Continuous Medicaid During COVID-19 and Children's Health, Development, and Academic Outcomes.

OBJECTIVES: Medicaid can improve children's health, development, and education, and may reduce racial-ethnic inequities. This study estimates the association between implementing continuous Medicaid eligibility under the Families First Coronavirus Response Act (FFCRA) and children's health, healthcare access, development, and academic outcomes overall and by race-ethnicity. METHODS: We used National Survey of Children's Health data and difference-in-difference models to compare outcomes among children in states newly implementing continuous eligibility under the FFCRA to states with prior pol…

Opioid Prescriptions and Adverse Events Among Medicaid Enrollees with Disabilities, 2016-2021.

INTRODUCTION: Individuals with disabilities experience disproportionately high rates of prescription opioid use and opioid-related harms, yet national trends among Medicaid enrollees with disabilities remain poorly understood. METHODS: This repeated cross-sectional study used data from the T-MSIS Analytical Files 2016-2021 to examine trends in opioid prescriptions and adverse events among Medicaid enrollees with disabilities. Outcomes included annual opioid prescription status (none, short-term, long-term), total opioid dosage, daily opioid dosage, and opioid-related emergency department visi…

Cardiovascular Multimorbidity and Associated Mortality Among Medicare Beneficiaries Dually Eligible for Medicaid.

BACKGROUND: Patients dually eligible for Medicare and Medicaid have higher mortality rates and disparate outcomes for acute cardiovascular conditions compared with those eligible for Medicare alone. However, the association between dual eligibility, cardiovascular disease (CVD) multimorbidity development, and subsequent mortality has not been well characterized in patients dually eligible for Medicare and Medicaid. METHODS: We analyzed Medicare claims data from a 5% random sample of fee-for-service beneficiaries (2010-2019). Our primary exposure was Medicaid dual eligibility. The primary outc…

Medicaid Telehealth Policy in Marginalized Communities: Perspectives From Virginia Patients and Providers.

We sought to understand stakeholder experience with telehealth services, through interviews with patients, providers, and health plans, to inform Medicaid policy after the COVID-19 public health emergency. Our primary aim was to examine whether and how such telehealth policies affect equitable care delivery and to uncover any remaining policy gaps. Applying the Framework for Digital Health Equity our study identified digital determinants that operate at the individual, interpersonal, community and societal levels. Across respondents, telehealth expansion was viewed as overwhelmingly positive a…

Experiences with Medicaid unwinding among low-income adults in 4 southern states.

After states initiated Medicaid redeterminations following the end of the Medicaid continuous coverage provision, millions of people lost Medicaid coverage. To date, there is limited evidence evaluating how people experienced this "unwinding" process. Using a survey of low-income adults in 4 southern states, we assessed awareness of unwinding, experience with state-based outreach, and understanding of coverage changes. Over half of respondents with some Medicaid enrollment experience (57%) reported having heard "a little," "some," or "a lot" about unwinding, with media as the most common sourc…

Facility Medicaid Payer Burden and Nonelective Admission for Chronic Limb-Threatening Ischemia.

IMPORTANCE: Chronic limb-threatening ischemia (CLTI) is a major public health issue that requires considerable human and physical resources to provide optimal patient care. It is essential to characterize the disease severity and resource needs of patients with CLTI presenting to facilities of varying resource capacities. OBJECTIVE: To investigate the association between facility-level Medicaid payer proportions and the incidence of nonelective admissions among patients admitted for CLTI. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective multicenter cohort study, 876 026 CLTI-related …

Impact Of Medicaid Expansion On HIV Pre-Exposure Prophylaxis Coverage, 2012-23.

Although its availability has grown during the past decade, pre-exposure prophylaxis (PrEP) remains underused in the US. We evaluated the impact of Medicaid expansion on state-level PrEP prescribing outcomes, using HIV surveillance data from all fifty states and Washington, D.C., from the period 2012-23, using a staggered diffeence-in-differences approach. PrEP coverage (prescriptions per 100,000 population) increased over time but was not statistically significantly associated with Medicaid expansion. There were, however, significant increases in the PrEP-to-need ratio (4.44 PrEP prescription…

Participation in Tobacco Cessation Programs Among Medicaid Managed Care Enrollees in Florida.

