Identification of Potential Type II Diabetes in a Large-Scale Chinese Population Using a Systematic Machine Learning Framework.

BACKGROUND: An estimated 425 million people globally have diabetes, accounting for 12% of the world's health expenditures, and the number continues to grow, placing a huge burden on the healthcare system, especially in those remote, underserved areas. METHODS: A total of 584,168 adult subjects who have participated in the national physical examination were enrolled in this study. The risk factors for type II diabetes mellitus (T2DM) were identified by p values and odds ratio, using logistic regression (LR) based on variables of physical measurement and a questionnaire. Combined with the risk

Detection of prediabetes and undiagnosed type 2 diabetes in underserved communities using community pharmacy point-of-care testing: A prospective feasibility study.

AIMS: Early detection of type 2 diabetes (T2D) and prediabetes is critical to reduce disease burden, yet disadvantaged populations often remain undiagnosed. We assessed a pharmacy-based pathway using verified point-of-care HbA1c testing to identify prediabetes and enable engagement with a digital prevention programme and established the detection rates for undiagnosed T2D. METHODS: We conducted a prospective feasibility study across 16 community pharmacies in NHS Tayside, Scotland (Feb 2024-Jan 2025). Adults at high risk of T2D, identified using the Diabetes UK 'Know Your Risk' tool, were off

Assessment of renal function in routine care of people living with HIV on ART in a resource-limited setting in urban Zambia.

INTRODUCTION: Data on renal impairment in sub-Saharan Africa (SSA) remains scarce, determination of renal function is not part of routine assessments. We evaluated renal function and blood pressure in a cohort of people living with HIV (PLWH) on antiretroviral treatment (ART) in the Renal Care Zambia project (ReCaZa). METHODS: Using routine data from an HIV outpatient clinic from 2011-2013, we retrospectively estimated the glomerular filtration rate (eGFR, CKD-Epi formula) of PLWH on ART in Lusaka, Zambia. Data were included if adults had had at least one serum creatinine recorded and had bee

Obstetric fistula in low-resource countries: an under-valued and under-studied problem--systematic review of its incidence, prevalence, and association with stillbirth.

BACKGROUND: Obstetric fistula (OF) is a serious consequence of prolonged, obstructed labor in settings where emergency obstetric care is limited, but there are few reliable, population-based estimates of the rate of OF. Stillbirth (SB) is another serious consequence of prolonged, obstructed labor, yet the frequency of SB in women with OF is poorly described. Here, we review these data. METHODS: We searched electronic databases and grey literature for articles on OF in low-resource countries published between January 1, 1995, and November 16, 2014, and selected for inclusion 19 articles with o

Simple and inexpensive point-of-care tests improve diagnosis of vaginal infections in resource constrained settings.

OBJECTIVE: Bacterial vaginosis (BV) and Trichomonas vaginalis infection (TV) have been associated with adverse birth outcomes and increased risk for HIV. We compare the performance of simple inexpensive point-of-care (POC) tests to laboratory diagnosis and syndromic management of BV and TV in poor settings. METHODS: Between November 2005 and March 2006, 898 sexually active women attending two reproductive health clinics in Mysore, India were recruited into a cohort study investigating the relationship between vaginal flora and HSV-2 infection. Participants were interviewed and screened for re

A system for the delivery of remote obstetrical care by telepresence.

This paper describes the development of a system to provide obstetrical care to remote and/or medically underserved areas. The ROC project is an attempt to develop new telemedicine hardware and software which will enable the provision of high quality obstetric care to individuals who are normally denied access to this care by reasons of distance, handicap, economics, or other causes. As broadband data communications links and increasingly powerful and affordable computing resources become widely available, the barriers of distance and economics will no longer prevent the provision of this care…

Economic Benefit of Hand Surgical Efforts in Low- and Middle-Income Countries: A Cost-Benefit Analysis.

BACKGROUND: Hand surgeons can alleviate the burden associated with various congenital anomalies, burn sequelae, and trauma that debilitate individuals in low- and middle-income countries. Because few surgeons in these areas have the necessary resources to perform complex hand surgery, surgical trips provide essential surgical care. The authors aimed to determine the economic benefit of hand surgical trips to low- and middle-income countries to comprehensively determine the economic implications of hand surgery trips in low-resource settings. METHODS: The authors collected data from two major

The primary care physician workforce in Massachusetts: implications for the workforce in rural, small town America.

CONTEXT: Small towns across the United States struggle to maintain an adequate primary care workforce. PURPOSE: To examine factors contributing to physician satisfaction and retention in largely rural areas in Massachusetts, a state with rural pockets and small towns. METHODS: A survey mailed in 2004-2005 to primary care physicians, practicing in areas designated by the state as rural, queried respondents about personal and practice characteristics as well as workforce concerns. Predictors of satisfaction and likelihood of remaining in current or rural practice somewhere were assessed. FIND

Are there need-based geographical differences between international medical graduates and U.S. medical graduates in rural U.S. counties?

The objectives of this study are to compare the rural location of international medical graduates (IMGs) and U.S. medical graduates (USMGs) by specialty (primary care vs. specialty care) according to geographical measures of need. This study utilized a cross-sectional survey using the 1997 American Medical Association Physician Masterfile for all active post-resident allopathic physicians and the Area Resource File (ARF) (Bureau of Health Professions, 1996) for all active post-resident osteopathic physicians in 1995 in the rural U.S. physician work force (N = 69,065).
