Nephron-sparing surgery in the treatment of pediatric renal cell carcinoma associated with Xp11.2 translocation/TFE3 gene fusions.

PURPOSE: To investigate the safety and efficacy of nephron-sparing surgery (NSS) in the treatment of pediatric Xp11.2 translocation renal cell carcinoma (RCC). METHODS: Clinical characteristics of 9 RCC children (7 males and 2 females) with Xp11.2 translocation who received NSS between January 1973 and December 2015 were retrospectively analyzed. The mean age was 7.8years (range: 4.5-13.5years). Xp11.2 translocation RCC was found in the left side in 4 patients and right in 5. 3 tumors were located in the upper pole of the kidney, 1 in the middle dorsal, 1 in the middle ventral and 4 in the lo

[Multi-slice CT in the planning of nephron-sparing interventions for renal cell carcinoma: Prospective study correlated with histopathology].

OBJECTIVE: For nephron sparing surgery of renal cell carcinoma knowledge of the intrarenal extension of the tumor and infiltration of the renal pelvis, the intrarenal vessels and the perinephric fat is essential. This question is much more difficult to answer compared to the description of staging parameters. Aim of this study is to evaluate the value of multi-slice CT in planning of nephron sparing surgery. MATERIAL AND METHODS: In a prospective study 46 patients with suspected renal cell carcinoma underwent a triphasic multi-slice CT (unenhanced, delay 30 s and 120 s). A reconstructed slice

HIF activation identifies early lesions in VHL kidneys: evidence for site-specific tumor suppressor function in the nephron.

Mutations in the von Hippel-Lindau (VHL) gene are associated with hereditary and sporadic clear cell renal carcinoma. VHL acts in a ubiquitin ligase complex regulating hypoxia-inducible factor-1 (HIF-1), but the link between this function and cancer development is unclear. Here we show that in the kidneys of patients with VHL disease, HIF activation is an early event occurring in morphologically normal single cells within the renal tubules. In comparison, dysplastic lesions, cystic lesions, and tumors showed evidence of additional mechanisms that amplify HIF activation. Detection of cells with

Fibroepithelial polyp of the renal pelvis: nephron-sparing surgery after false-positive biopsy for transitional cell carcinoma.

Benign fibroepithelial polyps of the renal pelvis are extremely rare, with only 23 cases previously reported. The diagnosis is usually made following nephrectomy or nephroureterectomy for an assumed malignancy. This case involves a 66-year-old woman referred with presumed biopsy-proven transitional cell carcinoma of the renal pelvis. Radiographic findings were suggestive of a benign lesion. Pyelotomy and frozen section confirmed these suspicions. The polyp was excised and the kidney spared. The diagnosis and management of fibroepithelial polyps are discussed and the literature reviewed.

Beyond traditional nephrometry: a 3D quantitative framework for predicting pentafecta and evaluating mediation effects in nephron-sparing surgery.

PURPOSE: To establish a three-dimensional (3D) quantitative framework for predicting pentafecta achievement and to explore the underlying compensatory associations in nephron-sparing surgery (NSS). METHODS: We retrospectively analyzed 375 patients undergoing NSS. Preoperative 3D morphometric metrics-specifically contact surface area (CSA) and retained blood-perfused renal volume (RBRV)-were extracted. Pentafecta was strictly defined by optimal perioperative success and 1-year functional preservation (eGFR loss < 10% without chronic kidney disease upstaging). Multivariable logistic regression,

Bilateral Wilms Tumor: Multi-institutional Analysis of Nephron-Sparing Surgery at Centers with Focused Wilms Tumor Expertise.

PURPOSE: The proportion of children who undergo bilateral nephron-sparing surgery (BNSS) for bilateral Wilms tumor (BWT) is variably reported. We evaluated the frequency of BNSS versus unilateral radical nephrectomy with contralateral nephron-sparing surgery (NSS) at institutions with focused Wilms tumor expertise during the Children's Oncology Group (COG) AREN0534 era. We hypothesized that BNSS would be performed more frequently than the 35% frequency reported on AREN0534. METHODS: We identified the top 10 enrolling institutions (all renal tumors) on the COG Renal Tumor Banking Study (AREN03

Compensatory nephron hypertrophy: mechanisms, pathophysiology dynamics, and clinical implications in acute kidney injury and chronic kidney disease.

