Is there a relationship between mastoid pneumatisation and facial canal dimensions?

OBJECTIVE: To evaluate mastoid pneumatisation and facial canal dimensions. METHOD: In this retrospective study, 169 multidetector computed tomography scans of temporal bone were reviewed. Facial canal dimensions were evaluated at the labyrinthine, tympanic and mastoid segments using axial and coronal multidetector computed tomography scans of temporal bone. Mastoid pneumatisation and facial canal dehiscence were evaluated. Facial canal dehiscence was measured if it was found to be present. RESULTS: This study showed that facial canal dimensions decreased in pneumatised mastoids. Facial canal…

Temporal bone trauma effects on auditory anatomical structures in mastoid obliteration.

PURPOSE: The risk of temporal bone fractures in head trauma is not negligible, as injuries also depend on the resistance and integrity of head structures. The capacity of mastoid cells to absorb part of the impact kinetic energy of the temporal bone is diminished after open cavity mastoidectomy, even if the surgical procedure is followed by mastoid obliteration. The aim of our study was to evaluate the severity of lesions in auditory anatomical structures after a lateral impact on cadaveric temporal bones in which open cavity mastoidectomy followed by mastoid obliteration was performed, compar…

Congenital mastoid cholesteatoma presenting as a mass obstructing external auditory canal.

We describe the clinical presentation, investigation and management of an eventually fatal case of hypercalcemic crisis due to primary hyperparathyridism (PHPT). A 60 year-old lady with history of urolithiasis presented with worsening generalized bone pain, spinal scoliosis and a limp. Laboratory data showed hypercalcemia and raised alkaline phosphatase. Left hip x-ray revealed a subcapital femoral neck fracture. Intact parathyroid hormone was elevated, 187.6 pmol/L (1.6 - 6.9) and ultrasound showed an enlarged right parathyroid gland. Despite initial reduction of serum calcium with saline inf

Do high-speed drills generate high-frequency noise in mastoid surgery?

BACKGROUND: The introduction of increasingly high speed drills for mastoid surgery has heightened the concern that cochlea damage may occur in both the operated and nonoperated ear. It has been observed clinically that this damage could be associated with frequencies above 8,000 Hz and that, to observe these changes, high-frequency audiometry should be performed. Previous studies have investigated noise transmission to the cochlea at frequencies below 4,000 Hz only. There having been, until recently, limitations to the equipment available to measure higher frequencies. OBJECTIVE: To define th…

[Burow's solution treatment for external auditory canal and mastoid cavity cholesteatoma].

Burow's solution, 13% aluminum acetate dissolved in water developed as ear drops by German Dr. Karl August Burow in the mid-1800s, was confirmed by Mahoney (1980) and Thorp et al. (2000) to act on chronic suppurative otitis media without ototoxicity. We have found it satisfactory in treating otitis media and other intractable inflammation and fungal infection of the external auditory canal. We report its potent effect in 2 cases of external auditory canal cholesteatoma (EACC) and 1 of mastoid cavity cholesteatoma (MCC)-the first such report insofar as we knew. Case 1 of EACC involved an 8-year…

Pediatric external canal cholesteatoma with extensive invasion into the mastoid cavity.

Cholesteatoma in the external auditory canal (EAC) is an uncommon situation, and is especially rare in pediatric patients. We report two pediatric cases of external canal cholesteatoma with extensive invasion into mastoid cavity. Both cases had otalgia and poor hearing as the initial symptoms, and received operation according to the extent of the lesions. Since external canal cholesteatoma with extensive invasion into the mastoid cavity has the propensity to involve the vertical segment of the facial nerve, extreme care should be taken when performing any procedure in this area. Through thorou…

[Primary intracranial Ewing's sarcoma of the mastoid bone. A case report].

INTRODUCTION: Ewing s sarcoma (ES) is a tumorous process that is found mainly in long bones and the pelvis, but its primary location is not often the intracranial zone. CASE REPORT: We report the case of a 5 year old patient who visited surgery 30 days before hospital admission with increased volume of the left mastoid bone, torticollis, balance disorders and discoordinated movements. A computerised tomography brain scan confirmed our suspicions of a tumorous lesion that was destroying the left mastoid bone, and was invading the projection of the left hemispherium cerebri and edges of the bra…

Gas exchange function through the mastoid mucosa in ears after surgery.

Gas exchange function through the mastoid mucosa was investigated in ears after surgery using nitrous oxide. Increase in the mastoid pressure was assessed by a micropressure sensor placed in the mastoid cavity during the second-stage revision operation performed under general anesthesia using 67% nitrous oxide, 33% oxygen, and sevoflurane on 14 ears with chronic adhesive otitis media or cholesteatoma as well as on seven ears without inflammation as controls. All seven control ears showed pressure increase in the mastoid in various degrees. In the 14 postoperative ears, nine of the 10 ears on w…

Endolymphatic mastoid shunt for Menière's disease: do results change over time?

In 1983, results of a 5-year study using the endolymphatic mastoid shunt for treatment of Menière's disease were reported. Forty-eight patients were followed for between 1 and 5 years postoperatively, and 81% obtained satisfactory relief of vertigo. In the present analysis, 24 of these patients responded to a questionnaire, participated in a direct interview, and completed audiometric evaluations at 7 to 11 years after surgery. Eighty-three percent of these patients reported satisfactory relief of their vertigo. Results for hearing, tinnitus, and limitation of activities also varied little bet…
