Inhalation technique practical skills and knowledge among physicians and nurses in two pediatric emergency settings.

Introduction: Correct technique with a pressurized metered-dose inhaler (pMDI) equipped with a valved holding chamber (VHC) or spacer provides an important advantage for adequate control of asthma and virus-induced wheezing in young children. The aim of this study was to assess the ability and knowledge of physicians and nurses to use a pMDI with a masked VHC in two pediatric emergency units. Methods: Study design: Two-center observational study. Inhaler use technique was assessed in 50 physicians and 50 nurses using a child mannequin and a validated videotaped nine-step scoring method. The par

A Novel Device to Improve the Tolerability of Intranasal Corticosteroid Sprays for Allergic Rhinitis and Postoperative Chronic Rhinosinusitis: A Crossover Trial.

BACKGROUND: Allergic rhinitis (AR) and chronic rhinosinusitis (CRS) are commonly treated with intranasal corticosteroid sprays. Despite their efficacy, unpleasant sensations associated with their application, including post-nasal drip and nasal irritation, are often cited as reasons for poor compliance. Optimising the intranasal application experience is key to improving long-term adherence. We developed a nasal adaptor for metered-dose inhalers (MDIs) to reduce the unpleasantness of nasal administration, potentially improving patient compliance. METHODS: Two crossover trials were conducted i

Severe childhood asthma in low and middle-income countries.

Although severe asthma in low- and middle-income countries (LMICs) is relatively uncommon in children and adolescents (2.1 % and 4.3 %, respectively), it results in significant morbidity, occasional fatality, and great challenges of management, compared to high-income countries. Additionally, nearly 6 % of infants in LMICs experience nighttime symptoms on a weekly basis. Socioeconomic inequities and exposure to environmental risk factors contributed to variability in prevalence, with children from disadvantaged backgrounds being at a higher risk of developing the condition. Numerous preventabl

Achieving good adherence to inhaled corticosteroids after weighing canisters of asthmatic children.

Background: The metered-dose inhalers (MDIs) currently available for inhaled corticosteroid delivery do not offer an integrated dose counter; therefore, it is difficult to evaluate adherence of patients. The present authors developed a linear regression equation using canister weight to calculate the number of doses actuated from the MDIs. This study aimed to assess medical adherence after the integration of regular weighing of the canisters into the routine service.  Methods: A cohort study was carried out between May 2013 and April 2014. Children aged less than 8 years with a diagnosis of as

A systematic review of comparative studies of tiotropium Respimat® and tiotropium HandiHaler® in patients with chronic obstructive pulmonary disease: does inhaler choice matter?

BACKGROUND: In many countries worldwide, the long-acting anticholinergic drug tiotropium is available as a dry powder formulation delivered by means of the HandiHaler® inhalation device and as an aqueous solution delivered via the Respimat® Soft Mist™ Inhaler. Tiotropium HandiHaler® is a single-dose, dry powder, breath-actuated inhaler that provides delivered doses and lung deposition of tiotropium that are, over a wide range, not influenced by the severity of chronic obstructive pulmonary disease (COPD). Tiotropium Respimat® is a propellant-free, multi-dose inhaler that delivers a metered dos

[Strategic formulation study on dry powder inhalation system based on modulated molecular properties and controlled pharmacokinetics].

Cyclosporine A (CsA) has been widely used as an immunosuppressive agent, and recent outcomes from clinical studies are indicative of the potent therapeutic potential of CsA for chronic asthma and airway inflammation. The clinical use of CsA for airway inflammatory diseases is partly limited because of low oral bioavailability and severe systemic side effects. A number of CsA dosage forms have been proposed to overcome these drawbacks, for example, nebulizer formulation and metered-dose inhaler formulation for inhalation therapy, whereas these liquid formulations sometimes contain organic solve

Inhaled drug delivery: a practical guide to prescribing inhaler devices.

Direct delivery of medication to the target organ results in a high ratio of local to systemic bioavailability and has made aerosol delivery of respiratory medication the route of choice for the treatment of obstructive lung diseases. The most commonly prescribed device is the pressurized metered dose inhaler (pMDI); its major drawback is the requirement that inspiration and actuation of the device be well coordinated. Other requirements for effective drug delivery include an optimal inspiratory flow, a full inspiration from functional residual capacity and a breath hold of at least 6 s. Avail

Dose-effect relationship of the beta-agonists fenoterol and salbutamol in patients with asthma.

QUESTION: What is the relative per microgram potency and side effect profile of the beta-agonists salbutamol and fenoterol? METHOD: The relative bronchodilator (delta FEV1, V25, V50) potency and side effect profile (delta tremor, heart rate, breathlessness, BP) of nebulized salbutamol and fenoterol were evaluated by means of a randomized, double-blind, crossover, cumulative (50 to 2,500 micrograms) dose-response study. Both beta-agonists were administered to 12 patients with stable asthma over age 18 years with baseline FEV1 between 35 to 70 percent predicted. RESULTS: (1) Salbutamol and fen

Fluticasone propionate and bronchial hyperresponsiveness in childhood asthma.

