Peripheral T-cell lymphoma: review and updates of current management strategies.

The classification of T-cell and natural-killer- (NK-) cell lymphomas has been updated in the 4th edition of the World Health Organization (WHO) classification of tumors of the haematopoietic and lymphoid tissue published in 2008. Based on recent epidemiological studies, NK-cell lymphomas occur almost exclusively in Asia and South America, although T-cell lymphomas appear to occur in the East as commonly as in the West. Due to the low prevalence of this disease, diagnosis and optimal treatment of patients have not been studied prospectively in large randomized trials. Nevertheless, there has b

CDKN1B/p27 expression in peripheral T cell lymphoma not otherwise specified.

AIMS: Peripheral T cell lymphoma not otherwise specified (PTCL/NOS) is the commonest PTCL subtype. Recently, proliferation pathways have been found to be commonly altered in PTCL/NOS. CDKN1B/p27, a critical regulator of cell cycle and proliferation, has been suggested to be involved in T cell lymphomagenesis. This study aimed to evaluate the possible occurrence of CDKN1B/p27 aberrations in PTCL/NOS. METHODS: CDKN1B/p27 expression at RNA and protein level by DNA and tissue microarrays, in 28 and 98 cases, respectively, was studied. Additionally, direct sequencing of CDKN1B in 81 PTCL/NOS was p

Peripheral T-cell lymphoma gene expression profiling and potential therapeutic exploitations.

Peripheral T-cell lymphomas constitute a heterogeneous group with regard to diagnosis, treatment and prognosis. Efforts have been made to combine novel techniques with cytology and immunochemistry in order to more precisely define these entities. Molecular profiling has contributed to novel insights in the biology of T-cell lymphoma. Regarding anaplastic large cell lymphoma, low expression T-cell receptor signalling and high STAT3 target signatures have been associated with the ALK-positive subgroup. Gene expression profiling differentiates angioblastic T-cell lymphoma from other T-cell malign

Autologous transplantation for relapsed or primary refractory peripheral T-cell lymphoma.

Autologous transplantation (ASCT) is the standard of care for chemosensitive relapsed or primary refractory aggressive lymphoma, but little is known about its efficacy in the subset of patients with peripheral T-cell lymphoma (PTCL). We undertook a retrospective review of patients with PTCL who underwent ASCT for relapsed or refractory disease after responding to second-line therapy, excluding patients with indolent histologies and those with anaplastic lymphoma kinase (ALK) expressing anaplastic large cell lymphoma. The results of 24 patients with PTCL were compared with those of 86 consecuti

CT findings in peripheral T-cell lymphoma involving the gastrointestinal tract.

PURPOSE: To evaluate whether computed tomography (CT) accurately depicted gastrointestinal tract involvement in peripheral T-cell lymphoma (PTCL). MATERIALS AND METHODS: CT scans were retrospectively reviewed in 14 patients with pathologically proved PTCLs of the gastrointestinal tract for the following considerations: sites, patterns of involvement (ie, morphologic features, bowel wall thickness or mass size, and contrast enhancement pattern), and ancillary findings at other sites (ie, lymphadenopathy, bowel perforation, and involvement of other organs). RESULTS: PTCL involved the stomach i

[Hemifacial malignant granuloma as clinical manifestation of peripheral T-cell lymphoma].

The present clinical and pathological study describes the cases of two patients having infiltrate and ulcerous lesions of the nose, hard palate and rhinopharynx clinically consistent with a malignant midline granuloma. Histologically, infiltrates presented atypical lymphoid cells around small vessels and damage of the vascular wall. Immunohistologic study, using monoclonal antibodies on both fresh and paraffin embedded tissues, led to the positive diagnosis of peripheral lymphoma of T cells.

[Midline peripheral T-cell lymphoma--an immunohistochemical and electron microscopy study].

Twenty-six cases of nasal/nasopharyngeal midline peripheral T-cell lymphomas were studied clinically and pathologically with immunohistochemical markers and electronmicroscopy. A clearer understanding of the nature of midline malignant reticulosis has obtained. Immunohistochemical markers confirmed that except 5 small specimens which failed to show the monoclonal growth, 18 cases had peripheral T-cell lymphomas, 15 had TH, 3 Ts and 2 B-Cell lambda light chain and 1 HC. Morphological observations showed that 24 cases out of 26 exhibited middle and small polymorphic T-cells. This tumor had disti

Peripheral T-cell lymphoma with t(6;14)(p25;q11.2) translocation presenting with massive splenomegaly.

