The authors aim to propose combination of Surgically Assisted Miniscrew-Assisted Rapid Palatal Expansion (SAMARPE) with orthopedic traction miniplates placement in cases where a sagittal deficiency coexists with the necessity of carrying out a surgical maxillary expansion. Unlike the conventional orthopedic traction technique, where the miniplates are placed bilaterally in the infrazygomatic crest of the maxillary buttress, in this scenario the upper miniplates should be placed below the LeFort I osteotomy, and therefore a little bit angulated.

INTRODUCTION: Edentulism causes progressive bone resorption of the maxillae, which can lead to altered maxillo-mandibular relationships. The aim of the study was to evaluate the applicability of guided bone regeneration (GBR) to Le Fort I osteotomies with interpositional bone grafts for treatment of patients with severe maxillary atrophy.
MATERIALS AND METHODS: Twenty consecutive patients characterized by severely atrophic maxillae were treated from January 2003 to January 2006 in order to resolve maxillary edentulism. All patients underwent pre-prosthetic surgery, including a Le Fort I osteo

PURPOSE: Deformities of the maxilla may exist on all planes, and we treat these problems by mobilizing and repositioning the maxilla on all planes. Some authors have examined the effects of maxillary movement on the nasal airway. In clinical, practice, active anterior rhinomanometry is the most reliable and frequently used method to assess nasal respiratory function. The purpose of this study was to evaluate changes in nasal airflow and nasal airway resistance after maxillary movement performed by active anterior rhinomanometry.
MATERIALS AND METHODS: We studied 25 patients (18 female patient

BACKGROUND: Inferior scleral exposure (ISE) is an important determinant of periorbital esthetics and overall facial harmony. While effects of linear maxillary movements are documented, influence of sagittal-plane maxillary rotational vectors on ISE remains insufficiently investigated.
PURPOSE: The purpose of this study was to measure the association between sagittal-plane maxillary rotational changes and ISE in subjects undergoing LeFort I osteotomy.
STUDY DESIGN, SETTING, AND SAMPLE: This retrospective cohort study was conducted at Medipol Mega University Hospital (Istanbul, Türkiye) and in

Distraction osteogenesis (DO) can generate new bone in a gap between 2 vascularized bone surfaces in response to application of graduated tensile stress across the bone gap. The authors present the clinical result in a cleft patient with severe maxillary deficiency treated by a rigid external distraction (RED) device. A boy complained of both masticatory and psychological problems because of cleft with severe midfacial retrusion. The treatment aimed to create a well-balanced facial profile, increase maxillary incisal display, create proper overjet and overbite, and align his dentition. By the

Changes in dental and skeletal relationships were evaluated 5 years postsurgically in 49 patients whose maxilla had been superiorly repositioned by a Le Fort I osteotomy. All of the patients had at least 2 mm of intrusion at the maxillary incisor and molar; none had a mandibular ramus osteotomy or other osteotomy except genioplasty. Only 6.5% had 2 mm or greater of net vertical change in skeletal or dental landmarks at 1 year postsurgery. From 1 to 5 years postsurgery, minimal changes in skeletal and dental landmarks occurred in the majority of the patients, but approximately 25% of the patien

INTRODUCTION: When dealing with maxillary sinus pathology, some areas of the sinus remain difficult to examine. In this regard, the pre-lacrimal approach is a minimally invasive technique to reach anterolateral areas of the maxillary sinus while preserving the physiological nasal function.
MATERIALS AND METHODS: The present study aimed to provide technical hints related to pre-lacrimal approach acquired through a large number of performed procedures.
RESULTS: According to the results, the mucosa incision was performed more anteriorly than the osteotomy using the proposed surgical variant. Mo

Most of the "narrow faces" are secondary to a maxillo-malar hypoplasia consecutive to chronic nasal obstruction. Functional malar valgisation by transmalar osteotomy allows a simple and safe correction of this dysmorphic aspect. Interpositional bone graft and osteosynthesis by miniplate avoid mucous inflammation, maxillary sinusitis and/or relapse observed with use of biomaterials. Correction of "narrow faces" by malar osteotomy is natural and long term results are stable. Functional and esthetic rhinoplasty is often associated. Our clinical experience in 15 cases is reported.

The pterygomaxillary and zygomatic implants offer a viable alternative to maxillary sinus floor elevation in treating atrophic posterior maxilla, providing the benefit of immediate restoration. However, the limited visibility and the complex anatomy of the maxilla make the surgery more challenging. This brief clinical study describes a subsequent guide for placing pterygomaxillary and zygomatic implants and performing the osteotomy of the maxillary sinus bone window. This approach ensures precise implant placement and helps avoid intraoperative complications.

OBJECTIVES: During Le Fort I osteotomy, the separation of the pterygomaxillary junction (PMJ) is a difficult procedure for most surgeons because it is invisible. In this process, damage to the posterior structures constituting the sinus or those adjacent to it, including the maxillary sinus posterior wall and pterygoid plate, may occur. We would like to investigate the effects of this on the inside of the maxillary sinus after surgery and whether there are complications.
MATERIALS AND METHODS: One-hundred patients who underwent Le Fort I osteotomy from 2013 to 2020 using cone-beam computed to

A systematic review search was based on the PICOS approach, as follows: population: cleft lip and palate patients; intervention: Le Fort I osteotomy; comparator: different surgical protocols; outcome: stability, recurrence or surgical complications; study designs: only case reports were excluded from the review. No restrictions were placed on language or year of publication. Risk of bias was analyzed, heterogeneity was assessed, and subgroup analysis was performed using a level of significance of 1% (p = 0.01). The search identified 248 citations, from which 29 studies were selected and a tota

OBJECTIVES: Inaccuracies in orthognathic surgery can be caused during face-bow registration, model surgery on plaster models, and intermaxillary splint manufacturing. Electromagnetic (EM) navigation is a promising method for splintless digitized maxillary positioning.
STUDY DESIGN: After performing Le Fort I osteotomy on 10 plastic skulls, the target position of the maxilla was guided by an EM navigation system. Specially implemented software illustrated the target position by real-time multistage colored three-dimensional imaging. Accuracy was determined by using pre- and postoperative cone

INTRODUCTION: Becker's nevus is a dermatological disease sometimes associated with a malformation such as hemifacial bone dysplasia.
OBSERVATION: A 11-year-old male patient presented with a delayed growth of the left maxillary and dental impactions. At 15 years of age, Becker's nevus appeared on the upper torso, both shoulders and cheeks. Le Fort I segmented osteotomy with an iliac bone graft was complicated because of maxillary dysplasia.
DISCUSSION: The chronology of our patient's cutaneous and maxillary abnormalities was similar to previously reported cases. Conversely, the Becker's nevus

Maxillary advancement may result in movement of the posterior border of the hard palate with its soft palate attachment, which may cause impairment of velopharyngeal (VP) function. We examined VP function before and after Le Fort I osteotomy in 15 cleft lip and palate patients. The extent of maxillary advancement was measured by means of standard cephalometric radiographs taken before and after the operation. VP function was evaluated in terms of perceptual speech assessments, pressure-flow data and nasalance scores preoperatively and 2, 6 and 12 months after the operation. The results showed

Potential complications related to pterygomaxillary disjunction have been widely described in the literature, most of them being due to the inaccurate and blind approach involved. The present study used preoperative virtual planning to establish a surgical cutting guide for pterygomaxillary osteotomy. It was placed in the maxillary tuberosity supported by molars, and a flapless vertical osteotomy was performed with a piezoelectric saw. Then, maxillary down-fracture was performed with slight pressure through an anterior approach. The use of the surgical guide added accuracy and predictability t…
