Relationship Between Spinal Reflexes and Spastic Movement Disorders.

The central nervous system (CNS) generates various motor functions, in part by modulating the excitability of spinal reflex pathways. When a reflex becomes not modulable or its modulation becomes impaired due to spinal cord lesions or other CNS diseases, it could become a part of spastic movement disorders. For the CNS with or without chronic lesions, thus, reflexes can be used as tools to probe its state. Utilizing such features of spinal reflexes, it is possible to operantly condition a reflex to improve CNS activity and resulting motor function in spastic individuals with spinal cord lesion…

FDG-PET in Movement Disorders and Parkinsonian Syndromes.

FDG PET is a powerful adjunct for the differential diagnosis of parkinsonian syndromes because it reveals disease specific regional and network level metabolic signatures. Compared with dopamine transporter imaging, it better distinguishes Parkinson disease from progressive supranuclear palsy, corticobasal degeneration or corticobasal syndrome, multiple system atrophy, and Lewy body dementia such as Parkinson disease dementia and dementia with Lewy bodies. When combined with expert clinical assessment and multimodal imaging, FDG PET substantially improves diagnostic confidence, especially in e

Movement Disorders in Scrub Typhus: A Systematic Review.

BACKGROUND: Scrub typhus is a major rickettsial infection in Asia, occasionally presenting with movement disorders due to central nervous system involvement. This systematic review aimed to summarize their frequency, spectrum, diagnostic profile, treatment, and outcomes. METHODS: A comprehensive search of PubMed, Embase, Scopus, and Google Scholar was conducted up to August 2025. All case reports, case series, and cohort studies reporting movement disorders associated with scrub typhus were included. Data were extracted and analyzed descriptively. The protocol was prospectively registered wit…

Barriers to clinical genetic testing in movement disorders.

PURPOSE OF REVIEW: The number of known genetic movement disorders and potential treatments for these disorders have grown rapidly over the last few decades. Despite this, genetic testing for movement disorders remains relatively underutilized in clinical practice. In this review, we explore a number of barriers that prevent more routine and widespread use of genetic testing for movement disorders. RECENT FINDINGS: Cost and limited health insurance coverage as well as difficulty accessing genetic testing and counselling are major barriers to genetic testing and disproportionately affect low- a…

Advances in deep brain stimulation for movement disorders.

Deep brain stimulation (DBS) has transformed the management of medication-refractory movement disorders, including Parkinson's disease, essential tremor, and dystonia. This letter highlights recent clinical and technological advances that enhance precision, safety, and accessibility. Innovations such as directional leads, adaptive (closed-loop) stimulation, and telemedicine-enabled remote programming enable personalized therapy and improved symptom control. Country-specific experiences illustrate these trends: China has rapidly expanded DBS centers with remote programming, European centers are…

Sleep-related movement disorders in children: recent updates.

BACKGROUND: Sleep-related movement disorders (SRMDs), such as restless legs syndrome (RLS) and periodic limb movement disorder (PLMD), are common in pediatric sleep practice. There is increasing literature on RLS, PLMD, and a newly described sleep disorder called "restless sleep disorder (RSD)". We aimed to review and provide recent updates on SRMDs. DATA SOURCES: A comprehensive search for relevant English-language peer-reviewed publications focused on three common SRMDs, namely, RLS, PLMD and RSD, in a variety of indices in PubMed and SCOPUS. Both relevant databases and systematic reviews a…

Paroxysmal movement disorders: Paroxysmal dyskinesia and episodic ataxia.

Paroxysmal movement disorders have traditionally been classified into paroxysmal dyskinesia (PxD), which consists in attacks of involuntary movements (mainly dystonia and/or chorea) without loss of consciousness, and episodic ataxia (EA), which features spells of cerebellar dysfunction with or without interictal neurological manifestations. In this chapter, PxD will be discussed first according to the trigger-based classification, thus reviewing clinical, genetic, and molecular features of paroxysmal kinesigenic dyskinesia, paroxysmal nonkinesigenic dyskinesia, and paroxysmal exercise-induced …

Physiological Recordings of the Cerebellum in Movement Disorders.

The cerebellum plays an important role in movement disorders, specifically in symptoms of ataxia, tremor, and dystonia. Understanding the physiological signals of the cerebellum contributes to insights into the pathophysiology of these movement disorders and holds promise in advancing therapeutic development. Non-invasive techniques such as electroencephalogram and magnetoencephalogram can record neural signals with high temporal resolution at the millisecond level, which is uniquely suitable to interrogate cerebellar physiology. These techniques have recently been implemented to study cerebel…

Iron is probably as old as the universe itself and is essential for sustaining biological processes. The remarkable property of iron complex … vement disorder abnormalities. Finally, based on our understanding of the pathogenic role of iron, we have critically analyzed the limitations of chelation therapy in the current scenario and the various unmet needs that should be addressed for selecting t …

The Emerging Role of Phosphodiesterases in Movement Disorders.

Cyclic nucleotide phosphodiesterase (PDE) enzymes catalyze the hydrolysis and inactivation of the cyclic nucleotides cyclic adenosine monoph … ton's disease. The indication that PDEs are intimately involved in the pathophysiology of different movement disorders further stems from recent discoveries that mutations in genes encoding different PDEs, including PDE2A, PDE8B, and PDE10A, are responsible for rare forms of monogenic parkinsonism and chorea. We here aim to provide a translational overview of the pr …

Tics, tremors and other movement disorders in childhood.

Movement disorders presenting in childhood include tics, dystonia, chorea, tremor, stereotypy, myoclonus, and parkinsonism, each of which can be part of various clinical syndromes with distinct etiologies.  Some of these conditions are benign and require only reassurance; others are bothersome and require treatment, or may be clues that herald underlying pathology.  Answers lie in the inherent characteristics of the movements themselves, together with the clinical context provided in the history obtained by the examiner.  The aim of this review is to present an overview of the categories of in…

Treatable Hyperkinetic Movement Disorders Not to Be Missed.

Hyperkinetic movement disorders are characterized by the presence of abnormal involuntary movements, comprising most notably dystonia, chorea, myoclonus, and tremor. Possible causes are numerous, including autoimmune disorders, infections of the central nervous system, metabolic disturbances, genetic diseases, drug-related causes and functional disorders, making the diagnostic process difficult for clinicians. Some diagnoses may be delayed without serious consequences, but diagnosis delays may prove detrimental in treatable disorders, ranging from functional disabilities, as in dopa-responsive…

New-Onset Movement Disorders Associated with COVID-19.

INTRODUCTION: Movement disorders are increasingly described in hospitalized and milder cases of SARS-CoV-2 infection, despite a very low prevalence compared to the total patients. METHODS: We reviewed the scientific literature published in English, spanning from the initial descriptions of COVID-19 until January 25, 2021, in the PubMed/MEDLINE database. RESULTS: We identified 93 new-onset movement disorders cases (44 articles) from 200 papers screened in the database or reference lists. Myoclonus was present in 63.4% (n = 59), ataxia in 38.7% (n = 36), action/postural tremor in 10.8% (n = 10…
