Implementing an Electronic Medical Record-Based Reminder for Cardiovascular Risk Screening in Rheumatoid Arthritis.

OBJECTIVE: Although cardiovascular disease (CVD) is the leading cause of death among individuals with rheumatoid arthritis (RA), CVD risks are not being assessed frequently and systematically in RA. We implemented an electronic medical record (EMR)-based reminder in a tertiary care center and assessed the effects of this intervention on CVD risk screening by rheumatologists and primary care providers. METHODS: The EMR reminder was implemented in December 2013 and included the most recent value and target ranges for body mass index, blood pressure (BP), and lipid profiles. It was displayed for

Development and validation of an electronic medical record-based disease activity index for Behçet's disease.

OBJECTIVES: The disease activity of Behçet's disease is inadequately defined, and there is no consensus on how it should be measured. The aim of this study was to verify the usefulness of a simplified electronic medical record (EMR)-based activity index (EMRAI) for Behçet's disease. METHODS: A total of 73 Korean patients with Behçet's disease participated in this study. Two dermatologists interviewed each participant independently using two activity scoring systems: the EMRAI and the Behçet's Disease Current Activity Form (BDCAF). Overall agreement between raters, correlation between activity

Capture of osteoporosis and fracture information in an electronic medical record database from primary care.

In a large database of EMR records, we explore: 1) completeness in capture of bone mineral density (BMD) T-scores required for diagnosis of osteoporosis; 2) concordance of BMD exam information with other osteoporosis information; and 3) evidence of osteoporosis screening among fracture patients. To explore completeness of exam capture, BMD exams in the EMR were related to a provincial billing database. To explore concordance of information and screening rates, 7500 EMR records were reviewed for osteoporosis and fracture details. Results show that 98% of exams billed to the province for EMR pat

An electronic medical record-linked biorepository to identify novel biomarkers for atherosclerotic cardiovascular disease.

BACKGROUND: Atherosclerotic vascular disease (AVD), a leading cause of morbidity and mortality, is increasing in prevalence in the developing world. We describe an approach to establish a biorepository linked to medical records with the eventual goal of facilitating discovery of biomarkers for AVD. METHODS: The Vascular Disease Biorepository at Mayo Clinic was established to archive DNA, plasma, and serum from patients with suspected AVD. AVD phenotypes, relevant risk factors and comorbid conditions were ascertained by electronic medical record (EMR)-based electronic algorithms that included …

Symptom burden and splenomegaly in patients with myelofibrosis in the United States: a retrospective medical record review.

Myelofibrosis (MF) is a clonal hematopoietic malignancy characterized by constitutional and localized symptoms, progressive splenomegaly, bone marrow fibrosis, and cytopenias. Although MF is well studied, few studies exist regarding its symptomatic burden in routine clinical practice. This study aimed to characterize symptoms and other clinical features of MF among patients in the United States. We conducted a retrospective medical record review of adult patients with an MF diagnosis between 1 January 2005 and 31 March 2010, stratified by the presence of palpable splenomegaly. Eligible patient

Medical record validation of maternal recall of pregnancy and birth events from a twin cohort.

This study aims to assess the validity of maternal recall for several perinatal variables 8-10 years after pregnancy in a twin sample. Retrospective information was collected 8-10 years after the delivery event in a cohort of mothers from the University of Southern California Twin Study (N = 611) and compared with medical records for validity analysis. Recall of most variables showed substantial to perfect agreement (κ = 0.60-1.00), with notable exceptions for specific medical problems during pregnancy (κ ≤ 0.40) and substance use when mothers provided continuous data (e.g., number of cigarett…

Use of digital patient photographs and electronic medical record data as diagnostic tools in Japan.

An electronic medical record (EMR) system was introduced to the University of Miyazaki Hospital, in Japan, in 2006. This hospital is the only one in Japan to store digital photographs of patients within EMRs. In this paper, we report on the utility of these digital photographs for disease diagnosis. Digital photographs of patients were taken at the time of hospitalization, and have been used for patient identification by medical staff. More than 20,000 digital photographs have been saved, along with examination data and medical history classified by disease, since the introduction of EMR. In t

Obstacles encountered in the use of the medical record and cancer registry abstract in breast cancer research.

A historical-prospective study is being conducted to determine whether there is a relationship between obesity and breast cancer recurrence and survival in African-American women. The primary data sources for the study are the medical record and cancer registry abstract. It has been found that data items such as occupation, menopausal status, height, weight, estrogen receptor and progesterone receptor values, stage, tumor size, number of positive nodes, family history of cancer, and tumor status are not always documented. Suggestions for improving documentation of the primary data sources are

Agreement of self-reported ovarian number following gynecologic surgery with medical record reports.

In case-control studies of breast cancer, cardiovascular disease, and osteoporosis, a woman's ovarian function may be considered as a main exposure, confounding factor, or effect modifier. To evaluate the agreement between self-reported ovarian number following ovarian surgery for nonmalignant disease and medical record reports, we analyzed data from a population-based case-control study, the Cancer and Steroid Hormone Study. Reports on ovarian number after the most recent surgery for a who had been diagnosed with breast cancer and 496 control subjects. We then calculated agreement rates by co

Antibiotic use review in ambulatory care using computer-assisted medical record audit.

The use of a computer-assisted medical record audit (CAMRA) for reviewing antibiotic use in ambulatory patients was evaluated. A random sample of 40 medical records documenting treatment of streptococcal pharyngitis, otitis media, or acute, uncomplicated urinary tract infections at two family practice clinics was used to evaluate the accuracy and efficiency of computerized prescription screening and CAMRA relative to medical record audit (MRA) alone. Accuracy was the ability to correctly classify antibiotic therapy as appropriate or inappropriate. The initial computerized prescription screenin…

Electronic Medical Record Challenges: Discrepancies Between Clinical Scoring Tools in Pediatric Appendicitis.

BACKGROUND: Electronic medical record (EMR) transitions can impact patient care. With our EMR transition, the pediatric appendicitis score (PAS) was replaced by the Pediatric Appendicitis Risk Calculator (pARC). This study examines the discordance between PAS and pARC to identify discrepancies. METHODS: A retrospective review was conducted of patients (ages 3-17 y) who underwent appendectomy at a children's hospital (5/2021-3/2024). PAS and pARC scores were retrospectively calculated and assigned risk categories. Descriptive statistics, percent discordance, and the McNemar-Bowker test were ut

Advancing Family Medicine Through Dyadic Electronic Medical Record Linkage in Dementia Care.

Family physicians are well-positioned to support both persons living with dementia (PLWD) and their caregivers. However, current electronic medical record (EMR) systems do not capture the relational and interdependent nature of dementia care. Dyadic data, linking the records of persons living with dementia (PLWD) and their caregivers, could support more integrated, proactive, and personalized care. While there is growing interest in leveraging EMRs for this purpose, few systems enable routine dyadic linkage, limiting clinical utility and research capacity. This report describes the current stat

Factors associated with inpatient medical record data quality in hospital settings: a systematic review.

BACKGROUND: Inpatient medical record data are an important source of information for clinical care, hospital management, clinical coding, and reimbursement. Poor-quality record data may lead to incomplete documentation, coding errors, inaccurate performance assessment, and distorted reimbursement. In this review, data quality mainly refers to the accuracy, completeness, consistency, timeliness, and validity of inpatient medical record data, including front-page or home-page information and diagnosis/procedure coding data. Evidence on factors associated with these dimensions is scattered across
