Ankylosing spondylitis and multiple sclerosis.

Ankylosing Spondylitis can be associated with extra-articular involvement. Besides internal and ocular complications, neurological manifestations such as single root lesions, compression of the myelum or the cauda equina syndrome have also been described. We present a patient with ankylosing spondylitis who developed a monophasic myelopathy resembling multiple sclerosis. Literature data show no conclusive evidence for an increased association of ankylosing spondylitis and multiple sclerosis. However, a monophasic myelopathy may be a separate neurological manifestation associated with ankylosin…

Movement disorders in multiple sclerosis.

Movement disorders (MD), other than tremor, associated with multiple sclerosis (MS) occur infrequently. We report 14 new cases of whom nine had dystonia, three parkinsonism, and two had myoclonus. We also reviewed 135 such cases from the literature. From an analysis of the individual MDs and the site of the lesions described, we conclude that paroxysmal dystonias (tonic spasms), ballism/chorea, and palatal myoclonus can be caused by demyelinating lesions. Parkinsonism, dystonia, and other types of myoclonus, however, often appear to be coincidental.

Trigeminal neuralgia in multiple sclerosis.

Trigeminal neuralgia (TN) occurred in 35 patients (1.9%) from a large multiple sclerosis (MS) clinic population (N = 1,882). TN began on average 11.8 years after the first MS symptom but in five patients was the first symptom of MS, preceding the next MS symptom by 1 to 11 years. The onset of MS was later in the patients who had TN, and bilateral TN occurred more frequently than expected (in 14% of TN patients). The age at onset of TN was no younger than in idiopathic TN except when TN was the first MS symptom. Most patients had a good result from medical or surgical therapy.

Neuro-ophthalmology of multiple sclerosis.

Neuro-ophthalmic manifestations are important in multiple sclerosis. Optic neuritis is a frequent harbinger of the disease, and is characterized by subacute painful visual loss, with disproportionate loss of color and contrast sensitivity, central or cecocentral scotoma, and an afferent pupillary defect. Visual recovery is the rule, but tests have been developed to detect most cases of recovered optic neuritis. Intraocular inflammation has come to be recognized as a frequent accompaniment of multiple sclerosis. Because of the predilection of multiple sclerosis for the infratentorial white matt…

Natural history of multiple sclerosis.

Multiple sclerosis (MS) is the most common idiopathic inflammatory disease of the central nervous system. The distinction between MS and other benign or fulminant inflammatory demyelinating disorders is based on quantitative, rather than qualitative, differences in chronicity and severity. Primary progressive MS may differ from relapsing-remitting MS in MRI lesion frequency, immunogenetic profile, responsiveness to immunosuppressive treatment, and histology. In 60% of patients, MS begins as a relapsing-remitting disease and evolves secondarily into a progressive neurological illness. Life expe…

Interferon beta for multiple sclerosis.

OBJECTIVE: To introduce readers to the use of a new agent, interferon beta-1b (IFN beta ser), in the treatment of relapsing-remitting multiple sclerosis (RRMS). Therapeutic and economic issues surrounding IFN beta ser are discussed, as are its pharmacology, clinical efficacy, adverse effects, and dosage guidelines. DATA SOURCES: A MEDLINE search was used to identify pertinent literature, including clinical trials and reviews. STUDY SELECTIONS: All available trials were reviewed. DATA EXTRACTION: Since trials evaluating subcutaneously administered interferon beta are sparse, clinical trials …

Multiple sclerosis and blood transfusion.

It has recently been suggested that multiple sclerosis (MS) may be triggered by a retrovirally encoded superantigen. If true, then MS should be associated with risk factors that predispose to retroviral infection. Blood transfusions represent one such risk factor. 150 Scottish patients with 'clinically definite' or 'probable' MS and 150 age-/sex-matched neurological controls were questioned about blood transfusion. 7 (4.7%) of the patients and an equal number of controls had received a blood transfusion before the year of disease onset in the matched case (odds ratio 1.0, 95% C.I. 0.3-3.34). T…

Multiple sclerosis--making the diagnosis.

Although intensive research into all aspects of MS in the past 50 years has led to a great deal of information about pathological processes that may relate to its causation and expression, the disease is still an enigma--one of the greatest unsolved problems in medicine. Recent advances in neuroimaging and the ability to measure conduction in CNS pathways (evoked potentials) reveal subclinical effects, complementing clinical examination and frequently enabling a firm diagnosis in earlier phases of the disease than hitherto. In some situations earlier conclusive diagnosis facilitates management

[Benign forms of multiple sclerosis].

It is still difficult, in 1991, to evaluate precisely the position occupied by benign forms of multiple sclerosis (MS) in the natural history of the disease. The reality hidden behind the adjective "benign" varies from one author to another, and the estimated frequency of these forms is not the same when one refers to epidemiological studies on whole populations or to clinical studies in major hospitals. There is, however, no doubt that benign MS does exist, since about 10% of MS patients will suffer, throughout the whole duration of the disease (over 30 years), from no more than moderate disa…

[Steroid therapy in multiple sclerosis].

Since the 1950s steroids have been known to have a beneficial effect on recovery from acute exacerbations of multiple sclerosis. The main effect is a more rapid improvement after acute relapses due to resolution of edema in the central nervous system. Long-term steroid therapy showed no benefit in patients with a progressive course and involves dangerous side effects (Cushing's habitus, hyperglycemia, increased susceptibility to infections, peptic ulcers, osteoporosis, cataract). Intrathecal steroid administration offered no advantage over the conventional i.v. route. On the basis of a multice…

Pupillary function in multiple sclerosis.

Eighteen MS patients with visual acuity of 20/30 or better and without recent visual complaints, and 14 age- and sex-matched controls were studied by TV pupillography. The response amplitude and the maximum rate of pupillary constriction or dilation of the light, dark, accommodation and ciliospinal reflexes were unchanged. The latency of the pupillary light response (PLR), however, was prolonged and closely related to walking performance (R = 0.76; p less than 0.001). No relationship was established between PLR latency and previous history of optic neuritis or latency of the VEP. It is conclud

Visual function in multiple sclerosis.

126 patients with multiple sclerosis and normal visual acuity were submitted to a battery of tests of visual function. The investigation included visual evoked potential, contrast sensitivity by three methods, and a segment of the Farnsworth-Munsell 100 Hue test. 22 of the patients gave a history of unilateral acute optic neuritis and in these the abnormalities were greater in degree. Contrast sensitivity emerged as the most useful test and was abnormal in 92.2% of eyes. Visual evoked potential was delayed in 35.6% and colour vision was abnormal in less than a third. In general, contrast sensi…

Pain syndromes in multiple sclerosis.

To determine the prevalence and nature of pain in multiple sclerosis, we evaluated by questionnaire, interview, and chart review 159 patients residing in Middlesex County and followed in the MS Clinic at University Hospital, London, Ontario, Canada. Eighty-eight patients (55%) had either an acute or chronic pain syndrome at some time during their disease. Fifteen patients (9%) with acute pain syndromes had episodes of paroxysmal tic-like pain diagnosed in seven as trigeminal neuralgia. Chronic pain syndromes, present for a mean duration of 4.9 years, occurred in 76 patients (48%) and included …
