Evaluation of infection control practices during an AE.

The broadening global commitment of UK military personnel since the end of the Cold War has brought them into contact with a plethora of tropical diseases, some of which require isolation and air transfer to a UK medical facility. This presents a number of challenges to the safe repatriation of patients and attending medical and flight crews. During a recent repatriation of a patient with suspected Lassa fever from West Africa, the team's practices were evaluated and recommendations were made for improvements to future transfers by the deployable air isolator team. Valuable lessons were learne

Sibling and Parental Military Service and Suicidality Among Adolescents in the United States.

INTRODUCTION: This study aims to examine whether having a sibling and/or a parent on active duty in the military is associated with suicidality-think about death, better off dead, think about suicide, plan suicide, and attempt suicide-among 12- to 17-year-old adolescent girls and boys in the United States. MATERIALS AND METHODS: Descriptive and multivariable logistic regression analyses were conducted using pooled cross-sectional public-use data from the 2016-2019 National Survey of Drug Use and Health. Analyses were weighted and standard errors were adjusted for the complex sampling design.

Fertility Preservation Knowledge, Perceptions, and Expectations Among Military Health Care Students.

INTRODUCTION: The rising demand for assisted reproductive technology and the expansion of health care coverage by Congress underscores the need to equip future military clinicians with knowledge about reproductive care and fertility preservation. Medical and graduate nursing students at the Uniformed Services University of the Health Sciences (USUHS) are uniquely positioned to both personally engage with and professionally educate patients about these services. This study's objective was to evaluate health care students' perceptions of age-related fertility decline, knowledge of fertility pres

[Predictors of suicidality in German soldiers].

BACKGROUND: Increasing psychological distress due to out-of-area deployments is associated with a growing risk of suicidal behaviour in soldiers. MATERIALS AND METHODS: A total of 200 military psychiatric inpatients of German Armed Forces hospitals, including n=100 soldiers with and n=100 without (control group) acute suicidality prior to admission were compared concerning sociodemographic and clinical characteristics. RESULTS: Significant predictors of suicidal ideation were a family history of alcohol abuse and pathological scales of the Minnesota Multiphasic Personality Inventory (MMPI).

"Doing the right thing": Moral conflicts and ethical issues experienced by military nurses during wartime.

INTRODUCTION: The War on Terrorism, which included Operation Enduring Freedom (OEF) in Afghanistan from 2001 to 2014 and the concurrent Operation Iraqi Freedom (OIF) from 2003 to 2011, exposed military nurses to situations and challenges for which many reported feeling unprepared. Clinically, nurses faced multi-trauma injuries and devastating wounds suffered by military troops and civilians alike. Cultural issues and harsh living conditions added further complications to the care environment. The purpose of this study was to address the research question: How do military nurses identify, asses

Heat Adaptation in Military Personnel: Mitigating Risk, Maximizing Performance.

The study of heat adaptation in military personnel offers generalizable insights into a variety of sporting, recreational and occupational populations. Conversely, certain characteristics of military employment have few parallels in civilian life, such as the imperative to achieve mission objectives during deployed operations, the opportunity to undergo training and selection for elite units or the requirement to fulfill essential duties under prolonged thermal stress. In such settings, achieving peak individual performance can be critical to organizational success. Short-notice deployment to

Pediatric diseases and operational deployments.

Many nations now export military health as a proactive arm of the nation's contribution to the maintenance of international peace in trouble regions of the world; and all nations are called upon from time to time in emergency and disaster situations to help out in their regions of interest. Children and young teenagers constitute some 50% of war-stricken populations. This paper explores this increasingly important role of military medicine from the point of view of a practicing pediatrician and career doctor-soldier. Many international operational deployments undertaken in the last 5 years hav

Early-Onset Dementia in War Veterans: Brain Polypathology and Clinicopathologic Complexity.

The neuropathology associated with cognitive decline in military personnel exposed to traumatic brain injury (TBI) and chronic stress is incompletely understood. Few studies have examined clinicopathologic correlations between phosphorylated-tau neurofibrillary tangles, β-amyloid neuritic plaques, neuroinflammation, or white matter (WM) lesions, and neuropsychiatric disorders in veterans. We describe clinicopathologic findings in 4 military veterans with early-onset dementia (EOD) who had varying histories of blunt- and blast-TBI, cognitive decline, behavioral abnormalities, post-traumatic str

Trajectories of alcohol use in the UK military and associations with mental health.

