The use of extracorporeal life support in the treatment of influenza-associated myositis/rhabdomyolysis.

A 13-year-old girl presented to the emergency department with fatigue, headaches and muscle stiffness after returning from a family camping trip. Within 24 h, she was transferred to ICU with general oedema and low saturations, where she had a cardio-respiratory arrest and was placed on veno-arterial extracorporeal membrane oxygenation (ECMO). The patient was successfully supported with ECMO for profound myocardial dysfunction and haemofiltration for rhabdomyolysis and acute renal failure. Patients who present with profound myocardial dysfunction and myoglobinuria as a consequence of viral infe

[Factors which allow elderly persons living alone to continue living at home--analysis of the situation of elderly people living alone who are served by the Higashi Sapporo Hospital Home Care Support Center and Higashi Sapporo Visiting Nurse Station].

Amid the rapid ongoing shift from treating the elderly in hospitals to treating them at home, self-care by elderly people living alone is a serious and complicated problem. To investigate the support system that enables elderly living alone to maintain social independence and a high quality of life at home, the main factors which allow the person to continue living at home were sought through an analysis of the current situations of 44 elderly-persons living alone, who use the services of our home care support center and visiting nurse station. From this analysis, the elderly person who contin

[Support system for families with sick children who need medical care at home].

I reported an experience of my family which had a child with a severe chronic neurological disease (subacute sclerosing panencephalitis, SSPE) and activities of private groups to support children with intractable diseases and their families in Japan. Home care system for these handicapped children is very important and meaningful to increase "quality of life" of them and their families. However, their families have to have a lot of physical, psychological and economical stress to perform it. We cannot say that the present supporting system in medical and welfare fields in Japan is sufficient y

The potential organ donor.

The source of most organs for transplantation is the brain-dead cadaveric donor. The realistic potential donor is one for whom mechanical ventilation and other support, to the point of brain death, is provided in the best interests of that patient, and not solely for the purpose of organ donation. Brain death secondary to spontaneous intracranial haemorrhage now exceeds traumatic head injury as a source of organ donors in Australia. There are now few medical exclusions to organ donation. Age limits rise constantly. No patient should be excluded without referral to the transplant coordinator. A

[Management of the organ donor].

Brain death is associated with loss of hypothalamic, pituitary and brain stem function resulting in apnea, bradycardia and hypotension, poikilothermia, and diabetes insipidus. In order to preserve body functions mechanical ventilation is continued with the aim to maintain an arterial partial pressure of oxygen of more than 100 mmHg. Previous fluid restrictions and the application of diuretics during the treatment of high intracranial pressure frequently result in dehydration. Progressive vasodilation may induce severe hypotension and fluid replacement with cristalloids and if necessary colloid

Multidisciplinary management of Duchenne muscular dystrophy from childhood to adulthood.

Duchenne muscular dystrophy (DMD) is a severe, progressive neuromuscular disorder characterized by muscle degeneration and weakness, leading to significant physical disability and multisystem complications. The management of DMD requires a coordinated, multidisciplinary approach to address the evolving needs of patients from childhood through adulthood, with a focus on patient quality of life. Early diagnosis and proactive interventions are critical to optimizing outcomes and may include physical therapy, respiratory support, cardiac monitoring, and orthopedic care. In children, the focus is o

Pain management in sickle cell disease: palliative care begins at birth?

People with sickle cell disease (SCD) are living longer, but their lives are impacted even more by the unpredictable intermittent or constant pain that is often poorly managed over a lifetime. To address this problem, an interdisciplinary team approach is needed that brings the medical professionals together for optimal compassionate care that is coordinated from the beginning of life and throughout the patient's lifespan. The hematologist, whenever possible, should take the lead. Effective models that have been developed to address SCD pain, such as the Day Hospital, The Recidivism Project and

Psychological Flexibility as a Mediator Between Spiritual Coping and End-of-Life Care Attitudes in ICU Nurses: A Cross-Sectional Study.

BACKGROUND: Intensive care unit (ICU) nurses frequently encounter ethical dilemmas, patient death and emotional distress, which can shape attitudes towards end-of-life care. Spiritual coping and psychological flexibility may support nurses' well-being and caregiving behaviour, but the mechanisms by which these factors affect ICU nurses' attitudes towards end-of-life care remain underexplored, particularly in Eastern cultural contexts. AIMS: To examine associations between spiritual coping, psychological flexibility and attitudes towards end-of-life care among ICU nurses, and to test whether p

Social support and depressive symptoms in older adults receiving hospice care: the mediating role of developmental task attainment.

BACKGROUND: Recognizing and addressing depressive symptoms among palliative care patients is essential, given their impact on quality of life, connections to increased physical symptoms, and links with heightened mortality risk. The current study aimed to identify the relationships between social support, developmental task attainment, and depressive symptoms in older hospice patients. METHODS: The study included 120 Polish older patients aged 60-87 years receiving home palliative care. Respondents completed the Geriatric Depression Scale, the Developmental Tasks Questionnaire for Seniors, th

The Child Life Imperative in Adult Palliative Care.

Palliative care, whether in the form of early Supportive Palliative Care or later in the course of illness with Hospice, serves not only the seriously ill patient, but the patient's family members. When the family includes children associated with the seriously ill patient, the needs of those children often go unmet, especially in adult palliative care. This paper focuses on the work done and the methods used by Certified Child Life Specialists (CCLSs) working in adult palliative care. Using the literature and our 15+ year experience integrating Child Life Services into both community and acad

[Multidisciplinary model of preparation of geriatric patients with senile asthenia syndrome for planned surgical intervention].

INTRODUCTION: Global demographic dynamics is shaping a steady «silver shift.» The growing proportion of the elderly population and the increase in the number of elective surgical interventions in patients of older age groups pose a challenge to the healthcare system. Against this background, the problem of preoperative preparation of geriatric patients is of strategic importance. Patients with signs of senile asthenia need multidisciplinary preoperative training to reduce the frequency of postoperative complications and improve their quality of life. The aim of the study is to substantiate the

The FICUS cluster randomized controlled trial of a family support intervention in adult intensive care units: mental health and family functioning outcomes.

PURPOSE: To examine the effect of a multicomponent, nurse-led, interprofessional family support intervention on family functioning and mental health in adult intensive care units (ICUs). METHODS: A cluster-randomized controlled trial in 16 Swiss ICUs compared an intervention-a family care pathway with specially trained ICU family nurses engaging and liaising with families, giving psychoeducational and relationship-focused care, and providing structured, interprofessional communication-to usual care. Family members of patients with an expected ICU stay of ≥ 48 h and a high risk of death, serio