Bronchial asthma is now agreed as being a chronic inflammatory disease of the airways. Inhaled steroids are widely accepted as a preventive medication in asthmatic patients of all ages and severity. However, the optimal use of inhaled steroids and the important issue of safety and efficacy still remain of concern, particularly in children. Recently, fluticasone propionate (FP) has been developed for use as an inhaled preparation for the treatment of asthma. Because of its high topical potency and increased lipophilicity, it is claimed that FP has an improved risk/benefit compared with other in

Understanding Pulmonary and Systemic Pharmacokinetics of Inhaled Fluticasone Propionate in Healthy Subjects Using Population PK Analysis.

BACKGROUND AND OBJECTIVES: One of the main hurdles in pulmonary drug delivery is the inability to directly quantify drug concentrations at the site of action within the lungs, which are critical for therapeutic efficacy. This research attempted to address this limitation by applying population pharmacokinetic modeling to characterize the pharmacokinetics of fluticasone propionate (FP), integrating multiple pulmonary absorption pathways and identifying key covariates that influence its pharmacokinetics. METHODS: Plasma concentration-time data from 96 healthy volunteers participating in four ph

Albuterol-budesonide rescue inhaler for asthma: Patterns of use and safety in the MANDALA trial.

BACKGROUND: The MANDALA study (NCT03769090) in moderate-to-severe asthma served as the basis of the Food and Drug Administration's 2023 approval of albuterol-budesonide 180/160 µg pressurized metered-dose inhaler for the as-needed treatment or prevention of bronchoconstriction and to reduce exacerbation risk in patients with asthma aged 18 years or older. Clinicians would benefit from an understanding of the patterns of use of albuterol-budesonide vs albuterol and overall inhaled corticosteroid (ICS) exposure when ICS-containing rescue therapies are used alongside ICS-based maintenance therapi

[Drugs for asthmatic crisis].

The drugs used in asthmatic attacks must act on the two major mechanisms of the disease: bronchial obstruction and inflammation of the airways. Two main classes of drugs are available to reach these targets: bronchodilators, headed by beta 2-stimulants, and anti-inflammatory drugs of the corticosteroid family. Bronchodilatation obtained with beta 2-stimulants is the first and most effective treatment of the attack. These drugs are usually administered by inhalation: metered-dose aerosols with or without inhalation chambers, or nebulization for severe attacks. Very high doses can be used withou

Tachyphylaxis to systemic but not to airway responses during prolonged therapy with high dose inhaled salbutamol in asthmatics.

High doses of inhaled salbutamol produce substantial improvements in airway response in patients with asthma, and are associated with dose-dependent systemic beta-adrenoceptor responses. The purpose of the present study was to investigate whether tachyphylaxis occurs during prolonged treatment with high dose inhaled salbutamol. Twelve asthmatic patients (FEV1, 81 +/- 4% predicted), requiring only occasional inhaled beta-agonists as their sole therapy, were given a 14-day treatment with high dose inhaled salbutamol (HDS), 4,000 micrograms daily, low dose inhaled salbutamol (LDS), 800 micrograms

Comparison of the relative propensities of isoamyl nitrite and sodium nitrite to ameliorate acute cyanide poisoning in mice and a novel antidotal effect arising from anesthetics.

Isoamyl nitrite has previously been considered acceptable as an inhaled cyanide antidote; therefore, the antidotal utility of this organic nitrite compared with sodium nitrite was investigated. To facilitate a quantitative comparison, doses of both sodium nitrite and isoamyl nitrite were given intraperitoneally in equimolar amounts to sublethally cyanide-challenged mice. Righting recovery from the knockdown state was clearly compromised in the isoamyl nitrite-treated animals, the effect being attributable to the toxicity of the isoamyl alchol produced during hydrolysis of the isoamyl nitrite t

A subchronic toxicity study of two inhaled aerosolized atropine sulfate formulations in rats and dogs.

The potential subchronic (21 days) toxicity of inhaled metered aerosol formulations (solution and suspension) of atropine sulfate was investigated in rats and dogs. The doses administered to rats were 0.78 and 2.5 mg/kg/day (solution) or 1.4 and 3.2 mg/kg/day (suspension). In the dog, the daily doses achieved were 0.5 and 1.3 mg/kg/day, regardless of formulation. In both species, sham control animals inhaled air only and vehicle control animals inhaled either placebo solution or placebo suspension. There was no mortality or other evidence of a toxic effect of atropine sulfate. The expected myd

[Sudden unexpected death and near death among children with asthma].

During four winter months 1989-90, two children died suddenly from asthma, and one suffered a life threatening attack. A 20 months old boy with moderately severe asthma, just recovered from an adenovirus infection, was symptom free at a routine consultation less than 12 hours before death. He died suddenly after a spell of coughing. A six year old girl with severe but well controlled asthma received unknown quantities of inhaled nebulized salbutamol during a three day exacerbation. She died at home due to respiratory arrest. A 16 year old boy who attended the clinic sporadically had a severe u

Childhood asthma--office management.

Childhood asthma is a major problem in office practice. For an acute life threatening attack (which is indicated by presence of severe distress, pulsus paradoxus, oxygen saturation less than 93%, cyanosis, peak expiratory flow rate of less than 50% of predicted) child should be directly admitted in intensive care unit. If it is a non life threatening attack the child can be managed in the office. Initially, the child should be given b2 agonists by inhalation route with either metered dose inhaler (MDI), MDI with spacer or nebuliser.