Compensatory nephron hypertrophy (CNH) is a critical adaptive response to nephron loss, observed in conditions such as living kidney donation, congenital solitary kidney, acute kidney injury (AKI), and chronic kidney disease (CKD). This review integrates current insights on the molecular mechanisms, pathophysiological consequences, and clinical significance of CNH, highlighting its dual role as both a protective and maladaptive process. At the cellular level, CNH is driven by hypertrophy rather than proliferation, mediated by the mTOR signaling pathway, with predominant effects in glomeruli an

Infantile Homocystinuric Urolithiasis With Staghorn Calculus: A Case Report of Nephron-Sparing Surgical Salvage in an Intraparenchymal Renal Pelvis.

Pediatric urolithiasis, though uncommon, carries significant morbidity when driven by underlying metabolic disorders. Classical homocystinuria, caused by cystathionine beta-synthase (CBS) deficiency, is a rare autosomal recessive inborn error of sulfur amino acid metabolism. While ectopia lentis, skeletal anomalies, and thromboembolic events define its classical triad, urolithiasis presenting within the first year of life remains exceptionally rare in surgical literature. A one-year-old male, born to consanguineous parents with known homocystinuria, presented with gravel-like urinary deposits

Kidney volume-to-birth weight ratio as an estimate of nephron endowment in extremely low birth weight preterm infants.

In humans, nephrogenesis is completed by 32-36 weeks gestation, with a highly variable total number of nephrons, ranging from 200,000 to over 2 million. Premature birth disrupts the development and maturation of the kidneys, leading to a reduction in the final number of nephrons. Due to significant genetic variability in the number of nephrons among individuals, it is crucial to identify premature infants with fewer nephrons at birth as early as possible. These infants are more susceptible to developing renal failure with advancing age compared to those with a higher nephron endowment. Bedside…

Case report: increased single-nephron estimated glomerular filtration rate in an adult patient with low birth weight.

BACKGROUND: Low birth weight (LBW) is associated with end-stage kidney disease and hypertension and is considered to be a surrogate marker of low nephron number. Low nephron number is hypothesized to contribute to glomerular hyperfiltration that may cause kidney injury; however, this is not yet proven. Until now, the hyperfiltration in LBW patients has not been shown directly yet. CASE PRESENTATION: A 23-years-old female was referred with the persistent proteinuria and decreased renal function (estimated glomerular filtration rate by cystatin C (eGFRcys); 41.86 ml/min). She was a premature ba

Nephron generation in kidney cortices through injection of pretreated mesenchymal stem cell-differentiated tubular epithelial cells.

Transplantation of artificially treated metanephroi or pluripotent stem cell-injected blastocyst-derived whole kidneys will be established in the near future as a useful therapeutic method for renal failure. We have attempted in vivo nephron generation for kidney repair by exploiting cellular interactions via conditioned media (CMs). In a previous report, we showed stimulative cross-talks between vascular endothelial cells (VECs) and tubular epithelial cells (TECs) on cell proliferation and morphological changes, the differentiation of mesenchymal stem cells (MSCs) into TECs by TEC-CM, and nep

Active Surveillance Versus Nephron-Sparing Surgery for a Bosniak IIF or III Renal Cyst: A Cost-Effectiveness Analysis.

OBJECTIVE: The objective of our study was to evaluate the cost-effectiveness of active surveillance (AS) versus nephron-sparing surgery (NSS) in patients with a Bosniak IIF or III renal cyst. MATERIALS AND METHODS: Markov models were developed to estimate life expectancy and lifetime costs for 60-year-old patients with a Bosniak IIF or III renal cyst (the reference cases) managed by AS versus NSS. The models incorporated the malignancy rates, reclassification rates during follow-up, treatment effectiveness, complications and costs, and short- and long-term outcomes. An incremental cost-effect

Nephron-Specific Deletion of Circadian Clock Gene Bmal1 Alters the Plasma and Renal Metabolome and Impairs Drug Disposition.

