The diagnosis of lesions clinically suggestive of hairy leukoplakia was assessed by light and electron microscopic examination of cytologic smears. We found exfoliative cytology to be a simple and noninvasive technique that can confirm the clinical diagnosis of hairy leukoplakia. Our results suggest that electron microscopy may be more sensitive and reliable than light microscopy in confirming the clinical diagnosis of hairy leukoplakia.

To provide a better understanding of the morphologic changes that take place in the intestine and colon in acquired immunodeficiency syndrome (AIDS), electron microscopy was performed on intestinal or colonic biopsy specimens obtained from 6 patients with AIDS and from 2 patients with AIDS-related lymphadenopathy syndrome. For the first time we present electron-microscopic results of an actinomycosis of the lacrimal canaliculus. The interior of the actinomycotic conglomerate showed no evidence of a cellular defence reaction but in the loosly woven outer network of hyphae we observed a massive granulocytic reaction. Also, after phagocytosis the structure of the actinomycotic micro-organisms within the granulocytes is not significantly damaged. Within the tissue of the lacrimal canaliculus adjacent to the actinomycotic conglomerate we observed an increased number of plasmacells but no pathogenic organisms.

Selected examples of neoplasms of the central nervous system are reviewed in which the electron microscope has been instrumental in establishing the diagnosis. These include ependymoma, ependymoblastoma, epithelial cyst, paraganglioma in the cauda equina and cerebellar neuroblastoma.

The use of electron microscopy as a further method of diagnosis of disease in cultured skin fibroblasts and cultured amniotic fluid fibroblasts is presented. It was demonstrated that Tay-Sachs disease, Fabry's disease, and metachromatic leukodystrophy had distinctive abnormalities in both cultured skin fibroblasts and cultured amniotic fluid fibroblasts. It was shown that control of culturing conditions made it possible to distinguish normal cell lines from certain cell lines carrying known genetic diseases.

In the past few years many clinically significant new glomerular lesions have been reported from observations made on renal biopsy specimens by electron microscopy. The clinical applicability of electron microscopy is illustrated by examples of specific ultrastructural patterns of glomerular reaction making possible a more accurate diagnosis, prognosis, and prediction of the response to treatment.
