Exercise-induced mild rhabdomyolysis associated with oclacitinib in a dog.

A four-year-old Toy Poodle treated with oclacitinib for one year presented with transient myoglobinuria associated with elevated serum creatine kinase (CK) levels only after exercise. An association between myoglobinuria and oclacitinib was suspected. An exercise stress test with or without oclacitinib was repeated twice on different days. Myoglobinuria and increased serum CK levels occurred only after testing with oclacitinib. The dog was diagnosed with exercise-induced mild rhabdomyolysis, which was presumably provoked by oclacitinib based on the results of the exercise stress tests. Rhabdom…

Dermatomyositis presenting with rhabdomyolysis and acute renal failure; an uncommon manifestation.

Rhabdomyolysis and myoglobinuria are a rare complication of dermatomyositis. Such patient can land up in acute renal failure. Recognition of this fact has important therapeutic implications as patients require immunotherapy in addition to the symptomatic treatment for renal failure. We report a case of dermatomyositis with evidence of rhabdomyolysis and myoglobinuria presenting with acute renal failure. The patient responded dramatically to corticosteroid therapy.

Myoglobin determination by high-performance liquid chromatography.

Urine and serum myoglobin have been separated on an anion-exchange column, packed by the slurry technique. Urine or serum was injected directly into the column and eluted isocratically with Tris buffer. Freshly prepared myoglobin from human muscle gives two peaks in the chromatogram. Upon storage, one peak slowly decreases while the other increases in size and has the same capacity factor as horse myoglobin. In myoglobinuria, the latter peak is usually detected in urine, while in serum both peaks are detected. For routine assays, horse myoglobin is used as a standard. The minimum detectable le…

Excretion of myoglobin in urine after cardiac catheterisation.

Myoglobin levels were assayed in each urine specimen voided during 12 hours before and 48 hours after routine cardiac catheterisation in 146 patients using an indirect haemagglutination method detecting concentrations in excess of 0.015 mg/ml. Myoglobinuria was found in only one patient before but in 39 patients after cardiac catheterisation (27%), either in the first (34 patients) or the second (5 patients) post-catheterisation urine sample. Once detected, myoglobin was present in all subsequent urine specimens for the next 3 to 22 hours (mean 11.8 hours). The mean amount excreted +/- SE was …

Acute renal failure due to carnitine palmitoyltransferase deficiency.

A young, apparently healthy, soldier developed acute muscle weakness and rhabdomyolysis following prolonged exercise. The resultant myoglobinuria caused severe acute renal failure. Further investigation revealed the presence of carnitine palmitoyltransferase deficiency as the cause of the rhabdomyolysis. Renal function subsequently returned to normal. This rare metabolic disorder should be considered in cases of unexplained myoglobinuria and renal failure.

Acute myopathy associated with gasoline sniffing.

Acute myopathy and myoglobinuria with markedly elevated creatine kinase (CK) activity developed in an 18-year-old boy on two occasions after gasoline sniffing. No signs of CNS involvement were seen, and the symptoms receded in a few days. The amount of lead in blood and urine exceeded the reference value for subjects with occupational exposure. The component of gasoline that was responsible for the myopathy remains unknown. Acute myoglobinuria appears to be a rare complication of gasoline sniffing.

Rhabdomyolysis after cerebral perfusion pressure-guided management in severe head injury.

Intravenous infusion of norepinephrine is usually effective and safe to maintain adequate cerebral perfusion pressure for the management of posttraumatic intracranial hypertension. We report the case of a 17-year-old woman who suffered from traumatic intracranial bleeding and hypotension; she developed rhabdomyolysis, myoglobinuria and acute renal failure after receiving high dose norepinephrine postoperatively. Hemodialysis was begun 3 days after the onset of myoglobinuria when acute renal failure was noted, despite aggressive fluid supplementation and alkaline diuresis. After aggressive trea…

Atropine-induced rhabdomyolysis: an uncommon and potentially fatal adverse drug reaction.