The circadian clock controls a wide variety of metabolic and homeostatic processes in a number of tissues, including the kidney. However, the role of the renal circadian clocks remains largely unknown. To address this question, we performed a combined functional, transcriptomic, and metabolomic analysis in mice with inducible conditional knockout (cKO) of BMAL1, which is critically involved in the circadian clock system, in renal tubular cells (Bmal1lox/lox/Pax8-rtTA/LC1 mice). Induction of cKO in adult mice did not produce obvious abnormalities in renal sodium, potassium, or water handling. D

Nephron-sparing surgery for localized renal cell carcinoma with a normal contralateral kidney: a European three-center experience.

OBJECTIVES: To assess the long-term effectiveness and safety of nephron-sparing surgery for the treatment of localized renal cell carcinoma with a normal contralateral kidney. METHODS: Since 1973, 118 patients have undergone nephron-sparing surgery for renal cell carcinoma on an elective basis at our institutions. The vast majority of these tumors were incidental findings, with a mean tumor diameter of 3.35 cm (range 0.7 to 5.6). The median follow-up was 8.5 years (range 0.5 to 18), and of those patients alive, 27 (28%) were followed up for more than 10 years. RESULTS: The pathologic stage w

From segmental glomerulosclerosis to total nephron degeneration and interstitial fibrosis: a histopathological study in rat models and human glomerulopathies.

BACKGROUND: Focal segmental glomerulosclerosis (FSGS) is consistently associated with tubular degeneration and interstitial fibrosis, altogether, accounting for the progressive decline in renal function. The mechanisms which link glomerular injury to tubulo-interstitial fibrosis are controversial. The present study describes the step-by-step sequence of histopathological events, i.e. the evolution of the injury from the initial lesion in the glomerulus to total nephron destruction. METHODS: The investigation was performed in male hypertensive Fawn-hooded rats (6-, 9-, and 12-month-old) and 14

Successful bilateral nephron-sparing surgery for recurrent stage V Wilms tumor with hepatic capsule involvement: a case report.

INTRODUCTION: Wilms tumor is the Most common renal malignancy in children, but bilateral Wilms tumor (BWT) is rare and poses competing priorities for cure and renal preservation. Neoadjuvant chemotherapy followed by nephron-sparing surgery is standard, yet optimal timing when resistance emerges remains challenging. This case describes advanced BWT with secondary chemotherapy resistance and hepatic capsular involvement, successfully managed with timely, aggressive bilateral nephron-sparing surgery and liver capsule resection. Renal proteomics of male offspring exposed to maternal protein restriction: molecular, epigenetic, and nephron-specific signatures of metabolic programming.

Chronic kidney disease is an increasing global public health concern, and the Developmental Origins of Health and Disease (DOHaD) concept proposes that adverse conditions during critical developmental windows predispose offspring to chronic disorders later in life. Maternal protein restriction (MPR), a well-established experimental model reflecting food insecurity, has been shown to impair nephrogenesis and promote long-term renal dysfunction. In this study, we investigated renal metabolic-epigenetic programming induced by gestational and lactational MPR in post-weaning male rats using a globa

The safety of percutaneous renal biopsy for acute kidney injury in metastatic renal cell cancer patients with reduced nephron mass.

BACKGROUND: Percutaneous renal biopsy (PRB) provides valuable information to guide treatment decisions in patients with metastatic renal cell carcinoma (mRCC) who develop acute kidney injury (AKI) after systemic anticancer therapy (SACT). The rising incidence of renal cell carcinoma (RCC) and the substantial impact of SACT on overall survival suggest a higher prevalence of RCC patients with reduced nephron mass and a solitary kidney (SK) requiring PRB for AKI. However, safety data on SK biopsies are scarce, and the potential for dialysis-requiring complications may deter clinicians. METHODS:
