In the article is described the congenital pathology of the hand, characterized by the absence of one or several fingers with the corresponding metacarpal bones and bones of the wrist. There is given the clinical picture of 104 patients with the hand radius aplasia. There are singled out main forms of pathology, presented the data of the additional investigation methods (angiography, rheovasography, histologic structure of the affected hand tissues). There are presented the main principles of the hand radius aplasia treatment.

Based on two independent personal cases and a pair of sibs from the literature, we delineate a new category of bone dysplasia with cup-shaped large metaphyses, for which the name metaphyseal acroscyphodysplasia is suggested. The main clinical features are severe growth retardation, micromelia predominating in the lower limbs, knee flexion, and severe brachydactyly. The radiological aspect of the knees is very specific: the lower femoral and upper tibial epiphyses embed themselves in their metaphyses, which are severely cup-shaped. Premature central epiphyso-metaphyseal fusion and gross deforma

Hand radiographs of 21 Danish patients suffering from achondroplastic chondrodystrophia were obtained to study the metacarpophalangeal relations. Mean pattern profiles based on z-scores were made for males, females, children, and adults. Metacarpal index and relative slenderness for metacarpals and proximal phalanges were calculated. The second to fourth metacarpals and third and fourth proximal phalanges were disproportionately short, and the distal phalanges were disproportionately long. The relation between hand size and height seemed to be the same for the achondroplasts as for normal pers

Radiographs of the hand in a group of 40 patients with the Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome were studied. Most of the patients had the following abnormalities: brachymesophalangy of digits 2-5 (22/39 patients), small distal phalanx of digit 1 (22/39 patients), long proximal phalanx of digits 3-4 (19/39 patients), and long metacarpals of digits 1-4 (20/39 patients). In addition, three patients had distinct radial dysplasia and abnormalities of the carpals.

Stable fractures of the metacarpal and proximal phalanges can be treated satisfactorily by closed methods. The problem in those unstable fractures is whether to treat with open or closed reduction. Stiffness of the proximal interphalangeal joint (PIP), malunion, rotation deformities, shortening, and nonunion are known complications of fractures of the proximal phalanx. Fractures at the base of the proximal phalanx. Fractures at the base of the proximal phalanx have low incidence of complications if they are treated closed. Fractures at the shaft carry a much larger risk of complications with e

Twenty-two patients with 26 fractured metacarpals were treated by internal fixation using A. O. mini-fragment screws and plates. This form of internal fixation was reserved for metacarpal fractures which were multiple, unstable, displaced or rotated. Nine patients had open fractures, two had divided extensor tendons and four had multiple fractures. Fractures consistently united without infection or deformity. The mean duration of postoperative immobilization was seven days and mean time off work was six weeks. All but one patient did manual work. No patients had persistent symptoms which cause

Giant cell reparative granuloma (GCRG) usually occurs in the jaw, but a histologically identical lesion is occasionally seen in the small bones of the hands and feet. A 24-year-old man had GCRG involving the second metacarpal and the capitate. The involvement of a carpal bone and the presence of multiple lesions are unusual for this entity.

Two new cases are added to the literature on four previously reported cases of simultaneous dislocation of the five carpometacarpal joints. The priorities of management of other more extensive injuries often delays definitive treatment and in the two cases reported here open reduction and internal fixation was necessary to restore the functional structure of the hand. Restoration of normal grip and pinch strength was obtained in both patients with stable, pain-free carpometacarpal joints.

Nine cases of aneurysmal bone cyst arising in juxtaepiphyseal locations with gross invasion of the adjacent growth plate are reported. In five of these patients an abnormality of growth, due to premature fusion of the affected growth plate, ultimately developed. Treatment of these lesions should attempt to avoid this complication, which appears to be more common than has been appreciated in the past. These nine cases represent 23% of 39 cases of aneurysmal bone cyst occurring in a long bone adjacent to an open epiphyseal plate. This series was extrapolated from a total of 198 cases of aneurysm

A case of a "juxta-cortical" aneurysmal bone cyst on the third metacarpal is reported with more than three years of follow-up. No recurrence was observed. The nosology and the treatment are discussed. The literature is reviewed.

A 6-week-old white-tailed deer fawn (Odocoileus virginianus), found in Vermont, was presented with carpal contraction, 90 degrees medial deviation of the rear legs from the hock distally, and an abnormal coat color. Radiographically there was lateral deviation of both medial metacarpal bones and a Grade IV medial patellar luxation.

A girl with a "new" variant of mesomelic dysplasia is reported. The disorder is characterized by mesomelic brachymely, especially of the arms, a short ulna, relatively long fibula, brachymetacarpy, minor symmetrical changes at the hands, developmental arrest of the spine, contractures, and micrognathia. The clinical and radiological differential diagnostic features are discussed.

Altogether 15 partially independent measurements of bone mass in 100 women with clinical and roentgenological signs of osteoporosis were correlated to the alkaline phosphatase activities of the same individuals. There was a slight but significant negative correlation indicating an increasing alkaline phosphatase activity with decreasing bone mass. This correlation was not caused by interaction of age. There was no correlation or morphological signs of osteomalacia. The changes could not be explained by fractures. It is suggested that a slight increase in the alkaline phosphatase activity in wo

Osteogenesis imperfecta is often regarded as a form of osteoporosis. In many cases, particularly those in whom the first fracture occurs outside the neonatal period, bones that have not been fractured may appear radiologically normal. In a group of 24 adults with osteogenesis imperfecta the thickness of the metacarpal cortex was normal but their bones were often slender. Osteoporosis is probably not an inevitable feature of such cases, and some of the radiological abnormalities reported may be the results of previous fractures and their treatment.

Two new pedigrees of polydactyly associated with brachymetapody are discribed. In one the two defects occur in different members of the family, while in the other both occur in the same individuals. Both anomalies appear to be inherited as dominants, the polydactyly showing incomplete manifestation.

An investigation of a family with congenital X-linked cataracts, microcorneas, supernumerary incisors, anteverted pinnae and shortened metacarpals is presented. The carriers had posterior sutural opacities and cone-shaped teeth. There have been four previously reported families with X-linked cataracts, one of which also presented microcorneas. Four other families have been reported with X-linked cataracts, microphthalmia and somatic and mental anomalies. Linkage studies could be used to demonstrate whether these diseases result from allelic or nonallelic genes but were uniformative in the pres
