BACKGROUND: Rituximab, a monoclonal anti-CD20 antibody, in combination with chemotherapy improves overall survival compared to chemotherapy alone when used for induction therapy for patients with newly diagnosed or relapsed indolent lymphoma. Randomised controlled trials have demonstrated that maintenance treatment with rituximab prolongs progression-free survival but evidence of effect on overall survival is lacking.

The importance of maintenance care for controlling periodontitis and preventing disease recurrence is well-documented. Supportive periodontal therapy is essentially based on periodontal risk assessment, oral hygiene reinforcement and professional cleaning. As most of the referred periodontal patients return to the general practitioner at some point, supportive periodontal therapy should be continued in general practice. This manuscript offers the clinician, in particular the general practitioner, a number of guidelines to organize maintenance care.

Two studies assessed the role of mental imagery in the maintenance of fear of common phobic stimuli. A comparative study of withdrawal states of 7 methadone maintenance patients and 21 heroin addicts was carried out. The protracted and low predictable course of methadone withdrawal states, along with relatively high resistance to clonidine and other medications, was demonstrated. Similarity of mental disorders, including addictive personality disorders, was found in both groups of patients.

Two 18-month, randomised, double-blind trials have compared lamotrigine, lithium, and placebo as maintenance treatment in a total of 1315 recently manic or depressed patients with bipolar I disorder. We describe the case of a 65-year-old woman with long-standing, atypical facial pain and fluctuating mental symptoms. A variety of psychiatric diagnoses were made over a period of nine years but no effective treatment was found. Ultimately, the patient was found to suffer from a psychotic depression indicating Cotard's syndrome. Two series of ECT were then given with temporary short remissions. Subsequently, continuation ECT was introduced. To our knowledge, continuation ECT in Cotard's syndrome is rarely, if ever, described in the literature.