Background/Objectives: Tobacco use remains a significant public health issue, particularly among individuals with low incomes, including Medicaid recipients who often face multiple barriers to quitting. This study aimed to identify barriers, from the perspective of Medicaid managed care organizations (MCOs), influencing Medicaid recipient participation in tobacco cessation programs. Methods: Focus group interviews were conducted with Florida Medicaid MCOs to elicit processes for case identification, outreach, referral, program participation, and incentives. Answers were synthesized into themes…

Expanding comprehensive pregnancy care for Emergency Medicaid recipients: a cost-effectiveness analysis.

BACKGROUND: Emergency Medicaid is a restricted benefits program for individuals who have low-income status and who are immigrants. OBJECTIVE: This study aimed to compare the cost-effectiveness of 2 strategies of pregnancy coverage for Emergency Medicaid recipients: the federal minimum of covering the delivery only vs extended coverage to 60 days after delivery. STUDY DESIGN: A decision analytical Markov model was developed to evaluate the outcomes and costs of these policies, and the results in a theoretical cohort of 100,000 postpartum Emergency Medicaid recipients were considered. The payo…

Perceived Social Status and Oral Health Among Medicaid Insured Adults in Iowa.

PURPOSE: Perceived social status (PSS), a measure of social status, reflects cumulative lifetime effects of an individual's relative social status based on resources and lived experiences. PSS is hypothesized to better capture social status compared to traditional measures of socioeconomic status (SES) (i.e., education, occupation, and income). Although recognized as a predictor of health-related morbidity and mortality, limited research has explored PSS and oral health. This study investigated PSS as a predictor of self-reported oral health among low-income adults. METHODS: In spring 2018, a

Examining recurrent hurricane exposure and psychiatric morbidity in Medicaid-insured pregnant populations.

The wide-ranging mental health consequences of a major hurricane have been widely documented, but important gaps remain in understanding the mental health burden of recurrent exposure to multiple hurricanes. The objective of this study was to examine the mental health burden in at-risk pregnant populations recurrently exposed to major hurricanes Matthew (2016), Florence (2018), Michael (2018), and Dorian (2019). Daily emergency department (ED) admissions were obtained on pregnant Medicaid beneficiaries for incident maternal disorders of pregnancy (MDP), perinatal mood and anxiety disorders (PM…

Reexamining Medicaid Patient Review and Restriction Programs Amid the Evolving Opioid Epidemic.

PURPOSE: Accurate quantification measurement of tear meniscus is vital for the precise diagnosis of dry eye. In current clinical practice, the measurement of tear meniscus height (TMH) relies on doctors' manual operation. This study aims to propose a novel automatic artificial intelligence (AI) system to evaluate TMH. METHODS: A total of 510 photographs obtained by the oculus camera were labeled. Three thousand and five hundred images were finally attained by data enhancement to train the neural network model parameters, and 60 were used to evaluate the model performance in segmenting the cor

Utilization of Left Ventricular Assist Devices in Vulnerable Adults Across Medicaid Expansion.

BACKGROUND: Continuous-flow left ventricular assist device (LVAD) implantation is a payor sensitive procedure influenced by preoperative comorbidities and social factors. Whether expansion in insurance coverage will further influence device utilization is unknown. This study sought to assess the effects of Medicaid expansion on vulnerable populations (namely racial-ethnic minorities and those with low-income status) undergoing continuous-flow LVAD implantation after the enactment of the 2014 Affordable Care Act (ACA). METHODS: Data from the 2012 to Q3 2015 State Inpatient Database were used t…

Why some disabled adults in Medicaid face large out-of-pocket expenses.

Using national survey data, we found that the Medicaid program effectively limits out-of-pocket health care spending for the majority of Medicaid enrollees who are disabled but are not in institutions, such as nursing homes. However, we also found that for 10 percent of these enrollees, annual out-of-pocket spending is $1,200 or higher--a significant sum for people who are low-income. Other research has shown that such cost barriers can deprive Medicaid enrollees of cost-effective care in favor of cost-ineffective care or no care at all. As state policy makers contemplate benefit reductions th…
