Upper urinary tract urothelial cell carcinoma: location as a predictive factor for concomitant bladder carcinoma.

OBJECTIVE: To investigate the existence of predictive factors for concomitant, primary UUT-UCC and BC. Upper urinary tract urothelial cell carcinoma (UUT-UCC) is a pan-urothelial disease of the transitional epithelial cells. Although several studies have shown the association of bladder recurrence following UUT-UCC, little is known on the incidence of concomitant UUT-UCC and bladder cancer (BC) without previous BC. MATERIALS AND METHODS: A retrospective review of 673 patients diagnosed and treated for UUT-UCC was performed. Patients with history of BC were excluded. We investigated age, sex,

[Open surgery of calculus of the kidney pelvis. Results and complications in the era of extracorporeal lithotripsy].

From 1989 to 1993, 11,535 extracorporeal shock wave lithotripsy (ESWL) sessions were performed in 6,066 patients in our centre. In contrast, simple or extended pyelolithotomy was the treatment of choice in a small group of patients. The object of this paper is to review the results and complications of this type of operation. A total of 57 pyelolithotomies was performed, including 33 extended pyelolithotomies, in 55 patients (26 men and 29 women). The mean age of the series was 50 years (range: 5-77). The operated kidney was the left kidney in 34 cases and the right kidney in 23 cases. The mea

Safety and complications of upper pole puncture in supine percutaneous nephrolithotomy.

OBJECTIVE: Supine percutaneous nephrolithotomy (PCNL) has gained attention as a viable alternative to the traditional/historical prone approach for the treatment of large renal stones. However, concerns remain regarding the safety and feasibility of upper calyx punctures in the supine position, particularly for upper-pole access. This multicenter, retrospective cohort study evaluated the safety and stone-free rates of upper-pole punctures in supine PCNL, comparing outcomes with middle and lower calyx punctures and with literature on prone PCNL. METHODS: This retrospective study analyzed supin

Megacalycosis: a rare condition.

Megacalycosis is an extremely rare condition. We report our experience with two cases and discuss its pathogenesis, diagnosis and management in children. Our two patients had presented a prior diagnosis of congenital hydronephrosis. An increased number of calyces with a significant disproportion between the degree of calyceal dilatation and a mildly dilated renal pelvis were found in each case. Megacalycosis must be considered in the differential diagnosis of congenital hydronephrosis, polycalycosis, and infundibular stenosis. The diagnosis is suggested by ultrasound and confirmed by diuretic

Staghorn Caliceal Hem-o-lok Stone: A Long-Term Complication of Robotic Partial Nephrectomy - A Case Report and Literature Review.

INTRODUCTION: Hem-o-lok clips (HOLCs) are a useful tool in mini-invasive surgery, especially for renal surgery. However, in rare cases, they could migrate into the collecting system and be calculogenic. CASE PRESENTATION: We present a case of a 53 years old man with an incidental CT finding of a left staghorn caliceal stone increasing its size in the last 2 years. He had a medical history of ipsilateral robotic partial nephrectomy (RAPN) at another institution 8 years before and a previous renal colic with spontaneous stone elimination about 20 years before. The stone was located in the upper

Challenges in the diagnosis of calyceal diverticulum: A report of two cases and review of the literature.

BACKGROUND: Calyceal diverticula outpouchings that occur rarely in the upper collecting system of the kidney and is often difficult to detect. In this study, we present two cases of calyceal diverticula and discuss their clinical characteristics and radiologic features. PATIENTS AND METHODS: In the presented two cases, we applied several imaging examinations, including delayed intravenous pyelography, retrograde pyelography and axial computerized tomographic (CT) scanning of the kidneys with and without contrast. Serum creatinine levels in fluid withdrawn from the diverticula were found to be

[RIRS through semi-rigid ureteroscope and holmium laser in the treatment of ureteral stones retropulsion].

Retrograde displacement of ureteral stones into the renal cavities during ureteroscopic lithotripsy represents a frequent and adverse event that leads to additional procedures (ESWL, PCNL, Retrograde Intra-renal lithotripsy with flexible instruments, DJ stent placement and subsequent EWSL) to obtain full clearence of calculi. All these procedures require a further time of treatment. Between 1/2008 and 3/2009, a total of 48 patients harbouring proximal (21 cases) and distal (27 cases) ureteral stones underwent Holmium Laser lithotripsy. In 3 patients previous percutaneous nephrostomy was perfor

Urinary tract dilatation in utero: classification and clinical applications.

