High case fatality cholera outbreak in Western Kenya, August 2010.

INTRODUCTION: Cholera is a disease caused by the bacterium Vibrio cholera and has been an important public health problem since its first pandemic in 1817. Kenya has had numerous outbreaks of cholera ever since it was first detected there during 1971. In August 2010 an outbreak of cholera occurred in Kuria West District spreading to the neighboring Migori District. We conducted an investigation in order to determine the magnitude of the problem and institute control measures. METHODS: In order to update the line lists we reviewed records in Migori and Kuria district hospitals and conducted ac…

High altitude cerebral oedema during adventure training on Mount Kenya.

The trekking ascent to Point Lenana (4,985m) on Mount Kenya is a popular objective for soldiers on adventurous training in Kenya. The standard route previously taken has been the Naro Moru route which involves an ascent rate far in excess of that recommended to avoid altitude illness. This article describes the case of a British soldier who developed high altitude cerebral oedema during an ascent of Point Lenana via the Naro Moro route. Recommendations to reduce the risk of altitude illness on Mount Kenya include alternative and more gradual routes of ascent. Early symptom recognition and desc…

Craniofacial anomalies amongst births at two hospitals in Nairobi, Kenya.

The pattern of congenital oral and craniofacial anomalies (CFAs) in the Kenyan population remains unknown. The objective of this study was to describe the pattern of occurrence of CFAs at two hospitals in Nairobi. A descriptive cross-sectional study at the Kenyatta National Hospital and Pumwani Maternity Hospital was carried out from November 2006 to March 2007. Mothers who delivered at the hospitals consented to an interview and physical examination of their babies within 48 h of delivery. The anomalies were classified for type and magnitude. Data were analysed to determine the association of…

Chronic diarrhoea among HIV-infected adult patients in Nairobi, Kenya.

OBJECTIVES: Chronic diarrhoea and wasting are well recognized features of AIDS in Africa. However, because of resource constraints few comprehensive aetiological studies have been conducted in sub-Saharan Africa which have included a broad range of microbiological investigations. We undertook a prospective cross-sectional study of adult patients admitted to a government hospital in Nairobi, Kenya, to determine possible bacterial, mycobacterial, parasitic and viral causes of diarrhoea; to consider which may be treatable; and to relate microbiological findings to clinical outcome. METHODS: Stoo…

Antibody to Haemophilus ducreyi among trucking company workers in Kenya.

BACKGROUND AND OBJECTIVES: To determine the prevalence, correlates, and incidence of Haemophilus ducreyi antibodies, a cohort of East African trucking company employees was evaluated. STUDY DESIGN: Human immunodeficiency virus (HIV)-1-seronegative men working in six trucking companies in Mombasa, Kenya, were evaluated with a questionnaire and serologic testing for antibodies to H. ducreyi and other sexually transmitted pathogens. Men who were initially H. ducreyi seronegative were retested at 1 year of follow-up. RESULTS: The H. ducreyi seroprevalence among 501 men at enrollment was 26.5%. S…

PVO / NGO initiatives, Africa. Women Fighting AIDS in Kenya (WOFAK).

Women Fighting AIDS in Kenya (WOFAK) was founded in order to improve the effectiveness of existing AIDS services delivered to women in Kenya. Founded by both seropositive and seronegative women, WOFAK is unique in that it is the only AIDS support organization (ASO) in Kenya run by and for women. WOFAK contributes to the empowerment of HIV positive women and girls by assisting them in gaining control over their own health and serostatus, to live positively with the infection, and to protect their families and communities from becoming infected. WOFAK founders feel that existing ASOs address wom…

Epidemiology of Schistosoma haematobioum in Ruruma Location, Kilifi, District, Kenya.

A study of the distribution of S. haematobium in Ruruma Location of Kilifi District, Kenya, was undertaken between December 1988 and June 1990. Schistosomiasis haematobia was found to be highly endemic with prevalence rates of upto 85%. The distribution of prevalences of infection in this location is very varied ranging from 1.6% in the East to 85% in the Southwest. Prevalences of infection were also noted to be lower in those areas closer to the main roads than farther away, a pattern that coincided also with the distribution of piped water in the area. Gross haematuria, observed by naked eye…

Serological evidence of arboviral infections among humans of coastal Kenya.

A serosurvey was conducted during September 1987 for evidence of human arboviral infections in the Coast Province of Kenya. Sera were collected from 1624 outpatients at three hospitals and tested for antibody to eight arboviruses by the indirect immunofluorescent antibody technique. Antibody prevalence rates were: Rift Valley fever, 2.8%; Sindbis, 2.6%; dugbe, 2.1%; dengue-2, 1.0%; West Nile, 0.9%; chikungunya, 0.7% and Nairobi sheep disease, 0.3%. Evidence of Crimean-Congo haemorrhagic fever viral antibody was not detected. The data suggested low arbovirus activity since 1982, when an epidemi…

Cutaneous leishmaniasis caused by Leishmania major in Baringo District, Kenya.

Leishmania major was isolated from lesions of a patient suffering from cutaneous leishmaniasis in Baringo District of Kenya. Isoenzyme mobilities of this strain were compared with those of L. major, L. donovani, L. aethiopica and L. tropica reference strains and also L. major from a sand fly, Phlebotomus duboscqi, and a rodent, Arvicanthis niloticus, trapped in the same region. The patient's isolate had similar banding patterns to the L. major reference strain and also the rodent and the sand fly strains with the 9 enzymes examined. This is the first report in Kenya of an indigenous case with …

Seroincidence Rate of Typhoidal Salmonella in Children, Kenya, 2017-2018.

Enteric fever, caused by Salmonella enterica serovars Typhi and Paratyphi, results in high rates of illness and death globally. The lack of reliable diagnostic assays limits surveillance, leading to major gaps in understanding the population-level burden in low- and middle-income countries. We applied a novel serologic tool measuring IgG responses to hemolysin E to assess typhoidal Salmonella infection rates in children from 4 communities: 2 in western Kenya (Kisumu and Chulaimbo) and 2 in coastal Kenya (Ukunda and Msambweni). We found a substantially higher enteric fever seroincidence rate in…
