Intrahepatic cholestasis of pregnancy and offspring neurodevelopment: bile acid-mediated mechanisms and long-term neurodevelopmental outcomes.

PURPOSE: Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific liver disorder characterized by pruritus and elevated maternal serum total bile acids. While clinical management has focused on bile acid-based risk stratification and delivery planning to reduce perinatal risks, emerging evidence suggests possible longer-term effects on offspring neurodevelopment. This review synthesizes current epidemiologic and mechanistic evidence linking ICP with offspring neurodevelopmental outcomes. METHODS: PubMed, Embase, and Web of Science were searched using controlled vocabulary and free-

Parents' son preference, childhood adverse experience and mental health in old age: Evidence from China.

BACKGROUND: Son preference is an enduring phenomenon in China and may often be related to childhood adverse experiences. According to a life-course perspective, adverse experiences during a childhood period may have a long-term effect on mental health in later age. However, little is known about this relationship between parents' son preference, childhood adverse experiences and adulthood mental health in China. OBJECTIVE: The study aims to evaluate the association of parents' son preference and individual mental health in old age in China. The mediating role of childhood adverse experiences

Update of survival and cost of metastatic melanoma with new drugs: Estimations from the MelBase cohort.

PURPOSE: Since 2011, significant progress was observed in metastatic melanoma (MM), with the commercialisation of seven immunotherapies or targeted therapies, which showed significant improvement in survival. In France, in 2004, the cost of MM was estimated at €1634 per patient; this cost has not been re-estimated since. This study provided an update on survival and cost in real-life clinical practice. METHODS: Clinical and economic data (treatments, hospitalisations, radiotherapy sessions, visits, imaging and biological exams) were extracted from the prospective MelBase cohort, collecting in

The decade-long effect of work insecurity on husbands' and wives' midlife health mediated by anxiety: A dyadic analysis.

Although the detrimental physical health effects of work insecurity have been noted in previous research, less is known about the mediating processes, such as anxiety symptoms, that link work insecurity to physical health. Even less research has explored these effects at specific life stages and how these effects may impact significant others, even though the impact of this stress may vary across the life course and the mutual influences between married partners may cause dyadic effects stemming from partners' work insecurity. To fill these gaps, the current study incorporates theories that em

Bone marrow transplantation versus periodic prophylactic blood transfusion in sickle cell patients at high risk of ischemic stroke: a decision analysis.

Measurement of cerebral blood velocity (CBV) by transcranial Doppler has been used to identify patients with sickle cell disease (SCD) who are at high risk of ischemic stroke. This study examines outcomes of bone marrow transplantation (BMT) and periodic blood transfusion (PBT) as a basis for making treatment recommendations for patients who have elevated CBV and no other indications for BMT. Decision analysis was used to compare the number of quality-adjusted life years (QALYs) experienced by a population of patients with SCD at high risk for stroke who were treated with PBT or BMT. Interventions for Persons with Young-Onset Dementia and Their Families: A Scoping Review.

BACKGROUND: Dementia occurring before age 65 is known as young-onset dementia (YOD), with Alzheimer's disease being the most common type. YO … tudies are urgently needed, especially in developing countries, with a focus on family-centered and life course perspectives. In future intervention research design, a more extensive incorporation of stakeholder involvement is essential for successful implementation. Moreover, the integration of new technologies shows promise as a potential avenue for intervention advanc …

Initial symptoms and late complication in Lyme neuroborreliosis from the perspective of patients and relatives: a qualitative study.

BACKGROUND: Lyme borreliosis is by far the most common vector-borne infection in Western Europe. The most severe manifestation of Lyme borreliosis is Lyme neuroborreliosis (LNB). In LNB symptoms vary from mild to severe and may include late complications that involve both physical and/or neurocognitive constraints. An estimated 25-28% of the LNB population suffers from late complications. This study investigates patient and relative perspectives on everyday life with LNB symptoms, diagnosis, and treatment to identify areas for improvement of healthcare. METHODS: A focus group was conducted at

Cost-effectiveness of acupuncture care as an adjunct to exercise-based physical therapy for osteoarthritis of the knee.

BACKGROUND: The delivery of acupuncture alongside mainstream interventions and the cost-effectiveness of "alternative" treatments remain areas of controversy. OBJECTIVE: The aim of this study was to assess the cost-utility of adding acupuncture to a course of advice and exercise delivered by UK National Health Service (NHS) physical therapists to people with osteoarthritis of the knee. DESIGN: A cost-utility analysis was performed alongside a randomized controlled trial. METHODS: A total of 352 adults (aged 50 years or older) were randomly assigned to receive 1 of 3 interventions. Cultural influence and narrative reconstruction: resilience- building among Chinese breast cancer survivors and implications for social work.