Recurrent chromosomal translocations associated to peripheral T-cell lymphomas (PTCL) are rare. Here, we report a case of PTCL, not otherwise specified (NOS) with the karyotype 46,Y,add(X)(p22),t(6;14)(p25;q11) and FISH-proved breakpoints in the IRF4 and TCRAD loci, leading to juxtaposition of both genes. A 64-year-old male patient presented with mild cytopenias and massive splenomegaly. Splenectomy showed diffuse red pulp involvement by a pleomorphic medium- to large-cell T-cell lymphoma with a CD2+ CD3+ CD5- CD7- CD4+ CD8+/- CD30- TCRbeta-F1+ immunophenotype, an activated cytotoxic profile,

Innovative approaches for managing relapsed or refractory peripheral T-cell lymphoma.

INTRODUCTION: Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of hematologic malignancies arising from mature, post-thymic T- and natural killer (NK) cells. This review explores innovative approaches to managing relapsed or refractory PTCL, emphasizing the need for tailored treatment strategies given the generally poor prognosis associated with these entities. AREAS COVERED: Literature selection followed a comprehensive search of PubMed, MEDLINE and EMBASE up to 2026. The conventional frontline approach remains cyclophosphamide, doxorubicin, vincristine, and prednisolone (CHOP)-

Safety considerations with the current treatments for peripheral T-cell lymphoma.

INTRODUCTION: Peripheral T-Cell Lymphomas (PTCL) constitute a heterogeneous group of aggressive T - and natural killer (NK)-cell disorders and are associated with a poor prognosis. Frontline treatments often consist of anthracycline-based combination chemotherapy with the exception of NK-T cell lymphomas, where such combinations are ineffective due to the presence of P-glycoprotein which leads to multidrug resistance. Infectious and immune mediated side effects might be more pronounced in or unique to T-cell lymphomas due to the selection of agents which target multiple T-cell subtypes and als

Clinicopathologic Features of Peripheral T-Cell Lymphoma in Sub-Saharan Africa.

OBJECTIVES: Peripheral T-cell lymphomas (PTCLs) are heterogeneous, clinically aggressive, and rare. Subtype distribution varies by geographic location; however, data from sub-Saharan Africa (SSA) are lacking. We sought to elucidate clinicopathologic features of PTCL in SSA. METHODS: We reviewed PTCL consultation cases from three SSA countries. PTCL subtype was determined per 2017 World Health Organization classification. Cases with sufficient material were evaluated by polymerase chain reaction for human T-cell leukemia virus type 1 (HTLV-1) and T-cell receptor γ (TCRG) rearrangement. RESULT

Epigenetic alterations and advancement of treatment in peripheral T-cell lymphoma.

Peripheral T-cell lymphoma (PTCL) is a rare and heterogeneous group of clinically aggressive diseases associated with poor prognosis. Except for ALK + anaplastic large-cell lymphoma (ALCL), most peripheral T-cell lymphomas are highly malignant and have an aggressive disease course and poor clinical outcomes, with a poor remission rate and frequent relapse after first-line treatment. Aberrant epigenetic alterations play an important role in the pathogenesis and development of specific types of peripheral T-cell lymphoma, including the regulation of the expression of genes and signal transductio

The Future of Combination Therapies for Peripheral T Cell Lymphoma (PTCL).

PURPOSE OF REVIEW: Peripheral T cell lymphoma is a rare heterogeneous group of diseases which are characterized by poor outcomes to treatment and short overall survival. In the past decade, several new therapies targeting T cell biology have been approved in the relapsed setting. These new therapies, such as pralatrexate, romidepsin, belinostat, and brentuximab vedotin, have begun to make their way into practice. Despite these advances, outcomes have not changed dramatically. In recent years, efforts have been made to incorporate these new therapies into combination strategies to treat this ch

Molecular Insights Into Pathogenesis of Peripheral T Cell Lymphoma: a Review.

PURPOSE OF REVIEW: Peripheral T cell lymphoma (PTCL) is a heterogeneous group of lymphoproliferative neoplasms, with at least 29 distinct entities described in current WHO classification. Using present diagnostic approaches, more than a third of PTCL cases cannot be classified, hence designated as PTCL-not otherwise specified (PTCL-NOS). Herein, we summarize the current genomic findings and their role in the molecular pathogenesis in different PTCL entities. RECENT FINDINGS: Gene expression profiling (GEP) studies have identified distinct molecular signatures for accurate diagnosis and elucid

Endoscopic detection of the gastric lesions of peripheral T-cell lymphoma.

An 82-year-old Japanese man presented with a gastric involvement of peripheral T-cell lymphoma, not otherwise specified. Although gastrointestinal lesions were not detected on computed tomography, oesophagogastroduodenoscopy revealed a slight elevation of the gastric mucosa, with changes in mucosal colour and the presence of abnormal microvessels. This led to the prompt detection of gastric involvement in lymphoma. This case highlights the usefulness of detailed observation of the gastric mucosa for the endoscopic detection of gastric involvement of peripheral T-cell lymphoma.
