Direct versus indirect laryngoscopy using a Macintosh video laryngoscope: a mannequin study comparing applied forces.

PURPOSE: Upper airway injury and sympathetic activation may be related to the forces applied during laryngoscopy. We compared the applied forces during laryngoscopy using direct and indirect visualization of a standardized mannequin glottis. METHODS: Force transducers were applied to the concave surface of a GlideScope T-MAC Macintosh-style video laryngoscope that can also be used as a conventional direct-view laryngoscope. Thirty-four anesthesiologists performed four laryngoscopies (two direct and two indirect views) on an Ambu mannequin in a randomized sequence. During each laryngoscopy, pa…

Cost-effectiveness of fiberoptic laryngoscopy prior to total thyroidectomy for low-risk thyroid cancer patients.

BACKGROUND: Flexible fiberoptic laryngoscopy is performed prior to thyroid surgery to evaluate the function of the recurrent laryngeal nerve. We assess the cost-effectiveness of preoperative laryngoscopy prior to total thyroidectomy for a low-risk thyroid cancer patient without dysphonia. METHODS: A decision tree analysis was performed from a third-party payer perspective. We assessed the cost-effectiveness of fiberoptic laryngoscopy prior to total thyroidectomy for T2N0M0 papillary thyroid carcinoma, such that an ipsilateral vocal fold paralysis alters the surgical plan to hemi-thyroidectomy…

Ultrasound for diagnosing new difficult laryngoscopy indicator: a prospective, self-controlled, assessor blinded, observational study.

BACKGROUND: Unpredictable difficult laryngoscopy (DL) remains a challenge for anesthesiologists, especially when difficult ventilation occurs during standard laryngoscopy. Accurate airway assessment should always be performed, but the common airway assessment methods only perform superficial screening. Thus, the deep laryngopharyngeal anatomy may not be evaluated. Ultrasound-based airway assessment has been recently proposed as a useful, simple, and non-invasive bedside tool as an adjunct to clinical methods, which may facilitate identification of DL. The present study aimed to determine the c…

NBI flexible laryngoscopy targeted tissue sampling in head and neck cancer patients with difficult airways.

Direct suspension laryngoscopic biopsy of neoplasms in larynx, oropharynx, and hypopharynx was an arduous procedure in patients with a history of head and neck cancer and difficult airways. This preliminary study was aimed to report the efficacy and safety of a narrow band imaging-guided biopsy of this category by flexible laryngoscopy. This is a retrospective chart review study conducted in setting of tertiary referral centre. Nineteen consecutive head and neck cancer patients with difficulty in general anesthesia and rigid endoscopic approach due to trismus, craniofacial deformities, and/or …

Ultrasound quantification of anterior soft tissue thickness fails to predict difficult laryngoscopy in obese patients.

Morbid obesity is associated with difficult laryngoscopy and intubation. In the general population, bedside indices for predicting difficult intubation (i.e. Mallampati classification, thyromental distance, sternomental distance, mouth-opening and Wilson risk score) have poor-to-moderate sensitivity (20-62%) and moderate-to-fair specificity (82-97%). In the obese population, although the risk of difficult intubation after a positive Mallampati test is 34%, it is still not sufficient to be used as a single predictive test. An abundance of pretracheal soft tissue anterior to the vocal cords, as …

Three cases of ortho-phthalaldehyde-induced anaphylaxis after laryngoscopy: detection of specific IgE in serum.

BACKGROUND: Ortho-phthalaldehyde (OPA) has recently been used as a disinfectant for various medical apparatuses. OPA is not generally recognized as a potential allergen. CASE SUMMARY: Subsequent to our recent report describing a patient presenting with OPA-induced anaphylaxis following laryngoscopy, we experienced two more such cases. In all three cases, the basophil histamine release test was useful for identifying the allergen as OPA. OPA-specific IgE was successfully detected in the serum of the patients by ELISA. DISCUSSION: Physicians and co-medical workers need to be aware of potential…

Impact of Difficult Laryngoscopy on First-Pass Success and Adverse Events in Pediatric Emergency Department Intubations.

BACKGROUND: Airway management in the Emergency Department (ED) can be challenging, particularly in children. Difficult laryngoscopy reduces first-pass success and increases complications, but pediatric ED data remain limited. METHODS: We conducted a prospective, observational study of pediatric intubations recorded in the Australian and New Zealand Emergency Department Airway Registry (ANZEDAR) between 2012 and 2025, including 25 Pediatric Research in Emergency Departments International Collaborative (PREDICT) sites. The primary outcome was the incidence of difficult laryngoscopy. Secondary o…

Video laryngoscopy as a teaching tool in direct intubation in undergraduate medical education - a randomized controlled trial.

