Grade 3a Clavien grade complication following percutaneous nephrolithotomy. Related factors and review of the literature.

OBJECTIVE: We review the literature about bleeding complications of percutaneous nephrolithotomy (PCNL) and the application of the modified Clavien system classification. METHODS: We present a 38 year old man who underwent left PCNL with acute severe hematuria during the immediate postoperatory time. We review the literature and analyze the usefulness of the modified Clavien system to grade perioperative complications. RESULTS: Conservative management was insufficient so we had to perform arteriography and superselective embolization to solve the acute arterial bleeding. According to Clavien

Seoul National University Renal Stone Complexity Score for Predicting Stone-Free Rate after Percutaneous Nephrolithotomy.

OBJECTIVES: Currently, no standardized method is available to predict success rate after percutaneous nephrolithotomy. We devised and validated the Seoul National University Renal Stone Complexity (S-ReSC) scoring system for predicting the stone-free rate after single-tract percutaneous nephrolithotomy (sPCNL). PATIENTS AND METHODS: The data of 155 consecutive patients who underwent sPCNL were retrospectively analyzed. Preoperative computed tomography images were reviewed. The S-ReSC score was assigned from 1 to 9 based on the number of sites involved in the renal pelvis (#1), superior and in

Percutaneous nephrolithotomy for a partial staghorn calculus in a renal allograft following simultaneous pancreas-kidney transplantation.

Urolithiasis in renal allografts is rare, but large or complex stones present unique surgical challenges due to altered pelvic anatomy and proximity to vascular and visceral structures. Percutaneous nephrolithotomy (PCNL) is the gold-standard treatment for staghorn calculi but is technically demanding in transplanted kidneys. This complexity is amplified in recipients of simultaneous pancreas-kidney (SPK) transplants, where both grafts drain into the bladder. We describe a man in his 40s with type 1 diabetes and end-stage renal disease who underwent SPK transplantation in 2018. He presented wi

Retrograde pyeloperfusion: a novel technique to prevent stone fragment migration into the ureter during percutaneous nephrolithotomy.

To determine whether retrograde pyeloperfusion (RP) during percutaneous nephrolithotomy (PCNL) decreases antegrade ureteral stone fragment migration, normal, moderately, and severely hydronephrotic silicone kidney and ureter models were created using deidentified CT images. Two techniques to prevent fragment migration, ureteral access sheath (UAS) and RP via a ureteroscope, were compared to an empty ureter. Two grams of stone fragments were placed into the renal pelvis. Antegrade irrigation and suction were applied for 5 min. The weights of ureteral stone fragments were compared using the Krus

Querying the significance of patient position during computerized tomography on the reliability of pre-percutaneous nephrolithotomy planning.

BACKGROUND: Computerized tomography (CT) is considered indispensable in percutaneous nephrolithotomy (PCNL) planning. We aimed to define the reliability of pre-PCNL CT for planning renal access by assessing renal positional changes between supine and prone CTs. SUBJECTS AND METHODS: CT urographies (CTU) of 30 consecutive patients were reviewed for distances upper pole (UP)-diaphragm, UP-diaphragm attachment, renal pelvis (RP)-lateral body wall, RP- posterior body wall, and lower pole (LP)- anterior-superior iliac spine (ASIS). The posterior and lateral renal axes angles were also calculated.

[The analysis of perioperative complications of percutaneous nephrolithotomy in the treatment of nephrolithiasis with the use of modified Clavien-Dindo classification].

UNLABELLED: The improvement of surgical care requires transparent, consistent and accurate reports concerning surgical outcomes which are assessed and documented in a standardized manner. No consensus has yet been reached as to how to define and assess postoperative complications with regard to the specificity of urological procedures. Therefore, the comparison of data from different centres is difficult. The modified Clavien-Dindo classification allows for a more uniform analysis of surgical complications. This study analyses the occurrence of perioperative complications after percutaneous ne
