BACKGROUND AND METHODS: Fourteen patients with caustic necrosis of the digestive tract extending beyond the pylorus were included in a multicenter retrospective study to define a surgical strategy. Twelve patients underwent esophagogastrectomy. Two patients had total gastrectomy without esophagectomy. In addition, all patients underwent duodenal stripping (n = 7) or pancreaticoduodenectomy (n = 7). Immediate biliopancreatic reconnection was performed in ten patients. We report the medical case of a 40-year-old man with a history of years of cocaine abuse. He was admitted to our hospital for a septic process and purulent rhinorrhea. The clinical examination and craniofacial computed tomography revealed severe necrosis of the nasal septum, maxillary sinus, ethmoidal sinus, sphenoidal sinus and soft palate. In our experience, this is unusual, even for longstanding cocaine abusers.

Case reports about vasopressin-induced cutaneous necrosis are not frequent. Here we report a further case, of which skin manifestations included not only mottling, cyanosis, ecchymosis, bullae and gangrene, but also amber-like change in focal areas. Besides, intermittent paling of the skin with or without deep pain sensation of the limbs over non-injection sites was observed that might be a warning sign of impending skin necrosis. Based on the literature about vasopressin-induced skin necrosis we discuss the possible role of coagulation enhancement of this molecule.

Paclitaxel is an antineoplastic agent derived from the bark of the Pacific yew tree that has activity against many tumors including breast and ovarian carcinomas. In the past, its extravasation quality has been considered to be a local irritant; however, recent reports suggest that the agent may be a vesicant. Foci of necrosis with formation of heterophilic granulomas in the medullary region of the thymus were found in apparently normal slaughtered crocodiles from 4 South African crocodile farms. The comparative pathology of these lesions is discussed. From these preliminary results it may be assumed that thymic necrosis occurs commonly in intensively reared slaughter crocodiles. The cause of this condition, however, remains obscure.

The cases of 80 patients with necrosis of the femoral head who underwent operative treatment in the period April 1980 to April 1988 are reported. Altogether, 102 operative procedures were carried out: 48 intertrochanteric osteotomies, 50 arthroplasties, 3 subchrondral bone graftings, and in one case drilling of the necrotic focus.
