Reconstruction of Dentomaxillofacial Deformity Secondary to Mandibular Defect Using Concomitant Orthognathic Surgery and Fibula Free Flap.

The reconstruction of mandibular defects may be delayed or compromised for many reasons, especially in pediatric patients. With the growth of the remaining mandible and the maxilla in the malocclusion status, secondary dentomaxillofacial deformity is plausible. To treat the concomitant mandibular defect and secondary dentomaxillofacial deformity, a hierarchical algorithm using orthodontics, orthognathic surgery, and fibula free flap was developed. This retrospective case series included six patients with long-term mandibular defects caused by tumor resection without repair or with compromised

Donor-Site Morbidity Following Free Fibula Flap Harvest for Mandibular or Maxillary Reconstruction in Pediatric Patients.

INTRO AND AIM: The morbidity at the donor-site in the free fibula flap is a well-studied fact in the adult population, but in children, there are few reports of the long-term adverse effects at the donor site. In this study, we evaluate donor-site morbidity, complications, and functional outcome in pediatric patients undergoing free fibula flap harvest for mandibular or maxillary reconstruction, and the relation between pain and walking abilities. METHODS: A retrospective study of 22 pediatric patients undergoing free fibula flap harvest for mandibular and maxillary reconstruction between 200

Mandibular reconstruction using plates prebent to fit rapid prototyping 3-dimensional printing models ameliorates contour deformity.

BACKGROUND: Recently, medical rapid prototyping (MRP) models, fabricated with computer-aided design and computer-aided manufacture (CAD/CAM) techniques, have been applied to reconstructive surgery in the treatment of head and neck cancers. Here, we tested the use of preoperatively manufactured reconstruction plates, which were produced using MRP models. The clinical efficacy and esthetic outcome of using these products in mandibular reconstruction was evaluated. METHODS: A series of 28 patients with malignant oral tumors underwent unilateral segmental resection of the mandible and simultaneou…

Technical refinements in mandibular reconstruction with free fibula flaps: outcome-oriented retrospective review of 99 cases.

Congenital disease, major trauma, tumour resection and biphosphonate-related osteonecrosis can lead to partial, subtotal, or total loss of the mandibular bone. Minor defects can be easily reconstructed using bone grafts, whereas microvascular free tissue transfer may be unavoidable in the case of major bone loss or poor quality of soft tissue. Simple bone or composite osteocutaneous fibula free flaps have proven invaluable and remain the workhorse for microvascular mandibular reconstruction in daily practice. Our experience with 99 consecutive fibular free flaps confirms the available data in …

Mandibular reconstruction with microvascular re-anastomosed fibular free flaps - two complementary methods of postoperative transplant monitoring.

PURPOSE: Evaluation of two complementary methods, i.e. luminescence ratiometric oxygen imaging (LROI) and contrast-enhanced ultrasound (CEUS) for postoperative evaluation of the perfusion of osseocutaneous fibular free flaps used for mandibular reconstructions. PATIENTS AND METHODS: 15 patients with microvascular re-anastomosed osseocutaneous fibular flaps were included in this trial. Perfusion of the cutaneous part of the flap was determined by measuring a fluorescent oxygen sensor foil covering the flap's skin surface with a handheld fluorescence-microscope. The sensor contains a reservoir …

Three dimensional evaluation of facial asymmetry after mandibular reconstruction: validation of a new method using stereophotogrammetry.

The scientific literature is sparse on reports that evaluate facial asymmetry after mandibular reconstructive surgery objectively. The aim of this study is to introduce and validate a new method, using three dimensional (3D) stereophotogrammetry, that quantifies soft-tissue facial asymmetry in patients who have undergone mandibular reconstruction. To validate the new method, two observers applied the method on 3D photographs of five patients and five controls. An inter-observer difference of 0.04 mm (-0.08 to 0.17) was found with a measurement error of 0.13 mm. 15 3D photographs of the mandibu…

Distraction osteogenesis versus fibula free flap for mandibular reconstruction after gunshot injury: socioeconomic and technical comparisons.

BACKGROUND: Since the birth of maxillofacial surgery, ballistic injuries are a real challenge for surgeons. For more than 20 years, fibula free flap (FFF) became the criterion standard for mandibular reconstructions. But FFF is not always the perfect answer for mandibular reconstruction, and a technique named distraction osteogenesis (DO) was raised. The purposes of this study are to estimate the financial cost and reattempt for the patient each techniques in our experience to determine the best-choice criteria for mandibular reconstruction after gunshot injury. METHODS: We performed a retros…

Analysis of complications after reconstruction of bone defects involving complete mandibular resection using finite element modelling.

BACKGROUND: In a retrospective study, risk factors for complications after the bridging of mandibular defects using reconstruction plates were reviewed. Especially the loosening of the plate-screw-mandible complex should be analyzed with a finite element model in order to reduce plate complications in future. PATIENTS AND METHODS: We examined 60 patients who underwent a treatment with reconstruction plates after tumor resection during a period of 10 years. The problem of screw loosening was additionally reviewed by means of a finite element study, and a model for the loosening process was dev

Technical considerations for implant rehabilitation in mandibular reconstruction with fibula bone free flap in oncologic patients.

Mandibular reconstruction in oncologic surgery using free bone flaps allows restoration of facial symmetry, preservation of key orofacial functions, and facilitation of adjuvant treatments. Despite these advances, dental rehabilitation remains challenging. Implant-supported prostheses seem to improve functional outcomes compared with conventional removable prostheses; however, there is still no consensus regarding optimal implantation strategies. This technical note proposes a structured, implant-oriented decision-making framework for mandibular reconstruction using free fibula flaps. It integ…

A novel 3D-printed implant system for mandibular reconstruction (IPS® MotionPlate) - a finite element analysis.

After continuity resection of the jaw, free bony flaps are used for reconstruction. Despite implementation of a digital workflow and additively manufactured custom-made implants for flap fixation, plate complications such as bony non-union remain a clinical challenge. This is the first description of the finite element analysis of a novel 3D-printed implant system for mandibular reconstruction designed to overcome this limitation. Virtual surgical planning and patient-specific finite element analysis were performed in 12 cases requiring segmental mandibulotomy. The novel custom-made implants (…