BACKGROUND: Direct laryngoscopy (DL) is a crucial skill and continues to be indispensable. While the superiority of video laryngoscopy (VL) has been demonstrated regarding the rate of first attempt successful intubations, its application as a teaching tool for DL remains largely unexplored. We hypothesised a higher first-pass success and shorter durations of conventional intubation after video laryngoscopy supported training (VLF = video-laryngoscopy based feedback) compared to students trained without VL (DLF = direct-laryngoscopy based feedback). METHODS: All 211 medical students (DLF: 92/ V…

Hyperangulated video laryngoscopy in the emergency department: An analysis of errors and factors leading to prolonged apnea time.

BACKGROUND: Video laryngoscopy (VL) is a widely utilized method for endotracheal intubation that both increases first pass success and reduces esophageal intubations. Use of VL in a teaching environment allows for real-time feedback for new learners and creates an opportunity for education and quality improvement. There is limited research on the difficulties that VL users face during intubation attempts. This study aims to explore the errors practitioners make while using hyperangulated video laryngoscopes and investigate how the errors affect intubation time. METHODS: We conducted a retrosp…

Implementation of default videolaryngoscopy instead of direct laryngoscopy for tracheal intubation: a narrative review of evidence and experiences.

INTRODUCTION: Default videolaryngoscopy - use of a videolaryngoscope in preference to a direct laryngoscope - remains a hotly debated topic. High-risk tracheal intubations performed during the COVID-19 pandemic added to the extensive existing evidence of advantages of videolaryngoscopy for patients and staff. Despite this, and calls for implementation of default videolaryngoscopy, it has not been adopted widely. METHODS: We summarise current evidence for the benefits of videolaryngoscopy and discuss (and where appropriate dispute) the common reasons given for not using videolaryngoscopy. The …

A Comparison of Airway Ultrasound and Clinical Parameters for Predicting Difficult Laryngoscopy in the Emergency Department: A Study Protocol.

BACKGROUND: Airway assessment plays an important part in airway management. Over the years, ultrasound has emerged as an alternative modality for airway assessment. We aim to assess the accuracy of clinical parameters, ultrasonographic upper airway parameters, and laryngoscopic grade, using the Cormack-Lehane classification, to predict difficult laryngoscopy in the emergency department (ED). The primary objective is to determine the accuracy of ultrasonography parameters compared to Cormack-Lehane grading in predicting difficult laryngoscopy in adults. The secondary objective is to assess the …

Evaluation of two neck ultrasound measurements as predictors of difficult direct laryngoscopy: A prospective observational study.

BACKGROUND: Unpredictable difficult laryngoscopy remains a challenge for anaesthesiologists, especially if difficult ventilation occurs during standard laryngoscopy. Accurate airway assessment should always be performed, but the common clinical screening tests have shown low sensitivity and specificity with a limited predictive value. Ultrasound-based airway assessment has been proposed recently as a useful, simple, noninvasive bedside tool as an adjunct to clinical methods, but to date, few studies are available about the potential role of ultrasound in difficult airway evaluation, and these …

Time-to-intubation in obese patients. A randomized study comparing direct laryngoscopy and videolaryngoscopy in experienced anesthetists.

BACKGROUND: Airway management may be difficult in obese patients. Moreover, during prolonged intubation, oxygen desaturation develops rapidly. Videolaryngoscopy improves the view of the larynx, and the Storz® C-MAC™ has been shown to be superior to other videolaryngoscopes in terms of intubation time in obese patients. However, no effort has been made to compare the Storz® C-MAC™ with direct laryngoscopy. The aim of the study was to evaluate if the use of Storz® C-MAC™ may reduce intubation time when compared to direct laryngoscopy (classic Macintosh® blade). METHODS: Eighty patients with Bod…

[Cervical spine motion during laryngoscopy with the Pentax-AWS with a new thinner blade (Introck-T)].

BACKGROUND: The Pentax-AWS equipped with a new thinner blade (Introck-T) is an intubation device that provides a non-line-of sight view of the glottis. A non-line-of sight view is expected to cause less movement of the cervical spine during laryngeal visualization. We measured the degree of cervical spine movement during laryngoscopy with the device. METHODS: Twenty patients requiring general anesthesia were studied. Each patient underwent laryngoscopy using the Pentax-AWS with the Introck-T. Movements of cervical spine were measured by radiography in the same patient both at neutral head pos…