A 25 year old woman received daily injection of 0.6 mg atropine for seven days by the intra-venous route. In addition to fever, incomprehensible speech and alteration of sensorium she developed albuminuria, hemoglobinuria and myoglobinuria. A diagnosis of atropine-induced rhabdomyolysis was made on the basis of clinical presentations and investigations. Although, datura poisoning and intoxication with wild mushrooms are known to result in rhabdomyolysis, this can be considered to be the first case of atropine-induced rhabdomyolysis and myoglobinuria.

Molecular analysis of LPIN1 in Jordanian patients with rhabdomyolysis.

Recessive mutations in LPIN1, which encodes a phosphatidate phosphatase enzyme, are a frequent cause of severe rhabdomyolysis in childhood. Hence, we sequenced the 19 coding exons of the gene in eight patients with recurrent hereditary myoglobinuria from four unrelated families in Jordan. The long-term goal is to facilitate molecular genetic diagnosis without the need for invasive procedures such as muscle biopsies. Three different mutations were detected, including the novel missense mutation c.2395G>C (Gly799Arg), which was found in two families. The two other mutations, c.2174G>A (Arg725His…

Streptokinase therapy: complications of intra-arterial use.

An inordinately high rate of renal complications was encountered among 35 patients treated for thrombotic or thromboembolic occlusion of the leg by injection of streptokinase into a bypass graft or native artery. Five patients demonstrated massive myoglobinuria following restoration of flow to ischemic and necrotic tissues; acute tubular necrosis developed in 2 of them, and 1 patient died as a result of renal shutdown, electrolyte imbalance, hypofibrinogenemia, and mediastinal and retroperitoneal hemorrhage. Massive myoglobinuria was also noted in 5 out of 13 patients with compartment syndrome…

Severe periodic febrile myalgia in infancy due to carnitine palmitoyltransferase deficiency.

A 7 1/2-yr-old girl suffered, since early infancy, severe recurrent myalgia during periodic attacks of fever, vomiting and pharyngitis. Neither myoglobinuria nor exercise-induced muscle pain was present. She was found to have carnitine palmitoyltransferase deficiency (CPTD) in leukocytes, fibroblasts and muscle. This case exemplifies the importance of looking for an associated metabolic etiology of recurrent febrile myalgia even in the absence of myoglobinuria.

The forearm ischaemic work test--hazardous to McArdle patients?

A 57-year-old patient suffering from late-onset McArdle's disease developed myoglobinaemia, massive myoglobinuria and marked serum creatine kinase elevation subsequent to a routinely performed forearm ischaemic work test. Twenty hours after the test, enhancement of 99mTc methylene-diphosphonate activity was demonstrated exclusively in the tested forearm. It is concluded that the forearm ischaemic work test is potentially hazardous to McArdle patients, as it might induce myoglobinuria sufficient to result in acute myoglobinuric renal failure.

Severe acute kidney injury owing to rhabdomyolysis and intravascular haemolysis in an 11-year-old child with G6PD deficiency.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency leading to acute intravascular haemolysis and acute kidney injury (AKI) is a known clinical presentation. However, there is a paucity of information regarding the occurrence of rhabdomyolysis and myoglobinuria in G6PD-deficient individuals, especially children. An 11-year-old south Indian Tamil girl presented with severe anaemia and anuric AKI following a short febrile illness. Investigations demonstrated evidence of intravascular haemolysis and rhabdomyolysis, and on histopathology myoglobin deposits (casts) were detected in the renal tubule…

Rhabdomyolysis in a patient with nonoliguric renal failure: similarities to the toxic-shock syndrome.