INTRODUCTION: There are higher levels of alcohol misuse in the military compared to the general population. Yet there is a dearth of research in military populations on the longitudinal patterns of alcohol use. This study aims to identify group trajectories of alcohol consumption in the UK military and to identify associations with childhood adversity, deployment history and mental disorder. METHODS: Data on weekly alcohol consumption across an eight year period and three phases of a UK military cohort study (n=667) were examined using growth mixture modelling. RESULTS: Five alcohol trajecto

Evaluating a Tool for Assessment of Adjustment Disorder in the U.S. Military: The Adjustment Disorder-New Module 20 for Military (ADNM-20-MIL).

OBJECTIVES: Adjustment disorder (AjD) is a highly prevalent diagnosis in the U.S. military. Psychometric evaluation of the AjD assessment tool, the Adjustment Disorder New Module-Military (ADNM-20-MIL), improves the accuracy of AjD assessment for military service members. METHODS: This study investigated the internal reliability, convergent, and divergent validity of the ADNM-20-MIL as well as its factor structure. U.S. active duty service members (N = 149) with and without a recent AjD diagnosis completed the ADNM-20-MIL, Depression Anxiety Stress Scales 21 (DASS-21), Posttraumatic Stress Di

Deployment-related toxic exposures, mental health problems, and suicide outcomes among Gulf War era U.S. veterans.

Exposure to toxins, such as pesticides, smoke from burning oil fields, and nerve gas, during military operations have been linked to poor physical health among veterans. However, less is known about how these exposures may affect mental health outcomes, especially those relevant to military populations (e.g., posttraumatic stress disorder [PTSD], depression, alcohol use disorder, suicide). The goal of the present study was to examine how any toxic exposure and total toxic exposures were associated with probable current diagnoses of PTSD, major depressive disorder, and alcohol use disorder as w

Facilitators and barriers to health promotion interventions in the National Defence Forces of Ethiopia: a grounded theory study.

BACKGROUND: The Ethiopian military implements health promotion interventions to improve and maintain fitness, health, and military readiness. However, distinct organizational features of military forces can affect the health of their members. This study aimed to explore facilitators and barriers to implementing health promotion interventions within Ethiopian military settings and develop a conceptual model based on the findings. METHODS: A grounded theory approach was used in this qualitative study. We identified 26 participants among health experts engaged in health promotion and disease pre

Duckweed Proteins in Neuro-Nutrition: Mechanistic Insights and Translational Relevance for Military and Extreme Environments.

Modern military performance increasingly depends on cognitive resilience and neurological health, making nutrition a strategic determinant of combat readiness. This review integrates nutritional neuroscience and military medicine to explore duckweed as a next-generation neuro-nutritional resource. Duckweed species possess protein content (25-45% dry weight) and a complete essential almost amino acid (EAA) profile that may contribute to pathways involved in neurotransmitter synthesis, neuroplasticity, and stress adaptation. Beyond amino acids, duckweed delivers neuroprotective compounds such as

US Malaria Casualties and Chemoprophylaxis during the Vietnam War.

Malaria caused mass casualties among US military personnel during the Vietnam War, resulting in as many hospitalizations as combat wounds in 1965. Its military impact was magnified because nearly all the disease-related casualties came from front-line units, particularly the infantry, with rates exceeding 10 per 1,000 man-days of exposure. The imperfect chemoprophylaxis using chloroquine-primaquine tablets developed during the Korean War proved inadequate. Adding daily dapsone was more effective but resulted in rare cases of agranulocytosis and at least eight deaths. After Vietnam, malaria in

Tuberculosis as a force health protection threat to the United States military.

Tuberculosis (TB) is a communicable disease that poses a threat to force health protection to the U.S. military. The rate of TB disease in the military is low; however, there are unique challenges for its control in this setting. As a low-risk population, TB testing in the U.S. military can be scaled back from the universal testing approach used previously. Reactivation of latent TB infection (LTBI) present at accession into service is the most important factor leading to TB disease; therefore, its diagnosis and treatment among recruits should be given a high priority. Deployment and overseas

Stigma, American military personnel and mental health care: challenges from Iraq and Afghanistan.

BACKGROUND: Since 2001, more than 2.5 million United States military personnel have been deployed for combat. Over one million have served multiple deployments. Combat generally involved repeated exposure to highly traumatic events. Personnel were also victims of military sexual trauma (MST), a major risk factor for psychiatric illness. Most survivors do not seek or receive mental health care. Stigma is one of the main barriers to that care. AIMS: To explore the impact of stigma on personnel with psychiatric illness, and suggest some innovative ways to potentially reduce stigma and improve ca