A morphologic classification of in utero urinary tract dilatation is presented. Ninety-two hydronephrotic fetal kidneys diagnosed with ultrasound were graded according to the proposed classification. The findings suggest that grade I dilatation (anteroposterior diameter of the renal pelvis less than 10 mm) should be considered normal. Grades II and III constitute an intermediate hydronephrosis, requiring postnatal urologic surgery in nearly half the cases. Grade IV (moderate dilatation of the calyces, with easily identified residual renal cortex) and grade V (severe dilatation of the calyces w

Harmonic versus conventional ultrasound imaging of the urinary tract in children.

BACKGROUND: Harmonic imaging (HI) is a relatively new US method. Its usefulness in children has yet to be determined. OBJECTIVE: To evaluate the value of HI in urinary tract imaging of infants and children. MATERIALS AND METHODS: The bladder and kidneys of 29 unselected patients, aged 2 months to 12.8 years (mean, 6.8 years), were examined from ventral and dorsal approaches using conventional (= fundamental) imaging (FI) (6.5 and 3.4 MHz) and HI (3.3 MHz). In addition to global image quality, visualization of bladder wall, retrovesical space, renal contrast to liver/spleen, the pelvicalyceal

Spontaneous Large Urinoma Secondary to Obstructing Ureteral Calculus: A Kidney Pop-Off Mechanism.

Introduction and Background: Spontaneous rupture of the urinary collecting system with extravasation of urine is a rare complication of obstructive stone disease. Most of the cases are caused by obstructive ureteral stones. We herein present a case of a spontaneous caliceal rupture with a large perirenal urinoma formation that was silent on presentation and managed with endoscopic stenting and percutaneous catheter drainage. Case Presentation: A 56-year-old man presented with complaints of vague right flank discomfort. A noncontrast CT scan revealed a 9.4 mm right mid ureteral obstructive calc

[Neonatal uro-haematoma secondary to posterior urethral valve].

The authors report the case of a male neonate presenting with a heterogeneous cystic mass in the right renal area with no identifiable renal unit. The diagnosis of right perirenal extracapsular uro-haematoma secondary to posterior urethral valve was proposed on the basis of the antenatal clinical history. The late discovery at 39 weeks of amenorrhoea (pregnancy not followed) of a foetus with trabeculated bladder associated with left ureteropelvic dilatation, and a cystic mass occupying the right lumbar fossa was highly suggestive of right urinoma secondary an infravesical obstacle. In this cas

Laparoscopic dismembered pyeloplasty combined with port entrance flexible renoscopic lithotripsy.

INTRODUCTION: Ureteropelvic junction obstruction and concomitant calculus disease may coexist. We demonstrate our use of flexible renoscopy during laparoscopic pyeloplasty for caliceal stone removal. PATIENT AND METHODS: A 28-year-old female patient presented with recurrent attacks of flank pain of two years duration. When noncontrast-CT and DTPA were performed, the patient was diagnosed with ureteropelvic junction stenosis and 3 stones with a total burden of 14mm in the lower pole of right kidney. After pneumoperitoneum was established in right flank position, three 10mm trocars were placed

A new cause of curvilinear renal calcification: calcified hydrocalycosis.

Although many causes of curvilinear renal calcifications have been described, calcified hydrocalycosis mimicking hydatid cyst has not previously been reported. We report a case of hydrocalycosis which was appeared as a curvilinear calcified cystic lesion resulted from staghorn calculus associated with xanthogranolomatous pyelonephritis (XGPN) on intravenous pyelography (IVU) and computed tomography (CT).

The initial renal ultrasound examination in children with urinary tract infection: the prevalence of dilated uropathy has decreased.

BACKGROUND: Pelvocalyceal dilatation (PCD) is often detected by sonography during the prenatal period. Prenatal detection of PCD should affect the prevalence of PCD in children with urinary tract infection (UTI). PURPOSE: To determine the prevalence of abnormal sonographic findings in a population of children undergoing first imaging evaluations after UTI and to determine if the prevalence has changed from an earlier study at the same institution. MATERIALS AND METHODS: The findings on consecutive first renal and bladder sonographic examinations in 844 children (745 girls, 99 boys) with UTI

Predictors of Stone-Related Events in Asymptomatic Untreated Intrarenal Calculi.

Purpose: There is a lack of data on the natural history of asymptomatic intrarenal calculi. In this study, we investigate stone-related events (SREs) in patients with untreated intrarenal calculi. We also investigate predictive factors for SREs. Methods: All patients found with an asymptomatic intrarenal calculus on CT kidney, ureter, bladder managed conservatively with interval imaging for ≥6 months were included. Patients were evaluated for any SRE. The rate of event according to calculus size, location, and number of calculi was also analyzed. Multivariate logistic regression analysis was p
