Factors Influencing Clinical Reasoning Among Clinical Nurses: An Integrative Review Using the Decision-Making Ecology Model.

BACKGROUND: Clinical reasoning (CR) is a complex, context-dependent process that is essential for safe nursing care. Existing reviews have primarily focused on nursing students rather than practicing clinical nurses. Rapid changes in patient acuity and the increasing integration of digital technologies into healthcare highlight the need for an updated synthesis that reflects contemporary clinical environments. AIMS: To identify and synthesize factors influencing CR among nurses in contemporary healthcare settings using the decision-making ecology (DME) model. DESIGN: Integrative review. MET

Innovative strategies for the elimination of viral hepatitis at a national level: A country case series.

Viral hepatitis is a leading cause of morbidity and mortality worldwide, but has long been neglected by national and international policymakers. Recent modelling studies suggest that investing in the global elimination of viral hepatitis is feasible and cost-effective. In 2016, all 194 member states of the World Health Organization endorsed the goal to eliminate viral hepatitis as a public health threat by 2030, but complex systemic and social realities hamper implementation efforts. This paper presents eight case studies from a diverse range of countries that have invested in responses to vir

[Health for refugees - the Bremen model].

The Bremen model recognizes that refugee health care has to go beyond merely checking for the prevalence of contagious diseases. Elementary health care offered in the reception centre and transitory facilities is based on voluntary acceptance by the refugees. At the same time, legal requirements for the medical reception of refugees are observed. In addition, doctors performing the initial medical examination are enabled to cover acute care on the spot. During the preliminary phase of immigration refugees are allowed to see a doctor in their facility repeatedly. After a certain time, they are

Research into practice: Collaboration for Leadership in Applied Health Research and Care (CLAHRC) for Nottinghamshire, Derbyshire, Lincolnshire (NDL).

BACKGROUND: To address the problem of translation from research-based evidence to routine healthcare practice, the Collaboration for Leadership in Applied Health Research and Care for Nottinghamshire, Derbyshire, and Lincolnshire (CLAHRC-NDL) was funded by the National Institute for Health Research as one of nine CLAHRCs across England. This paper outlines the underlying theory and its application that CLAHRC-NDL has adopted, as a case example that might be generalised to practice outside the CLAHRC, in comparison to alternative models of implementation. DISCUSSION: Conventional approaches to

Cross-sector working: speech and language therapists in education.

Current policies and practices concerning speech and language therapy provision in schools are reviewed and issues which remain to be addressed are considered A general move to cross-sector approaches has resulted in a move to indirect therapy through assistants, and the effectiveness of this requires to be established. Funding streams for therapy differ across the UK, and despite extensive research and policy development remain somewhat unclear and vulnerable. The implications for policies on 'joined up thinking" are considered.

Propagation, not aggregation: linking individual psychological flexibility to organizational functioning.

Individual psychological flexibility is reliably associated with worker health, and interventions that raise it improve how individuals fare; yet such interventions do not reliably change the organizations those individuals belong to. This paper takes that disparity as its starting point and argues that the missing link is propagation rather than production: organizational flexibility is not the aggregate of flexible individuals but a pattern of coordinated, values-consistent action that is retained and spread across an organization, or absorbed and extinguished, depending on the conditions it

Generations and Generational Differences: Debunking Myths in Organizational Science and Practice and Paving New Paths Forward.

Talk about generations is everywhere and particularly so in organizational science and practice. Recognizing and exploring the ubiquity of generations is important, especially because evidence for their existence is, at best, scant. In this article, we aim to achieve two goals that are targeted at answering the broad question: "What accounts for the ubiquity of generations despite a lack of evidence for their existence and impact?" First, we explore and "bust" ten common myths about the science and practice of generations and generational differences. Second, with these debunked myths as a bac

Shaping science and society through law: Governance logic and public roles in the revised Law of the People's Republic of China on the Popularization of Science and Technology.

In this article, the 2002 and 2024 revised editions of the Law of the People's Republic of China on the Popularization of Science and Technology ("the Law") are examined to explore how the state shapes the science-society relationship. Relevant explanatory reports on the revision of the Law, policies, and central/local materials about the implementation of the first "National Science Popularization Month" are reviewed. Based on four dimensions (political-normative, organizational-responsibility, capacity-instrument, and oversight-accountability), the new Law is proposed to promote science popu

Factors affecting the implementation of employee whole health in the veterans health administration: a qualitative evaluation.

