A quantitative study of glomeruli in idiopathic nephrosis with minimal or no glomerular lesions.

A quantitative investigation was performed of glomeruli in renal ibiopsies from patients with nephrotic syndrome or persisting proteinuria associated with normal light microscopic findings or slight changes not considered significant ("minimal change disease", lipoid nephrosis). The mesangial area was widened (+ 26 per cent, p less than 0.05) and there was an increase in the mean of the total number of cellular nuclei per glomerulus, the number of mesangial (+ 16 per cent) and edothelial nuclei (+ 23 per cent), but of these only endothelial hypercellularity was significant (p less than 0.025).

[Histological classification of chronic glomerular diseases].

The various clinicopathological forms of primary glomerulonephritis (GN) in humans include lipoid nephrosis with its histological variants (minimal change disease, focal segmental glomerular sclerosis, diffuse mesangial proliferation, collapsing glomerulopathy), membranous GN, IgA nephropathy, membranoproliferative GN (type I and type II) and crescentic GN. Each of these entities is characterised by pathological, clinical, and immunological features. Although the precise aetiology of primary GN remains unknown, progress has been recently achieved in the understanding of the mechanisms of the g

Inhibition of lymphocyte blastogenesis by plasma of patients with minimal-change nephrotic syndrome.

Plasma from 7 patients with minimal-change nephrotic syndrome (M.C.N.S.) (lipoid nephrosis) confirmed by renal biopsy inhibited the blastogenic response of lymphocytes to the mitogen phytohaemagglutinin (P.H.A.) and to allogeneic lymphocytes in mixed lymphocyte culture. Plasma from 14 patients with nephrotic syndrome due to other glomerular diseases did not have this effect. 5 of the 7 patients with M.C.N.S. achieved remission; 4 with prednisone and 1 spontaneously. While in remission, plasma from these 5 patients no longer inhibited the response of lymphocytes to P.H.A. These findings suggest

[Pathomorphology of kidney lesions induced by non-steroidal anti-inflammatory drugs].

Pathomorphological findings on nephrobiopsies from two groups of patients with NSAID-induced kidney lesions are provided. In the first group (9 patients) the disease developed in patients of different age after short-term use of NSAID and was clinically manifested by acute renal failure with extrarenal allergic symptoms (skin, rash, eosinophilia). Morphological features were acute tubulo-interstitial nephritis without glomerular lesions. In the second group (23 patients) the disease developed after longterm use of NSAID in older patients with a compromised renal blood circulation presenting cl

Normal growth of nephrotic children during long-term alternate-day prednisone therapy.

Statural growth has been evaluated in 20 prepubertal nephrotic children who received alternate-day prednisone therapy for a year at least. Bone age was assessed in 16 of these children after 1-4 years of therapy. During the follow-up 12 children showed variations in height standard deviation score (SDs) below 0.5, 7 gained more than 0.5 SDs and 1 lost 0.5 SDs. Bone age fell within the normal range for chronological age in all the children studied. On the while alternate-day prednisone therapy does not affect statural growth and bone maturation of children with lipoid nephrosis.

Nephrotic syndrome in two patients with cured Hodgkin's disease.

Two cases of lipoid nephrosis (minimal change glomerulonephritis) in patients cured of Hodgkin's disease are reported and the literature is reviewed. Cases reported to date have shown a close temporal relationship between this renal lesion and the presence of Hodgkin's disease. The patients reported are 11 and 9 years without evidence of active malignancy after successful treatment for Hodgkin's disease. Each had abnormal immunologic parameters, depressed T4 (helper) cells and increased T8 (suppressor) cells, which may predispose to the development of the nephrotic syndrome. However, the adven

Renin-induced massive proteinuria in man.

A 30 year old man developed renovascular hypertension and extreme elevation of plasma renin activity. Daily proteinuria ranged from 13 to 31 g. There were no criteria for the diagnosis of malignant hypertension. A primary glomerulopathy was excluded by a basically normal renal biopsy from the unprotected kidney. On electron microscopy the epithelial cell foot processes were not fused, thus ruling out simultaneous lipoid nephrosis. The source of renin was removed by means of a left nephrectomy. Following the procedure the patient became normotensive, the renin values normalized and the proteinu

[The role of immediate-type allergic reactions in the pathogenesis of the nephrotic syndrome].

The study was made of 52 patients with various morphological forms of glomerulonephritis and history of allergy versus 22 healthy subjects. Mean age of the patients was 28.5 +/- 7.2 years. High IgE levels occurred in 37 cases (71.1%). Most of the patients had lipoid nephrosis. Normal levels of IgE were more frequently associated with steroid-resistant nephrotic syndrome characterized by a progressive decline of renal function and severe damage to the glomeruli. A correlation existed between high IgE level and activation of antistreptococcal immunity. Two cases are reported of nephrotic syndrom

Drug-induced interstitial nephritis. Coexistence with glomerular disease.

Drug-induced interstitial nephritis is being recognized with increasing frequency. Pharmacologic agents responsible for inducing this entity include antibiotics, diuretics, and nonsteroidal anti-inflammatory drugs. We recently examined five patients with glomerular disease and drug-induced interstitial nephritis. In three patients prior biopsy specimens documented their glomerular disease (membranous nephropathy, crescentic glomerulonephritis, and presumptive lipoid nephrosis). A second biopsy specimen showed acute interstitial nephritis and the glomerular lesion. Two additional patients had s
