A meta-analysis of selected maternal and fetal factors for perinatal mortality.

BACKGROUND: In several developing countries, achieving Millennium Development Goal 4 is still off track. Multiple maternal and fetal risk factors were inconsistently attributed to the high perinatal mortality in developing countries. However, there was no meta-analysis that assessed the pooled effect of these factors on perinatal mortality. The purpose of this meta-analysis was to identify maternal and fetal factors predicting perinatal mortality. METHODS: In this meta-analysis, we included 23 studies that assessed perinatal mortality in relation to antenatal care, parity, mode of delivery, g

Pregnancy with hypertrophic cardiomyopathy--a potential risk factor for maternal mortality.

We report a case of a 21 year old married woman diagnosed to have severe hypertrophic cardiomyopathy at 12 weeks of gestation by echocardiograph at Lata Mangeshkar Hospital, Nagpur on 12th June, 2012. Her mother's and maternal uncle's sudden death at young age along with severe form of disease brought a lot of apprehension in the beginning considering the possible catastrophic effect of hemodynamic changes associated with pregnancy and labor. She was asymptomatic before pregnancy but developed dyspnoea on exertion during pregnancy and hence was started on calcium channel blocker verapamil. She

Implementing a maternal mortality surveillance system in Morocco - challenges and opportunities.

OBJECTIVE: To describe the development of the maternal death surveillance system (MDSS) in Morocco and discuss the initial results. METHOD: The nationwide MDSS was implemented in 2009 with the involvement of health professionals and local authorities. It comprises (i) notification of all deaths of women of reproductive age (from 15 to 49 years); (ii) a preliminary survey to identify pregnancy-related deaths; (iii) a confidential enquiry into all pregnancy-related deaths. The information thus obtained describes socio-demographic characteristics of the women, their obstetric and medical history

[Impact of maternal antecedents on neonatal mortality in a regional perinatal hospital].

OBJECTIVE: Determine the prevalence of maternal risk factors and evaluate their impact on neonatal mortality in a regional perinatal center. MATERIALS AND METHODS: A cohort of 25,365 live newborns was studied between January 1st 2000 and December 31st 2004. Maternal antecedents were registered in a data base: sociodemographic; medical history; obstetric antecedents of previous pregnancies; as well as evolution of current pregnancy and birth. Newborn birth weight, gestational age and condition at discharge were registered too. Neonates who died were considered cases and controls those discharg

The paradox of obstetric "near misses": converting maternal mortality into morbidity.

OBJECTIVE: To systematically review evidence of obstetric near-misses and their consequences. DATA SOURCES: PUBMED, OVID, and references of retrieved articles were used. METHODS OF STUDY SELECTION: Only 13 original articles describe the occurrence of obstetric/maternal near-miss morbidity to date. All were included in this review, in addition to other articles related to the epidemiology and consequences of severe acute maternal morbidity. TABULATION, INTEGRATION, AND RESULTS: Serious forms of maternal morbidity occur in about 1% of women in the United States compared to 3.01 to 9.05% in so

Access to care and maternal mortality in Jamaican hospitals: 1993-1995.

BACKGROUND: As part of the reproductive health quality assurance programme, the Ministry of Health sought to review maternal deaths in public hospitals. These hospitals attend 95% of institutional births and 82% of all births. METHODS: Deaths among females 10-50 years in public hospitals during 1993-1995 were reviewed to identify pregnancy-related deaths. Cause of death and access to care were compared with previous studies (1981-1983 and 1986-1987 [12 months]). RESULTS: The maternal mortality ratio of 106.2 per 100 000 live births, was no different than the 119.7 observed in 1986-1987 and 1…

Influence of maternal ethnicity on infant mortality in Chicago, 1989-1996.

This study compared infant mortality rates between large ethnic groups in Chicago from 1989-1996. Infant mortality information about ethnic groups was compared using data from annual reports published by the Epidemiology Program, Department of Public Health, City of Chicago and vital statistics documents in Illinois, which include information on ethnicity. Chi-squared analysis was used to evaluate the differences between the proportions. A P value of < .05 was considered significant. During the study period, there were 461,974 births and 6407 infant deaths in Chicago. African Americans contrib

The outcome of 32 pregnancies in renal allograft recipients is reported. The mean age at the time of conception was 27.3 years (range, 20 to 37) with an average interval of 47 months from the time of transplantation to conception (range, 2 to 163). Twenty-nine patients received the graft from a living related donor, one from a living no related donor an 2 from cadaver donors. All patients continued their immunosuppressive regimen during pregnancy and only 6 patients were taking cyclosporine A. Hypertension during pregnancy was observed in 10 patients (31%), superimposed preeclampsia in 4 (14%)

Increased maternal mortality in The Netherlands from group A streptococcal infections.

OBJECTIVE: To assess genital tract sepsis-related maternal mortality in The Netherlands during 1983-1992. STUDY DESIGN: A nationwide Confidential Enquiry into the causes of maternal death. RESULTS: Genital tract sepsis-related maternal mortality during 1983-1987 was 0.11 per 100,000 live births (1/893,998) and 0.93 per 100,000 live births (9/968,990) during 1988-1992. The relative risk of dying due to sepsis in the second 5-year period as compared to the first period was 10.1 (1/72 versus 9/72 maternal deaths; 95% CI 1.3-82.3; P < 0.01). CONCLUSIONS: Also in The Netherlands an increase of d…

Maternal mortality at twelve teaching hospitals in Indonesia-an epidemiologic analysis.

Records on 36,062 maternity cases admitted to 12 teaching hospitals throughout Indonesia between 1977 and 1980 were analyzed. A hospital maternal mortality rate of 37.4/10,000 cases (39.0/10,000 live births) was derived that was about ten times higher than rates reported from developed countries in the early seventies. Hemorrhage, infection and toxemia accounted for 91.2% of deaths resulting from direct obstetric causes and for 86,1% of total deaths. It is postulated that if all pregnant women received adequate antenatal care, and if all women wanting no additional children were sterilized, ma…
