy recovered after treatment with crystalloid fluids, synthetic and natural colloids and jejunostomy tube feeding.

Familial Near-Total Intestinal Aganglionosis.

Near total aganglionosis represents the most extreme and rare form of Hirschsprung's disease. It can affect more than one member of family. We report three cases of near total intestinal aganglionosis in a family presenting with intestinal obstruction at birth. All of them were operated and a jejunostomy was performed. Outcome was dismal.

Esophago-gastric dissociation complicating devascularization procedure for portal hypertension.

Devascularization surgery for portal hypertension, indicated in selected patients, is considered safe. A 15-year-old boy with cirrhosis underwent elective modified Sugiura's devascularization procedure for secondary prophylaxis of variceal bleeding. He developed esophago-gastric dissociation, which was successfully managed with emergency feeding jejunostomy and restoration of gut continuity three months later.

[Pyloric exclusion for disinsertion of the papilla following closed trauma of the abdomen. Apropos of 2 cases].

Two cases of disinsertion of the papilla following closed trauma to the abdomen were treated by suction of the papilla by a Y loop associated with temporary pyloric exclusion, gastroenterostomy and a feeding jejunostomy. The postoperative course was uneventful in the two patients. This technique is therefore indicated for associated pancreatic lesions which do not require cephalic duodenopancreatectomy.

Pneumopericardium: report of a case.

An unusual case of pneumopericardium is described. It is due to a fistulous communication between the small bowel and pericardium. 4 years previously a gastro-jejunostomy was performed, and now a peptic ulcer in the jejunum had developed with abscess formation, thus giving rise to a fistulous communication between intestine and pericardium. The cause, symptoms and diagnostic procedures are described.

Fecal microbiome and bile acid metabolome in adult short bowel syndrome.

Loss of functional small bowel surface area causes short bowel syndrome (SBS), intestinal failure, and parenteral nutrition (PN) dependence. The gut adaptive response following resection may be difficult to predict, and it may take up to 2 yr to determine which patients will wean from PN. Here, we examined features of gut microbiota and bile acid (BA) metabolism in determining adaptation and ability to wean from PN. Stool and sera were collected from healthy controls and from patients with SBS (n = 52) with ileostomy, jejunostomy, ileocolonic, and jejunocolonic anastomoses fed with PN plus ent…

Conservative Management of a Rare Gastrobronchial Fistula Following Minimally Invasive Esophagectomy: A Case Report.

A gastrobronchial fistula (GBF) is an uncommon condition in which an abnormal connection develops between the stomach and the lungs. It can occasionally arise as a complication following esophagectomy for esophageal adenocarcinoma. Management of GBF includes both surgical and nonsurgical approaches, depending on the patient's condition and comorbidities. Conservative management focuses on diversion to promote fistula closure; options include esophageal stents, fasting, nasogastric drainage, proton pump inhibitors, and jejunostomy feeds. Surgical options include resection of the fistula, anasto…

[Choledochal cyst (author's transl)].

This paper reports six pediatric patients with congenital choledochal cyst. From the long-term follow-up results of the six patients authors conclude that primary excision of the choledochal cyst and Roux hepatic jejunostomy are the first choice treatment for this lesion. However, it may not be possible in all cases, and choledochocystoenterostomy would then be useful as a second choice.

[Choledochal cyst].

The persistence of recurrent abdominal pains in a 6 year-old girl without medical history lead to an ultrasonography being performed that showed a liquid formation, assigned to a dilated biliary duct. An oral cholecystography showed that this formation corresponded to a voluminous choledochal cyst. Surgical treatment consisted of a cyst excision with an hepatico-jejunostomy as biliary drainage.
