Marfan syndrome is classified as a heritable disorder of connective tissue, and it has been assumed to be an inborn error of protein metabolism particularly in collagen or elastin. We examined 41 Marfan syndrome patients and found in the corneal endothelium guttata-formation, considerable cell pleomorphism, non-reflecting endothelial black spots and in some cases abnormal looking endothelial cells. The relation of pathological endothelium to lens subluxation, axial length and other corneal findings is discussed.

Although the removal of the lens from patients with ectopia lentis has long been associated with a high rate of surgical complications, the surgical outlook for these patients is improving now that more sophisticated surgical modalities are available. We treated a series of eight consecutive patients with pars plana lensectomy (12 eyes) whose visual acuity could not be satisfactorily improved with optimal optical correction (phakic or aphakic). Visual acuity could not be improved in any of the subjects. Preoperative visual acuities ranged from 5/400 to 20/70. Eleven of 12 eyes maintained a vis

INTRODUCTION: Hypermature cataract is a form of late-stage cataract progression that can lead to a variety of complications. Spontaneous capsular rupture with lens nucleus displacement in hypermature cataracts has rarely been reported. We describe 2 cases of spontaneous dislocation of the lens nucleus in a hypermature cataract and perform a review of the literature on this complication.
PATIENT CONCERNS: We report 2 rural men aged 50 and 76 years with deteriorating vision.
DIAGNOSIS: The final diagnosis was senile hypermature cataract with dislocation of the lens nucleus in both patients and

The authors describe two brothers with Marfan syndrome who developed severe complications after iris-suture fixation of an intraocular lens (IOL). A secondary iris cyst developed in one and the IOL dislocated into the vitreous in the other, requiring pars plana vitrectomy and IOL removal. Although the visual outcome was good in both cases, severe complications of iris-suture fixated IOLs must be considered when surgical correction of aphakia is attempted in children with Marfan syndrome.

Anterior megalophthalmos (AM) is an uncommon developmental anomaly of the anterior segment of the eye with a constellation of findings that includes enlarged cornea, deep anterior chamber, posterior positioning of the iris and lens, iris stroma atrophy, hypoplasia of iris dilator, pupil displacement, large capsular bag, lens subluxation, prematurely cataract and the tendency to retinal detachment. AM, especially when symptoms are mild, is not an easy disease to diagnose. We present 3 AM cases that were misdiagnosed as congenital cataract with weak zonule and megalocornea. Intraocular lenses (I…

Results of surgical treatment 142 eyes of patients with traumatic cataracts complicated with accompanied pathology (corneal scars, anterior and posterior synechia, lens subluxation, anterior and posterior capsule defects and others) are analyzed. In 107 cases phacoemulsification was done, in other cases--lensectomy, intracapsular extraction or phacoaspiration of traumatic cataract. Indications and contraindications for traumatic cataract phacoemulsification are formulated.