BACKGROUND: There is increasing recognition of the need to focus on the health and well-being of healthcare employees given high rates of burnout and turnover. Employee wellness programs are effective at addressing these issues; however, participation in these programs is often a challenge and requires large scale organizational transformation. The Veterans Health Administration (VA) has begun to roll out their own employee wellness program-Employee Whole Health (EWH)-focused on the holistic needs of all employees. This evaluation's goal was to use the Lean Enterprise Transformation (LET) mode

Investigating measurement invariance for multiple covariates in organizational research using exploratory factor analysis and confirmatory factor analysis trees.

Organizational research often deals with unobservable (latent) variables such as, for example, job satisfaction or leadership styles. When comparing these latent variables across groups, a comparability of the measurements is important-so-called measurement invariance (MI) considered a prerequisite. Common methodology to test whether MI holds or to explore noninvariance can only be used with established measurement models and specific hypotheses about potential violations of MI in mind. Therefore, exploratory factor analysis trees and confirmatory factor analysis trees have recently been devel

The Impact of the Introduction of the Breast Unit Model in a Northwestern Italian Region.

Breast cancer is the most common tumor in middle-aged and older women. In 2003, the European Parliament recommended to Member States that all women with breast cancer should be treated by a multidisciplinary team and that a network of certified breast centers be organized (the centers have been called Breast Units (BUs)). With the present study, we aim to explore the impact of the introduction of the BU organizational model in the Liguria region, Italy, through different outcome indicators. An explorative retrospective analysis was conducted through the period from 2013 to 2019 to assess the i

The marketing firm and consumer choice: implications of bilateral contingency for levels of analysis in organizational neuroscience.

The emergence of a conception of the marketing firm (Foxall, 1999a) conceived within behavioral psychology and based on a corresponding model of consumer choice, (Foxall, 1990/2004) permits an assessment of the levels of behavioral and organizational analysis amenable to neuroscientific examination. This paper explores the ways in which the bilateral contingencies that link the marketing firm with its consumerate allow appropriate levels of organizational neuroscientific analysis to be specified. Having described the concept of the marketing firm and the model of consumer behavior on which it

Great minds don't think alike: collaborative maternity care at San Francisco General Hospital.

For more than 30 years the collaboration between obstetrician-gynecologists and certified nurse-midwives at San Francisco General Hospital has led to the provision of high-quality care to women and families. This enduring partnership has been sustained by shared goals and values, most notably a commitment to excellence in both providing care to the underserved and training the next generation of physicians and nurse-midwives. Success has also depended on a mutual respect for differences that has enabled the collaborative to capitalize on the distinct expertise of each partner. The balance stru

Determinants of Implementation and Sustainability of Transitional Opioid Programs Within Rural and Urban Safety Net Hospitals.

BACKGROUND: Hospitalization presents a critical juncture to engage patients with opioid use disorder, but safety net hospitals are less likely to offer transitional opioid programs (TOPs), which initiate treatment and connect patients to care after discharge. This study explored determinants of TOP adoption in 4 diverse safety net health systems. METHODS: We interviewed key participants affiliated with safety net health systems and partner community-based organizations about TOPs within their organization. We used inductive coding informed by grounded theory to identify emerging themes relate

An exploration of the organizational performance of a care pathway applied to ambulatory surgery: a systems approach.

BACKGROUND: Ambulatory surgery is experiencing significant growth in France and OECD countries, accompanied by a profound transformation of care pathways. Existing performance evaluation approaches remain fragmented, limited to a single dimension, an isolated segment of the pathway, or the perspective of a single category of stakeholders. A systemic and multidimensional approach, grounded in real-world practices, is lacking. OBJECTIVE: As part of research aimed at developing a framework for evaluating the organizational performance of care pathways in ambulatory surgery, the primary objective

Bridging the Gap between Intent and Implementation: A Mixed-Methods Systematic Review of Barriers and Enablers of Shared Decision Making in Postmastectomy Breast Reconstruction.

BACKGROUND: Postmastectomy breast reconstruction (PMBR) is a preference-sensitive decision that requires alignment between clinical evidence and patient values. Shared decision making (SDM) supports patient-centered care, yet its systematic implementation in PMBR remains inconsistent. This mixed-methods systematic review synthesizes barriers and facilitators to SDM adoption and identifies strategies to improve decision quality, patient experience, and equity. METHODS: A systematic review was preregistered (OSF: https://osf.io/mu9hv) and conducted in accordance with PRISMA 2020 guidelines. Six