OBJECTIVE: This study investigates how Chinese breast cancer survivors reconstruct damaged identities and negotiate cultural norms to build resilience within a specific socio-cultural context. METHODS: Using narrative inquiry and a life-course perspective, in-depth interviews were conducted with 15 female breast cancer survivors in Beijing. The study employed thematic narrative analysis to identify cross-cutting patterns while preserving individual story integrity. Rigor was ensured through data saturation and member checking. RESULTS: Resilience is manifested as a transformative narrative p

Do children's labour market experiences affect the mental health of ageing parents?

In this study we investigate the influence of adult children's adverse labour market experiences on the mental health of their ageing parents. Taking a life course perspective, we examine the trajectory of adult children over nearly a decade following graduation from a school or university. We utilise data from Swedish registers linked to the Västerbotten Intervention Programme (VIP), a survey that provides detailed information about health outcomes in northern Sweden. We utilise sequence analysis and cluster analysis to categorise labour market trajectories of adult children. Next, we use lin…

State-of-the-art treatment of postpartum bipolar disorder.

PURPOSE OF REVIEW: To summarize recent updates in the treatment of postpartum bipolar disorder (PBD).PBD requires timely and comprehensive management, as childbirth is a period of elevated relapse risk in women with preexisting illness and may also mark first-onset presentations. Episodes can manifest as depression, mixed states, mania, or psychosis, with severe consequences for maternal safety, infant well being, and early bonding. Sleep loss around labor and postpartum further increases vulnerability. RECENT FINDINGS: PBD demands a multifaceted therapeutic approach, with pharmacotherapy as

Revictimisation via Informal Disclosure: Understanding How Family, Friend, and Partner Responses Can Harm Victim-Survivors of Sexual Violence.

Despite disclosure research often focusing on formal disclosures (e.g. to the police), evidence suggests that informal disclosure is more common. Prior research has examined the impact of informal disclosures of sexual violence (SV), though understanding is still limited, particularly in the United Kingdom context. This research aims to explore the impact of "negative" informal disclosure responses, from the perspective of the victim-survivor, ultimately arguing that responses are often harmful rather than negative. This was particularly salient for minoritised victim-survivors. This study pro

Infertility as Trauma: Understanding the Lived Experience of Involuntary Childlessness.

Infertility, to those who are affected by it, is much more than whether one manages (or not) to have a child: it can be a traumatizing experience. Based on a clinical case study that involved one-to-one psychotherapy sessions and semi-structured interviews with six involuntarily childless women living in Norway, this article develops the argument that there is a need to treat infertility as trauma, both conceptually and from the perspective of therapeutic practice. The analysis contributes to our understanding of trauma as a disruptive event that erodes a person's moral agency. It does so by o

Intervertebral Disc-on-a-Chip as Advanced In Vitro Model for Mechanobiology Research and Drug Testing: A Review and Perspective.

Discogenic back pain is one of the most diffused musculoskeletal pathologies and a hurdle to a good quality of life for millions of people. Existing therapeutic options are exclusively directed at reducing symptoms, not at targeting the underlying, still poorly understood, degenerative processes. Common intervertebral disc (IVD) disease models still do not fully replicate the course of degenerative IVD disease. Advanced disease models that incorporate mechanical loading are needed to investigate pathological causes and processes, as well as to identify therapeutic targets. Organs-on-chip (OoC)

Electroconvulsive therapy against the patients' will: A case series.

OBJECTIVES: Electroconvulsive therapy (ECT) is the most effective therapy for severe depressive and psychotic disorders. As patients may be subject to legal incapacity and lack of insight, treatment may be administered against the patient's will. There is only limited evidence on the use of ECT against the patient's non-autonomous will. METHODS: We report a series of eight patients who received ECT against their will in two German university medical hospitals between 2014 and 2016. The effectiveness, tolerability and patients' perspective are described. RESULTS: Seven of eight patients were

Pharmacological therapeutics in Friedreich ataxia: the present state.

Friedreich ataxia (FRDA) is a progressive, inherited, neurodegenerative disease for which there is currently no cure or approved treatment. FRDA is caused by deficits in the production and expression of frataxin, a protein found in the mitochondria that is most likely responsible for regulating iron-sulfur cluster enzymes within the cell. A decrease in frataxin causes dysfunction of adenosine triphosphate synthesis, accumulation of mitochondrial iron, and other events leading to downstream cellular dysfunction. Areas covered: Therapeutic development for FRDA currently focuses on improving mito

Bipolar disorder: role of immune-inflammatory cytokines, oxidative and nitrosative stress and tryptophan catabolites.

Bipolar disorder (BD) is a complex disorder with a range of presentations. BD is defined by the presentation of symptoms of mania or depression, with classification dependent on patient/family reports and behavioural observations. Recent work has investigated the biological underpinnings of BD, highlighting the role played by increased immune-inflammatory activity, which is readily indicated by changes in pro-inflammatory cytokines or signalling, both centrally and systemically, e.g. increased interleukin-6 trans-signalling. Here, we review the recent data on immune-inflammatory pathways and c
