BACKGROUND: In robotic right hemicolectomy for cancer, appropriate lymphadenectomy is essential. Visualization of draining lymph nodes and blood flow with near-infrared (NIR) fluorescence DaVinci® imaging system is a recent development. We present the technique of robotic right colectomy with complete mesocolic excision (CME) and D3 lymphadenectomy using Indocyanine Green (ICG) endoscopic submucosal injection to intraoperatively identify tumour lymphatic basin.
METHODS: The day before surgery, in patients scheduled for robotic right colectomy an endoscopic submucosal injection of 3 mg of ICG

AIM: The aim of this study was to describe all the possible approaches for laparoscopic splenic flexure mobilization (SFM), each suitable for specific situations, and create an illustrated system to show SFM approaches in an easy and practical way to make it easy to learn and teach.
METHODS: Two different phases. First part: Cadaver-based study of the colonic splenic flexure anatomy. In order to demonstrate the different approaches, a balloon was placed through the colonic hepatic flexure in the lesser sac without sectioning any of the fixing ligaments of the splenic flexure. Second part: A r

We report herein an unusual case of primary mesosigmoidal hydatid cyst that presented as acute left colonic obstruction mimicking sigmoid colon cancer. A 61-year-old man with a 3-day history of abdominal pain, distention, obstipation, vomiting, and fever was admitted to the emergency department of our hospital. Surgery was performed under a presumptive diagnosis of acute left colonic obstruction. The intraoperative findings were highly suggestive of sigmoid colonic carcinoma and Hartmann's procedure was performed. Histopathological examination of the pathological specimen revealed an isolated

A case report of sclerosing mesocolitis is presented. This is an unusual and poorly understood entity. A 49-year-old woman presented with episodic abdominal pain and a palpable abdominal mass. A fibrotic mass causing extensive compression of her cecum and transverse colon was found at laparotomy. The pathologic findings and treatment of sclerosing mesocolitis are discussed.

INTRODUCTION: Ultrasound examinations during pregnancy are routine procedures used to detect fetal congenital malformations. Ultrasound monitoring of sigmoid colon mesenterial development could be useful for early detection of subjects at risk of sigmoid colon volvulus.
OBJECTIVE: The aim of our paper was to assess the sigmoid colon length, and sigmoid colon mesentery width and height in the late fetal period, and, using the results, to estimate the surface area of the mesocolon (in mm2) in living fetuses. Moreover, we attempted to repeat some of these measurements in living fetuses using ult…

BACKGROUND: Bursectomy is regarded as a standard surgical procedure during gastrectomy for serosa-positive gastric cancer in Japanese gastri … th complete bursectomy using an outside bursa omentalis approach.
METHODS: Firstly, the transverse mesocolon and distal gastric membrane were separated from right to left, and the right gastroepiploica vessels were ligated at root with No. 6 lymph nodes (LNs) dissection followed by the pancreas membrane dissection from pancreas head to pancreas tail. Secon …

Between January 1, 1970 and March 1, 1985 530 patients underwent graft replacement of the abdominal aorta for infrarenal aortic aneurysms. 28 (5.3%) patients had inflammatory aneurysmal disease, in 3 cases (10.7%) the aneurysm was ruptured. Operation mortality was 17.3%. Characteristic appearance was an unusually thick aortic wall with diffuse, shiny white fibrotic reaction in the retroperitoneum. In most cases the duodenum, inferior vena cava, left renal vein, transverse mesocolon or the ureter were involved into the inflammatory mass. Thoracic aortic involvement was not observed. Contrast CT…

Following gastrojejunostomy in Roux-en-Y gastric bypass (RYGB), the mesentery is susceptible to Petersen's hernia due to a mesenteric gap created between the alimentary limb, transverse mesocolon, and retroperitoneum. Although the Petersen's hernia is a rare complication, it has increased due to the widespread adoption of gastric bypass surgeries for weight management and gastric cancer resections. Computed tomography (CT) is commonly used to evaluate suspected cases. We present a case of a 36-year-old female who developed Petersen's hernia 3 weeks post RYGB. This report highlights key radiolo…

BACKGROUND: Panniculitis mesenteritis is a rare, benign, and chronic fibrosing inflammation disease with unknown aetiology that affects the mesentery of small bowel and colon.
AIM: To report a new case of this entity and to highlight its diagnostic difficulties especially when it affects the mesocolon.
CASE REPORT: A 37 years-old women presented with chronic abdominal pain; physical examination found mass of left ilaca fossa. Closcopy and abdominal CT scan showed a thickness of bowel wall with negative biopsies. The patient underwent a colectomy. Histologic examination concluded to the diagn…

BACKGROUND: Complete mesocolic excision (CME) has been proposed as a refinement of oncological surgery for right-sided colon cancer, aiming to improve specimen quality, lymph node yield and oncological outcomes. However, its routine adoption remains controversial due to uncertainty regarding oncological benefit and concerns relating to technical complexity, training and service delivery. This position statement from the Association of Coloproctology of Great Britain and Ireland (ACPGBI) examines the contemporary evidence surrounding CME and its role within modern management of right-sided colo

BACKGROUND: Complete mesocolic excision (CME) is a surgical approach for right-sided colon cancer, involving the resection of the primary tumour along with an intact mesocolon, central vascular ligation, and exposure of the superior mesenteric vein. It has been postulated to improve oncologic outcomes such as disease-free survival and reduce local recurrence compared to standard right colectomy. However, the clinical benefits are still debated.
OBJECTIVE: This systematic review and meta-analysis, sponsored by the European Association for Endoscopic Surgery, aims to compare the oncologic outco…

AIM: Laparoscopic complete mesocolic excision (CME) with D3 lymphadenectomy for right colon cancer is gaining acceptance. However, this proc … his-the ligamentum teres and SMA/SMV are both midline structures. Giving traction on the transverse mesocolon just below the ligamentum makes the pulsatile SMA visible irrespective of the patient's body mass index. Giving traction at the ileocaecal junction mesentery makes the ileocolic pedicle prominent. These two landmarks for identification of the vascul …

BACKGROUND: Complete mesocolic excision (CME) for right colonic cancer is a more complex operation than standard right hemicolectomy but evidence to support its routine use is still limited. This prospective multicentre study evaluated the effect of CME on long-term survival in colorectal cancer centres in Germany (RESECTAT trial). The primary hypothesis was that 5-year disease-free survival would be higher after CME than non-CME surgery. A secondary hypothesis was that there would be improved survival of patients with a mesenteric area greater than 15 000 mm2.
METHODS: Centres were asked to