We have described a 13-year-old girl with idiopathic rhabdomyolysis, myoglobinuria, and nonoliguric renal failure. The biochemical abnormalities and enzyme and isoenzyme values and their interrelationship to the hosptial course are stressed. We believe our patient's condition had marked similarities to the toxic-shock syndrome. The case illustrates the importance of the rapid recognition of myoglobinuria so that its potentially fatal biochemical abnormalities may be expeditiously identified and treated.

Lactate dehydrogenase M-subunit deficiency: a new type of hereditary exertional myopathy.

Three families with a complete deficiency of the lactate dehydrogenase M subunit show exertional myoglobinuria. The response to ischemic forearm work is characteristic in these three families: an increase of venous lactate concentration after ischemic work was not observed and a marked increase of venous pyruvate was found. Glycolysis was markedly retarded in the patient's muscle in the glyceraldehyde 3-phosphate dehydrogenase (GA3PD) step. A significant increases in glyceraldehyde 3-phosphate, dihydroxyacetone phosphate and fructose 1,6-diphosphate were observed. The glycolysis retardation ma…

Rhabdomyolysis in children: a 3-year retrospective study.

The case reports of 4 pediatric patients illustrate the complex clinical scenarios in which childhood rhabdomyolysis/myoglobinuria occurs. Children ranged in age from 8-18 years. Presumed etiologies of rhabdomyolysis/myoglobinuria included Neisseria sepsis, exertion-related episodes, dialysis disequilibrium, and diabetic ketoacidosis. No child developed respiratory or renal failure. all children were discharged with normal muscle power, indicating the benign nature of this disease and the importance of aggressive management.

Rhabdomyolysis and acute kidney injury after acupuncture sessions.

Rhabdomyolysis is usually caused by muscle injury, drugs or alcohol and presents with muscle weakness and pain. It is characterized by rise in serum creatine kinase, aminotransferases and electrolytes as well as myoglobinuria. Myoglobinuria may cause acute kidney injury by direct proximal tubule cytotoxicity, renal vasoconstriction, intraluminal cast formation and distal tubule obstruction. Muscle pain and weakness as well as vascular injury have been reported after acupuncture. We report a case of severe rhabdomyolysis and acute kidney injury after acupuncture sessions.

Acute renal failure due to nontraumatic rhabdomyolysis.

Twenty-one patients developed acute renal failure in association with nontraumatic rhabdomyolysis and myoglobinuria. The illness followed an overdose of ethanol, heroin, or other depressant drug in 18 patients. Lethargy or coma was present in 17 patients and muscle swelling in 11. Evidence of rhabdomyolysis included markedly elevated creatine phosphokinase, myoglobinuria, and aldolase in blood. Initial biochemical findings were similar to those of acute renal failure due to other causes, but the abnormalities were exaggerated. There was a disproportionate rise in serum creatinine concentration…

Clinical spectrum of McArdle disease: three cases with unusual expression.

Three cases of myophosphorylase deficiency with unusual clinical expression are presented. The 1st had clinical characteristics suggesting a mild congenital myopathy, and the patient never experienced cramps or myalgias. The 2nd had a slowly progressive myopathy without cramps or myoglobinuria which was detected by chance. The 3rd presented with myoglobinuria and acute renal failure, unrelated to a triggering effort, and with permanent weakness and wasting. In all cases, muscle biopsy demonstrated a vacuolar myopathy with free glycogen increase and absence of myophosphorylase activity, confirm…

Legionnaires' disease with rhabdomyolysis and acute reversible myoglobinuric renal failure.

We have described a patient with rhabdomyolysis, myoglobinuria, and acute reversible renal failure. Serologic studies were diagnostic for legionnaires' disease and were negative for other viruses associated with rhabdomyolysis. Bacteriologic cultures were also negative, and other nontraumatic causes of rhabdomyolysis were ruled out. No other causes of acute reversible renal failure were found. With a clinical picture of pneumonia, rhabdomyolysis, myoglobinuria, and acute renal failure, legionnaires' disease should be considered in the differential diagnosis and appropriate antibiotic therapy i…
